Book Synopsis Colonic Motility by : Sushil K. Sarna
Download or read book Colonic Motility written by Sushil K. Sarna and published by Biota Publishing. This book was released on 2010-11-01 with total page 159 pages. Available in PDF, EPUB and Kindle. Book excerpt: Three distinct types of contractions perform colonic motility functions. Rhythmic phasic contractions (RPCs) cause slow net distal propulsion with extensive mixing/turning over. Infrequently occurring giant migrating contractions (GMCs) produce mass movements. Tonic contractions aid RPCs in their motor function. The spatiotemporal patterns of these contractions differ markedly. The amplitude and distance of propagation of a GMC are several-fold larger than those of an RPC. The enteric neurons and smooth muscle cells are the core regulators of all three types of contractions. The regulation of contractions by these mechanisms is modifiable by extrinsic factors: CNS, autonomic neurons, hormones, inflammatory mediators, and stress mediators. Only the GMCs produce descending inhibition, which accommodates the large bolus being propelled without increasing muscle tone. The strong compression of the colon wall generates afferent signals that are below nociceptive threshold in healthy subjects. However, these signals become nociceptive; if the amplitudes of GMCs increase, afferent nerves become hypersensitive, or descending inhibition is impaired. The GMCs also provide the force for rapid propulsion of feces and descending inhibition to relax the internal anal sphincter during defecation. The dysregulation of GMCs is a major factor in colonic motility disorders: irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), and diverticular disease (DD). Frequent mass movements by GMCs cause diarrhea in diarrhea predominant IBS, IBD, and DD, while a decrease in the frequency of GMCs causes constipation. The GMCs generate the afferent signals for intermittent short-lived episodes of abdominal cramping in these disorders. Epigenetic dysregulation due to adverse events in early life is one of the major factors in generating the symptoms of IBS in adulthood.

Book Synopsis Principles and Practice of Geriatric Surgery by : Ronnie Ann Rosenthal
Download or read book Principles and Practice of Geriatric Surgery written by Ronnie Ann Rosenthal and published by Springer Science & Business Media. This book was released on 2011-07-12 with total page 1364 pages. Available in PDF, EPUB and Kindle. Book excerpt: "In the preface to this impressive and well-produced book, the editors state that their aim is not to describe a new surgical specialty, since most surgeons will soon need to be "geriatric surgeons," but to assemble a comprehensive account that will allow "all providers of healthcare to the elderly to understand the issues involved in choosing surgery as a treatment option for their patients." This is a useful book that deserves to do well. I hope that the editors and their publisher will have the stamina to make this the first of several editions, as it is clear that updated information about surgery in the elderly will be required to keep pace with this important field." Book Synopsis Emergency General Surgery in Geriatrics by : Rifat Latifi
Download or read book Emergency General Surgery in Geriatrics written by Rifat Latifi and published by Springer Nature. This book was released on 2021-03-29 with total page 491 pages. Available in PDF, EPUB and Kindle. Book excerpt: There is a growing need for a book on emergency general surgery in the elderly, as adults age 65 and older are the fastest growing segment of the population worldwide, and their number is expected to double to 89 million between now and 2050. Based on these changing demographics, it is expected that there will be a concurrent rise in the demand for a variety of surgical services, including vascular surgery (with a projected growth of 31%) and general surgery (with a projected growth of 18%), as well as in all sub-disciplines of surgery (thoracic, cardiac, oncology, hepatobiliary, transplant, plastic colorectal, orthopedic, gynecologic, urology and neurosurgery). In addition, older surgical patients often require a different level of care than younger patients during the preoperative, intraoperative and postoperative phases. Many of our geriatric population have multiple chronic illnesses beyond the one for which surgery is required, and therefore are prone to developing postoperative complications, functional decline, loss of independence, and other undesirable outcomes. In response, this book offers surgeons, geriatrists and nurses state-of-the-art surgical approaches and essential information on the complex perioperative care of the elderly.

Book excerpt: The second, fully updated edition of this book applies and contextualizes up-to-date information on pediatric surgery for low and middle-income countries (LMICs). The book is organized in general anatomic and thematic sections within pediatric surgery, such as urology, oncology, orthopedics and gastroenterology and includes chapters addressing the unique challenges and approaches for pediatric surgery in low-resource settings. Each chapter has dual authorship LMIC author providing context-specific insights and authors from high-income countries (HICs) contributing experience from well-resourced settings. Written in a reader-friendly format, this book has a uniform structure in each chapter, with introduction, demographics, etiology, pathophysiology, clinical presentations, investigations, management, outcome, prevention, ethics, evidence-based surgery and references. This comprehensive volume fills the gap between up-to-date pediatric surgical scholarship and knowledge developed and applied in HICs, and the practical needs of practitioners in low-resource settings. This is an indispensable guide for postgraduate surgical trainees in Africa and other LMICs as well as general surgeons practicing in Africa and other LMICs, who need to care surgically for children.

Book Synopsis The ASCRS Textbook of Colon and Rectal Surgery by : Scott R. Steele
Download or read book The ASCRS Textbook of Colon and Rectal Surgery written by Scott R. Steele and published by Springer. This book was released on 2016-03-24 with total page 1269 pages. Available in PDF, EPUB and Kindle. Book excerpt: This third edition text provides a completely revised and updated new version of this unique, modern, practical text that covers the strategic evaluation, specific approaches, and detailed management techniques utilized by expert colorectal surgeons caring for patients with complex problems–whether they result from underlying colorectal disease or from complications arising from previous surgical therapy. The format follows that of both a “how to” manual as well as an algorithm-based guide to allow the reader to understand the thought process behind the proposed treatment strategy. By making use of evidence-based recommendations, each chapter includes not only background information and diagnostic/therapeutic guidelines, but also provides a narrative by the author on his/her operative technical details and perioperative “tips and tricks” that they utilize in the management of these complex surgical challenges. Distinctive to this book, is the reliance on experts in the field including past presidents of the ASCRS, as well as multiple other national and internationally recognized surgeons, to lend their personal insight into situations where data may be more sparse, but individual and collective experience is paramount to making sound decisions and thereby optimizing patient outcomes. The text includes chapters on the assessment of risk and methods utilized to minimize perioperative complications. In addition, it incorporates sections covering the medical and surgical therapies for abdominal, pelvic and anorectal disease. Moreover, the technical challenges of managing complications resulting from the original or subsequent operations is addressed. The underlying focus throughout the text is on providing pragmatic and understandable solutions that can be readily implemented by surgeons of varying experience to successfully treat complex colorectal problems. However, it also goes beyond the technical aspects of colorectal surgery and includes special sections highlighting the essence of a surgeon; covering aspects involving the medical-legal, ethical, and economic challenges confronting surgeons. Throughout the text, each author provides an ongoing narrative of his/her individual surgical techniques along with illustrations and diagrams to “personally” take the reader through the crucial steps of the procedure, and key points of patient care inherent to that topic. Additionally, where appropriate, links to online videos give the reader an up-front look into technical aspects of colorectal surgery.
