Available in PDF, EPUB and Kindle. Book excerpt: Primum non nocere... The fact that a surgical procedure can leave any kind of pain casts a shadow over this tenet, which is seen as the basis of medical practice and anchor of its principle ethic... It is all the more surprising in that medicine has only paid attention to this paradoxical chronic pain situation for the past few years. Clarifying the knowledge acquired in this field has become all the more urgent for any care-giver today confronted by a legitimate request from patients: Why and how can a surgical procedure, which is supposed to bring relief, leave behind an unacceptable sequela? This is the approach which the contributors to this new subject of major clinical interest invite you to follow as you work your way through this book.

Book excerpt: Interest in the assessment and management of children's pain has increased dramatically over the past few years. Recent advances in the understanding of the plasticity and complexity of pain processing and new information about internal pain-inhibitory systems have revolutionized traditional approaches to pain control. It is now known that children's pain is plastic--not simply and directly related to the nature and extent of tissue damage--and that there are many ways to alleviate their suffering. Using a multifaceted approach that encompasses the neural and psychological factors that mediate pain, and featuring specific examples drawn from the Pain Clinic at The Children's Hospital of Western Ontario, this uniquely practical volume presents the most up-to-date information on how to assess and control acute, recurrent, and chronic pain in children. In recognition of the multidimensional nature of children's pain experiences, the book opens with a discussion of what pain is to children, the types of pain they experience, and the developmental, familial, situational, and emotional factors that modify children's perceptions and behaviors. Physiological, behavioral and psychological pain measures and direct measures for evaluating pain in infants and children are described and evaluated. The complexity of the nociceptive system, which enables practitioners to alter children's pain perceptions, is covered in detail, and information is provided about how to teach children and parents about pain modulation. In the chapters that cover treatment, the primary pharmacological and non-pharmacological interventions for reducing pain are reviewed along with practical suggestions on how to select and use cognitive-behavioral techniques in ambulatory clinics and hospital settings. Separate chapters offer separate, in-depth discussions on the management of acute pain, recurrent pain syndromes (such as headaches, abdominal and limb pains), and chronic pain. Drawn from the author's experience as director of a multidisciplinary pain clinic for children, this straightforward volume, with its numerous case illustrations and research results, will be an invaluable resource to a all health professionals, particularly those in psychology, behavioral medicine, nursing and pediatrics. It also serves as an excellent text for students in these disciplines.

Available in PDF, EPUB and Kindle. Book excerpt: Currently, two-thirds of patients undergoing surgery experience acute postoperative pain (POP), which increases the possibility of immediate postoperative complications and facilitates the development of chronic POP. Post-surgical pain does not have a useful biological function, so it must be prevented, avoided, and/or eliminated. The objective of the rational management of acute pain after surgery is to facilitate the rapid recovery of patients with a prompt integration into their usual preoperative activities. The global crisis of opioid abuse has forced us to use other analgesic drugs as well as regional analgesia techniques that are part of effective multimodal analgesia. The non-pharmacological approach has also gained importance in the comprehensive management of acute POP. This book reviews advances in the understanding, management, and complications of POP.

Book excerpt: Postoperative pain management studies documented serious pain control inadequacies, from poor pain assessment methods to minimal, infrequent pain relief, in children's acute care settings. This retrospective study assessed overall quality of pain management in a Pediatric Intensive Care Unit (PICU). Records for the first 72 hours of postoperative treatment of 100 anonymous PICU patients, six months to 17 years old, were examined. Data collection documented pain assessment, medication (prescribed and administered), type of provider, and need for adjustment. Cross-sectional data analysis was done with an appropriate statistical software package. The scope of findings was limited by data collection restrictions (age, surgery dates) due to privacy concerns. The data indicate that, contrary to expectations, pain was well controlled in 97 percent of the patients, with initial pain medication orders altered in only one percent.

Book excerpt: Largely reorganised and much expanded in this second edition, Practice and Procedures brings together in a single volume general methods of pain assessment and presents the wide range of therapies that can be provided by a range of health care disciplines. Authored by a multidisciplinary team of experts, chapters can stand alone for readers looking for a general overview of the methods of techniques for pain management available to them or work to complement chapters in the preceeding three volumes, providing practical procedures and applications in the management of acute, chronic and cancer pain. The book is divided into three parts. Part One covers the principles of measurement and diagnosis, including history taking and examination, the selection of pain measures, diagnostic tests and novel imaging techniques. Part Two discusses the full range of therapeutic protocols available, from pharmacological therapies, through psychological techniques, physical therapy and international procedures, to techniques specific to pain assessment and management in paediatric patients. Part Three provides information on planning, conducting, analysing and publishing clinical trials, with invaluable guidance on the techniques of systematic review and meta-analysis in pain research. Part Four considers the role of multidisciplinary pain management teams, their organization, their place within different health care systems, and how best to manage change when implementing such a service. Part Five concludes the volume, investigating the use of guidelines, standards and quality improvement initiatives in the management of post-operative pain, and discussing the expert medicolegal report.
