Book excerpt: More than 140 years ago, lupus erythematosus (LE) was recognized as a disease entity by clinicians working in the field of dermatology, which had only recently become an independent medical discipline. Soon after cutaneous lupus was first reported, it was realized that, apart from the skin, the disease could involve other organs and thus be systemic in nature. The latter observations were first made by MORITZ KApOSI [1], whose work has attracted renewed attention re cently and who succeeded FERDINAND VON HEBRA to the chair of dermatology at the Medical Faculty in Vienna. The early description of lupus erythematosus in both its cutaneous and systemic manifes tations was thus intimately associated with Vienna and its Medical School. The next phase in the study of lupus was characterized by an in crease in knowledge of the type and extent of organ involvement. The work by OSLER [2], LIBMANN and SACKS [3], and KLEMPERER [4] best represents these advances. The increase in clinical knowledge of LE finally led to DUBOIS' famous monograph [5], which was pub lished at a time of renewed interest in SLE, elicited by the descrip tion by HARGRAVES et al. [6] of the LE-cell phenomenon. A more detailed analysis of this finding revealed that the disease was charac terized by an abnormal immune response, although its pathogenetic implications were still unclear.

Available in PDF, EPUB and Kindle. Book excerpt: Two fields have played a leading role in biomedical research in recent years, the biochemistry of nucleic acids and immunology. Yet, with the exception of those aspects which have been concerned with antibody synthesis as an example of protein synthesis, there was until recently a lack of direct association between the two fields. Until quite recently the antigenicity of nucleic acids was still in doubt and indeed represented a controversial subject. Also, the exact role of the various nucleic acids in various stages of antibody synthesis was uncertain. These skepticisms and uncertainties disappeared rapidly in the last few years. New experi mental approaches brought the realization that nucleic acids, under appropriate conditions, are indeed immunogenic, and that the resulting antibodies can furnish new tools for the exploration of the mplecular structure of the all-important family of nucleic acid molecules. ' At the same time, the recognition of the antigenicity of nucleic acids brought a new level of understanding to certain auto-immune diseases and pro vided new material for the exploration of the role of a carrier in immune responses. Side by side with this development was the almost explosive development of new experimental approaches and new ideas pertaining to the problem of antibody formation. Nucleic acids in their various forms were recognized as playing an expected major role in the activation of antibody-forming cells. Perhaps less to be expected was the role they can playas non-specific stimulators of antibody formation.

As well as covering common and rare rheumatological problems, there are also chapters on investigations and emergencies, designed for quick reference. The handbook includes dedicated topics on systemic diseases affecting rheumatology; the relevant clinical guidelines and information needed for a rheumatologist to successfully management a young patient; and, a coloured section for guidance on rash-related investigations. Paediatric Rheumatology is also fully endorsed by the British Society for Paediatric and Adolescent Rheumatology and the UK Paediatric Rheumatology Clinical Studies Group.

Book excerpt: The Social Security Administration (SSA) administers two programs that provide disability benefits: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Selected Health Conditions and Likelihood of Improvement with Treatment identifies and defines the professionally accepted, standard measurements of outcomes improvement for medical conditions. This report also identifies specific, long-lasting medical conditions for adults in the categories of mental health disorders, cancers, and musculoskeletal disorders. Specifically, these conditions are disabling for a length of time, but typically don't result in permanently disabling limitations; are responsive to treatment; and after a specific length of time of treatment, improve to the point at which the conditions are no longer disabling.

Book excerpt: For eight decades the Primer on the Rheumatic Diseases has been the standard text from which most medical students and house ofcers have learned rheumatology. I myself will never forget thumbing through an older edition of the Primer as a second-year resident, while waiting to review a perplexing patient with my tutor. Fortunately the tutor was r- ning late with his own patients, so I had time to fip through the book – then much thinner – a couple of times. While turning the pages, per- ing the features of those diseases whose names were still exotic to me, and considering my patient’s history of conductive hearing loss and p- monary nodules, a light went on when I stumbled eventually on a part- ular chapter. I still remember the jaw-dropping efect on my tutor of my announcement then that I had a patient with Wegener’s granulomatosis. I think I became a rheumatologist that very moment! Subsequent editions of the Primer have sufered from the inevitable “obesity creep,” making it an outstanding reference textbook but virtually impossible to fip through quickly while awaiting one’s tutor, and even more difcult to slip into the pocket of a white coat to carry on rounds. For this reason we have created the Pocket Primer, a mini version that cuts the larger book down to its essentials.

Available in PDF, EPUB and Kindle. Book excerpt: This book comprehensively sets out the common aetiopathogenetic mechanisms shared by many, apparently diverse, diseases of the immune system. Unlike most other texts it does not emphasise the differences between autoimmune diseases, but establishes their many common links including hormonal effects, dietary and immunogenetic influences, complement deficiencies and environmental factors. Special attention is given to the effects of ageing and the relationship with malignancies. The scope of the book is very broad so as to cover the integration of the many diverse components which interact to cause autoimmunity, and it contains many 1988 and 1989 references and over 100 figures and tables, offering an attractive, up-to-date guide to modern concepts. It will greatly assist immunologists wishing to enter the field of autoimmunity, and will serve as an invaluable reference work for those already working in it.
