All 90 persons on board were killed in the accident. The investigation concluded that the probable causes of the accident were pilot errors due to loss of situational awareness. Ethiopian Airlines refutes this conclusion. Other factors that could have lead to probable causes are the increased workload and stress levels that have most likely led to the captain reaching a situation of loss of situational awareness similar to a subtle incapacitation and the F/O failure to recognize it or to intervene accordingly. Ethiopian Airlines refutes the investigation. According to the airline the final report was biased, lacking evidence, incomplete and did not present the full account of the accident.

Book excerpt: On December 18, 1867, the Buffalo and Erie Railroad’s eastbound New York Express derailed as it approached the high truss bridge over Big Sister Creek, just east of the small settlement of Angola, New York, on the shores of Lake Erie. The last two cars of the express train were pitched completely off the tracks and plummeted into the creek bed below. When they struck bottom, one of the wrecked cars was immediately engulfed in flames as the heating stoves in the coach spilled out coals and ignited its wooden timbers. The other car was badly smashed. About fifty people died at the bottom of the gorge or shortly thereafter, and dozens more were injured. Rescuers from the small rural community responded with haste, but there was almost nothing they could do but listen to the cries of the dying—and carry away the dead and injured thrown clear of the fiery wreck. The National Transportation Safety Board determines that the probable cause of the accident was an explosion of the center wing fuel tank, resulting from ignition of the flammable fuel/air mixture in the tank. Contributing factors to the accident were the design and certification concept that fuel tank explosions could be prevented solely by precluding all ignition sources and the design and certification of the Boeing 747. The safety issues in this report focus on fuel tank flammability.

In 1940, the Royal Air Force Purchasing Commission acquired more than 100 Brewster B-339 Buffalo fighter planes from the US. But when the aircraft were deemed below par for service in the UK, the vast majority were diverted for use in the Far East, where it was believed they would be superior to any Japanese aircraft encountered should hostilities break out there. This assessment was to prove tragically mistaken. When war erupted in the Pacific, the Japanese Air Forces proved vastly superior in nearly all aspects. Compounding their advantage was the fact that many of the Japanese fighter pilots were veterans of the war against China. By contrast, most of the young British, New Zealand, and Australian pilots who flew the Buffalo on operations in Malaya and in Singapore were little more than trainees. Yet these fledgling fighter pilots achieved much greater success than could have been anticipated.
