Available in PDF, EPUB and Kindle. Book excerpt: Developmental biology of normal bone and cartilage including histogenesis, molecular/gene and biomechanical aspects is updated and expanded. The book outlines the biology of: bone repair with differing mechanical environments; cartilage repair at articular and physeal sites; and distraction osteogenesis. The generously illustrated text provides an in-depth presentation of the interplay between normal developmental biology, abnormal pathologic states and the influence of operative and non-operative orthopedic interventions on childhood orthopedic deformity. Thirty-four principles underlying the development, progression and management of skeletal deformity in the growing child are defined. Orthopedic management including surgical treatment is discussed for: skeletal dysplasias; epiphyseal growth plate fracture-separations; lower extremity length discrepancies; and deformities of joints and epiphyses due to metabolic, inflammatory, infectious, hematologic, and neoplastic disorders. Treatments are related to extent of deformity, remodeling post-surgery and possible recurrence. This 2nd edition of Pediatric Orthopedic Deformities has been expanded to cover more regions and disorders and is being presented in 3 volumes.

Available in PDF, EPUB and Kindle. Book excerpt: The brilliant yet simple idea of introducing a catheter percutaneously into an artery, without first dissecting it free, using a flexible guide wire, has led to a truly revolutionary breakthrough in abdominal x-ray diag nosis (SELDINGER, 1953). In the meantime, methods and techniques for injecting contrast media into various vessels have become largely standardized; innumerable publications have appeared which deal with every conceivable aspect of angiographic technique and interpretation. This volume is designed to present our experience with abdominal angiography. We deliberately refrained from any systematic discussion of the genitourinary tract, which has been adequately dealt with in the literature, also with respect to angiographic findings. Our interest in the retroperitoneal region is based mainly on its significance in differential diagnosis. In ten years of angiographic activity, our Department had made successful use of a simple technique which appears suitable also for smaller hospitals. We wish to point out its diagnostic potential and, at the same time, to outline its limitations. Our experience embraces 2804 abdominal angiograms, which we have classified according to clinical and morphologic anatomical criteria. Their diagnostic interpretation has been compared with the surgical or histopathological results. This may help others to avoid errors of the type which we discovered in our own work. Angiographic diagnosis requires not only familiarity with normal radiographic anatomy, but also specific knowledge of angiographic patho morphology. We have tried to identify those features which typify the individual findings and to derive therefrom valid generalizations with the aid of simple sketches.

Available in PDF, EPUB and Kindle. Book excerpt: The first edition of Surgery of the Hip loint has had certain measures of success. Its cover won the Outstanding Award for art at a publishers trade show. A year later it was translated into Spanish for exposure to the vast world of the Spanish speaking peoples. As I traveled through Europe, it was repeatedly a pleasant surprise to have the book recognized as an authorita tive reference. This was a great tribute to the experts whose diligent efforts made it all possible. Apparently the book has stood the test of time to judge from the many inquiries and constructive comments made toward urging us on to write a second edition. It was not an easy task to gather another cadre of authorities to update our knowledge of the hip joint. People who have earned respected positions in their field are unavoidably burdened with a busy schedule, so a chapter in this text must be appreciated as coming from someone devoted to giving up some of his precious time for the sake of sharing his knowledge with peers and students.

Book excerpt: At first sight it may appear strange that a volume of Progress in Pediatric Surgery should be devoted to the history of our specialty. One assumes that progress is concerned primarily with recent developments whilst history deals with matters of the past. However, in the past there has also been considerable progress in the development of our understand ing of paediatric surgical problems, otherwise we would not have pro gressed to our present achievements. The editors, therefore, do not apologize for compiling this volume but, on the contrary, feel that the publication of this volume is most timely. Modern paediatric surgery has now been practised for three genera tions. The handful of pioneers who were the founders of our specialty worked mainly before the last world war. A few dozen of the inter mediary generation started work immediately after the war, while the new generation who are now dominating our specialty must be counted in thousands. Two factors have radically altered paediatric surgery as practised by the intermediary and the present generation of surgeons.

Book excerpt: This handbook fully investigates reverse shoulder arthroplasty (RSA), presenting all the recent advances in the field to enable shoulder surgeons to treat patients with complex conditions, such as rotator cuff tears and instability, failed surgery and combined arthritis, or proximal humerus neoplasia. Reverse shoulder arthroplasty is becoming increasingly common because conventional total shoulder replacement may cause pain, loss of strength, simple or complex disabilities as well as limited motion, reducing general quality of life. The goal of a reverse prosthesis is to restore a painless, biomechanically valid joint. Drawing on the results of recent studies, the book covers all relevant aspects of RSA, including basic science, pathogenesis, clinical and instrumental evaluation, surgical techniques and complication management, helping readers to better understand when and how reverse shoulder arthroplasty should be implanted and what to do in cases of poor results. Written by leading shoulder specialists, the book provides surgeons and rehabilitation specialists, as well as residents and shoulder fellows, with a valuable, state-of-the-art guide for clinical practice.

Available in PDF, EPUB and Kindle. Book excerpt: Since RUTHERFORD MORISON left us with the concept of the Omentum being the 'abdominal policeman', clinicians have tacitly assumed that they know sufficient about the structure and function of this organ. However interest in the omentum and its relationship to clinical surgery has recently been develop ing. This book examines all aspects with special reference to surgery and should provide a welcome impetus in research and clinical practice. The editors and contributors have produced a book which is comprehensive and well illustrated and contains detailed refer ences to the important original sources - so essential in a work of this nature. It is written for those who wish to share the delight of acquiring knowledge - even about a comparatively humble organ - as well as for practical surgeons. Both will find ample information to arouse their interest and expand their surgical horizons in exciting ways of which they will almost certainly not have dreamt. I welcome a book of this calibre on a subject which deserves our increasing interest. I delight in the fact that it is dedicated to my friend and colleague MARTIN ALLGOWER.

Book excerpt: In medicine the understanding and interpretation of the complex reality of illness currently refers either to an organismic approach that focuses on the physical or to a 'holistic' approach that takes into account the patient's human sociocultural involvement. Yet as the papers of this collection show, the suffering human person refers ultimately to his/her existential sphere. Hence, praxis is supplemented by still other perspectives for valuation and interpretation: ethical, spiritual, and religious. Can medicine ignore these considerations or push them to the side as being subjective and arbitrary? Phenomenology/philosophy-of-life recognizes all of the above approaches to be essential facets of the Human Condition (Tymieniecka). This approach holds that all the facets of the Human Condition have equal objectivity and legitimacy. It completes the accepted medical outlook and points the way toward a new `medical humanism'.

Available in PDF, EPUB and Kindle. Book excerpt: Research on the hormonal control oflactation - the subject of this monograph - has long been the major interest of this laboratory. Studies were initiated in the mid 1930s by the late Professor S. 1. Folley, FRS, who directed the work with immense enthusiasm and devotion until his untimely death in 1970. This fruitful area of basic and applied research has, in recent years, attracted widespread attention; there have been many exciting events and developments with a dramatic increase in the number of publications. These events are diverse and include the identification, isolation and sequencing of human prolactin; the identification and isolation of placentallactogens in several ruminant species with recognition of their importance in mammary growth and differentiation; the introduction of highly sensitive bioassay, radioimmuno- and radioreceptor-assay techniques for mammotrophic hormones; the growing clinical appreciation of the immunological and nutritional importance of colostrum and milk to the newborn; the intensification of studies on normal mammogenesis to establish a sound basis for studies on mammary cancer; and the exploitation of the mammary gland, with its characteristic differentiation patterns and multiple synthetic abilities, as a valuable tissue for the investigation of basic mechanisms involved in the synthesis under hormonal control of enzymes and secretory proteins; and for the study of hormone - receptor interactions.
