Book excerpt: OBJECTIF: Etudier l'effet de l'implantation d'une lentille intraoculaire (IOL) torique pour la correction de l'astigmatisme cornéen préexistant dans la chirurgie de la cataracte, et le retentissement visuel, réfractif et abérrométrique de son désaxement. MATERIEL ET METHODE: Cette étude prospective observationnelle a inclus 30 yeux de 20 patients consécutifs, avec plus de 1,5 dioptrie d'astigmatisme cornéen, candidats à une chirurgie de la cataracte. L'implantation d'une lentille de type Acrysof toric (Alcon) ou AT TORBI 709M (Cari Zeiss) a été réalisée après phacoémulsification. L'acuité visuelle sans correction (AVSC), la meilleure acuité visuelle corrigée (MAVC), la sphère réfractive, le cylindre réfractif et kératométrique, l'axe de la IOL et les aberrations de haut degré ont été mesurés. RESULTATS: À un mois postopératoire, l'AVSC était de 0,15 logMAR avec une AVSC inférieure à 0,3 logMAR dans 96,6% et inférieure à 0,1 logMAR dans 46,6%. Le cylindre réfractif moyen a diminué significativement après la chirurgie de 2,93 D ± 1,19 à 0,95 D ± 0,53 (Pc.OOOl) avec un cylindre réfractif inférieur ou égal à 0,5 D dans 50% des cas. L'axe moyen de rotation de la IOL était de 6,86 ± 6,12 degrés. Il a été retrouvé une corrélation linéaire positive significative entre le désaxement de la IOL torique et l'AVSC (P=.0149, i?2=0,217) ainsi que le cylindre résiduel réfractif (P=.012, i?2=0,213). Il n'a pas été retrouvé de corrélation entre le désaxement et les aberrations optiques de haut degré, totales et individuelles. CONCLUSION: L'implantation torique en chambre postérieure dans le cadre de la chirurgie de la cataracte est une technique efficace et sûre pour corriger l'astigmatisme cornéen préexistant. Le mauvais alignement de la IOL torique induit une diminution des résultats visuels et réfractifs, sans altération de la qualité optique visuelle.
