Available in PDF, EPUB and Kindle. Book excerpt: Coronary artery disease (CAD) is a common cardiovascular disease. It is of various types, such as stable and unstable angina, myocardial infarction and cardiac death. It may lead to complications such as abnormal heartbeat and heart failure. The risk factors that contribute to the incidence of coronary artery diseases include smoking, diabetes, high blood pressure, obesity, lack of exercise, blood cholesterol, poor diet, alcohol consumption, etc. Individuals may have a predisposition to coronary artery disease depending on their genetics. MRI, exercise ECG, echocardiology, intravascular ultrasound, etc. are used for determining coronary artery disease. Stress echocardiography is used for diagnosing obstructive coronary artery disease. It can be treated through lifestyle changes, coronary interventions such as coronary stent, angioplasty, coronary artery bypass grafting, using drugs such as beta-blockers, nitroglycerin, cholesterol lowering medications, etc. Research in this domain is focused on stem cell therapies and new angiogenic treatment modalities. This book is compiled in such a manner, that it will provide in-depth knowledge about coronary artery disease. From theories to research to practical applications, case studies related to all contemporary topics of relevance to this field have been included herein. The topics covered herein offer the readers new insights in cardiology.

Book excerpt: Advanced ischemic heart disease is fast becoming one of the most challenging problems facing the modern cardiovascular physician and current established therapies often fail to adequately address this population of patients. As therapy of heart disease evolves, we need to address the challenging questions posed by this clinical problem. Treatment of Advanced Ischemic Heart Disease brings together the most recent advances in surgical techniques, protection of the heart and postoperative care, to aid the study and treatment of these patients who deserve the best that modern medicine can provide them. The diagnosis and treatment of coronary artery disease and heart failure has now been standardized to the point that there are very well established guidelines promoted by the American Heart Association and The American College of Cardiology. This text will pick up where the guidelines leave off and address the real challenge of patients with advanced ischemic heart disease. Though therapy is clearly challenging, new advances are making effective therapy of these patients truly possible. This text embodies that notion. Finally, there is much still to learn about the basic mechanisms of the progression of heart failure due to coronary artery disease and how best to assess these therapeutic options. The text will address these with a focus on treatment so that it may be a practical reference for the practicing clinician as well as the basic or clinical researcher and student.

Book excerpt: Landmark Papers in Cardiovascular Medicine provides a thorough and wide-ranging analysis of core examples of novel research, clinical trials and seminal papers published in the medical literature that have paved the way for breakthroughs in the management of the entire spectrum of cardiovascular disease. These papers may have produced positive, negative or equivocal findings but are regarded by the experts as having either stimulated a paradigm shift in therapeutic strategy or been the catalyst for new and improved methods of research, diagnosis or drug development. Our aim is to provide both a benchmark and inspiration for future work in the field of cardiovascular medicine and also to give the reader an insight into the mechanics and infrastructure of how high-quality evidence-based medicine has been produced. Each trial summary is punctuated by sections on 'strengths and limitations', 'impact on the field', 'learning points' and 'further reading' suggestions that allow for a completely holistic analysis of the data. The experts also give their views on what research is currently underway, their hopes for the future and what advances they predict will occur in each subspecialty field of cardiovascular medicine, making this book essential reading for all those individuals with an interest in the field.

Book excerpt: Few advances in cardiovascular medicine have so captured the inter est of physicians and the public alike as has coronary artery bypass surgery. Cardiologists who lived through the era of false hopes and frustrations of previous" operations" for coronary artery disease can fully appreciate what this major advance offers to their patients. The number of papers on this topic presented at national meetings attest to its popularity, but also make it increasingly difficult for anyone to assimilate all the data. Practitioners who must consider coronary artery bypass surgery for their patients may thus have difficulty in coming to some rational decision regarding the indications for the operation and its consequences. Depending on the "authority" that one reads, there is still some controversy as to which patients are candidates for coronary artery surgery. This monograph on the prac tice of coronary artery bypass surgery, therefore, comes as a welcome boon to cardiologists, surgeons, internists, and general practitioners who must consider referring their patients for such surgery. It presents a balanced and reasonable picture of the overall subject, in addition to containing important information on such topics as opera tive technique and economic impact. Dr. Miller has done an outstand ing job in bringing together in one text the important considerations related to the evaluation of patients, the natural history of coronary artery disease and the benefits and problems associated with bypass surgery.

Book excerpt: In recent years there have been tremendous advances in cardiac imaging techniques covering the complete spectrum from echocardiography, nuclear cardiology, magnetic resonance imaging to contrast angiography. With respect to these noninvasive and invasive cardiac imaging modalities, marked technological developments have allowed the cardiologist to visualize the myocardium in a far more refined manner than conventional imaging was capable of. Echocardiography has extended its domain with intravascular ultrasound, cardiovascular nuclear imaging has added positron emission tomography to its line of research, magnetic resonance imaging has been broadened with magnetic resonance angiography and spectroscopy, and finally contrast angiograp hy has widened its scope with excellent quantitation programs. For all these imaging modalities it is true that the application of dedicated quantitative analytic software packages enables the evaluation of the imaging studies in a more accurate, reliable, and reproducible manner. It goes without saying that these extensions and achievements have resulted in improved diagnostics and subsequently in improved patient care. Particularly in patients with ischemic heart disease, major progress has been made to detect coronary artery disease in an early phase of the disease process, to follow the atherosclerotic changes in the coronary arteries, to establish the functional and metabolic consequences of the luminal obstructions, and to accurately assess the results of interventional therapy.
