Book excerpt: Improving and Accelerating Therapeutic Development for Nervous System Disorders is the summary of a workshop convened by the IOM Forum on Neuroscience and Nervous System Disorders to examine opportunities to accelerate early phases of drug development for nervous system drug discovery. Workshop participants discussed challenges in neuroscience research for enabling faster entry of potential treatments into first-in-human trials, explored how new and emerging tools and technologies may improve the efficiency of research, and considered mechanisms to facilitate a more effective and efficient development pipeline. There are several challenges to the current drug development pipeline for nervous system disorders. The fundamental etiology and pathophysiology of many nervous system disorders are unknown and the brain is inaccessible to study, making it difficult to develop accurate models. Patient heterogeneity is high, disease pathology can occur years to decades before becoming clinically apparent, and diagnostic and treatment biomarkers are lacking. In addition, the lack of validated targets, limitations related to the predictive validity of animal models - the extent to which the model predicts clinical efficacy - and regulatory barriers can also impede translation and drug development for nervous system disorders. Improving and Accelerating Therapeutic Development for Nervous System Disorders identifies avenues for moving directly from cellular models to human trials, minimizing the need for animal models to test efficacy, and discusses the potential benefits and risks of such an approach. This report is a timely discussion of opportunities to improve early drug development with a focus toward preclinical trials.

Available in PDF, EPUB and Kindle. Book excerpt: This book describes, with references to key source materials, the background to, and conduct of, the principal nonclinical studies that are central to drug development. The chapters provide an understanding of the key components of the preclinical phase of drug development with a hands-on description, with core chapters addressing study conduct, types, and reporting. As such, it is a practical guide through toxicology testing and an up-to-date reference on current issues, new developments, and future directions in toxicology. Opening with a practical description of toxicology and its role in the development of pharmaceuticals, the book proceeds to detail international regulations (including the impact of the new REACH standards for chemical safety), interdisciplinary interactions among scientists in drug development, steps in toxicity testing, and risk management. Further, the book covers the methods of genetic toxicology (assays, genomics, in vivo screening) as a complement to “traditional” toxicology in the risk assessment and risk management of pharmaceuticals.

The book is an excellent reference for all professionals involved in the set up, performance and interpretation of results for drug testing programs using biological fluids (especially urine). U.S. and European perspectives are presented in relation to workplace testing. Organizational aspects for reliability of drug testing include topics ranging from sample collection, chain of custody, and laboratory strategies to legal and regulatory aspects. Critical reviews of analytical methodology involve descriptions and critical issues for the major presumptive and confirmatory techniques, including immunological and gas chromatographic-mass spectrometric methodologies. The book's interpretation of results takes into account the metabolic, pharmacokinetic, pharmacodynamic and clinical aspects. The final chapters of the book include topics addressing aspects for potential international harmonization.

Book excerpt: This book gives an overview of therapeutic drug monitoring (TDM) and its clinical application. It also highlights recent advances in toxicological studies, as they relate to therapeutic drug monitoring. This is one of the few books available on the market that covers TDM. Therapeutic drug monitoring (TDM) is a clinical decision-making tool that enables dosage regimen adjustments based on clinical and laboratory measurements. TDM not only involves the measuring of drug concentrations but also interpretation of the results. There is a strong correlation between drug concentrations in body fluids and outcome than between dose and outcome. The chapters include coverage of analytical techniques, pharmacokinetics, therapeutic indices, artificial intelligence and recent advances in toxicological studies. The book fills a gap in published literature and provides reliable information on; Analytical techniques in TDM and clinical toxicology TDM and pharmacokinetic studies TDM of drugs with narrow therapeutic indices Artificial intelligence in TDM and clinical toxicology Future directions and challenges

Available in PDF, EPUB and Kindle. Book excerpt: With the rapid spread of drug use and abuse in this country over the last decade, events relating to drugs have become increasingly prominent in the court of law as well as in society in general. It is anticipated that throughout the 1980s this problem will continue to be one of the major social problems in the United States. With the complexity of the Federal and state laws and their interrelationships, and with the increasing demand for the expert chemist or toxicologist, or both, in the courtroom, more education for these people relating to drugs must be implemented. The prosecuting and defense attorneys are also in need of accurate toxicological information to adequately handle cases in which drugs playa role. This book is intended to be a practical as well as an educational book-a resource tool for the active forensic chemist, as well as for the student. It is with the forensic chemist in mind that the material on controlled substances and substances found in cases of drug abuse has been arranged in a systematic order for quick and easy reference. This section includes synonyms, pharma ceutical preparations, biochemistry and/or pharmacognosy, toxicology and/or pharmacology, and how the substance is controlled under Federal law. Under the concept of scientific consideration as compared with the Federal law, this book enables one to deal with some of the major conflicts in the American legal system concerning scientific evidence and how it is presented in a court of law.

Book excerpt: This excellent volume was designed and edited with two major ideas in mind: firstly, the field of clinical toxicology is changing and an acknowledgement of these changes is warranted; secondly, no comprehensive compilation of recently published case reports of, and clinical studies on, human poisonings is available, which is in sharp contrast to the closely related field of drug-induced side-effects. The book focusses on issues of recent concern, or issues poorly documented in the past. It is important that clinical toxicologists gain a better knowledge of all the available techniques of toxicological analysis. A better understanding of the way a sound interpretation of results should be conducted for the benefit of the patient's management, and a comprehensive set of data on the kinetics of the most common pharmaceutical drugs and many chemicals is required. Human Toxicology is a timely reference work which will be welcomed by a broad audience of toxicology professionals.
