The full range of monogenic stroke disorders causing cerebrovascular disease, including ischaemic stroke, intracerebral haemorrhage, aneurysms and arteriovenous malformations, are covered. For each, clinical features, diagnosis, and genetics are described. Increasing evidence suggest that genetic factors are also important for the much more common multifactorial stroke; this evidence is reviewed along with the results of genetic studies in this area. Optimal and novel strategies for investigating multifactorial stroke, including the use of intermediate phenotypes such as intima-media thickness and MRI detected small vessel disease are reviewed. Book excerpt: This volume is the product of a February 1982 conference, cosponsored by the American Heart Association, the National Institutes of Health, and the Bowman Gray School of Medicine, which examined techniques for delineating quantitatively the natural history of atherosclerosis. Against the background of current pathologic and clinical knowledge of atherosclerosis, invasive and noninvasive evaluative methods now in use and under development are surveyed in depth. Correlative clinicopathologic studies of atherosclerosis pose special questions with respect to both luminal and plaque characteristics that are addressed in this volume. An old observa tion, based on the examination of arterial casts, suggested that the so-called nodose lesion of atherosclerosis may be at first flattened into the wall of a weakened, dilated artery, rather than raised into the lumen. This is now fully confirmed in vivo by ultrasonic and other imaging techniques. The morbid anatomist is challenged anew to describe lesions as they are likely to occur in vivo. To achieve closer correlation with natural conditions, perfu sion fixation of arteries under arterial pressure is becoming more widely used and has already demonstrated more valid quantita tion of the composition and configuration of lesions. While the noninvasive methods of B-mode and Doppler ultrasound are suitable only for the clinical study of superficial arteries, such as the carotid or femoral, the new and relatively noninvasive procedure of intravenous digital subtraction angio graphy can be effectively used for the examination of deep systems, such as cerebral vessels.

Book excerpt: The circulatory system is usually considered to be composed of tubes of various diameters, characterized by collateral and terminal branches. There is also a tendency to treat blood vessels merely as conducting tubes in which the various structures of the wall act as mechanical pumps wlrich modify their diameter. This is, of course, not so. In fact, we know that blood vessels, and in particular arteries, are organs with personalities of their own and a particular susceptibility to several diseases. In addition, blood vessels differ in structure, according to their localization, and age at differing rates. The experimental work car ried out so far clearly confirms the data that have come from spontaneous human pathology; experimentally induced arterial lesions have a definite tendency to appear in certain arteries and not in others, depending on the experimental procedures used, and in each specific artery the lesions appear to have a specific location. We now know that the arterial wall is a metabo licallyactive structure, in which a number of enzyme activities have been clearly demonstrated. It possesses a sensitive vasa vasorum apparatus and a specific reactivity to various lesion-inducing stimuli. We must also remember that the arterial wall is in continuous contact with the blood circulating through the endothelial cells lining the vascular bed. It is obvious, therefore, that any variation in the circulating blood mass can modify the morphology as well as the function of the vessel wall.

The first symposium was held three years ago in Athens, Greece, under the chairmanship 1 of Dr. Constantinos]. Miras , who, although he could not attend this symposium, participated actively in the deliberations of this Program Committee. Atherosclerosis together with its sequelae constitute the most important source of morbidity and mortality in civilized countries. While a major attack is being made on the consequences, the sequelae of this disease, not enough attention is being paid to the basic cause, atherosclerosis. Yet, if the basic disease were eradicated, the major concern of this symposium, the sequelae would constitute only a minor and rare disease group. The approach of the Program Committee was to bring together experts in the multiple disciplines which have a bearing on atherosclerosis. There is a great need for an exchange of ideas from various groups studying the basic process in many divergent ways. The hope we have is that those present (or those later studying the Proceedings) may be stimulated to attack the problem in new ways. Perhaps a breakthrough will be made or, at least, a brick or two added to build the structure, a rampart needed to defend against atherosclerosis. Better still, their contributions may help to confine it to a small area.
