Notre objectif était d'analyser l'activité des médecins généralistes français en fonction de leur lieu d'exercice rural ou urbain. Nous avons réalisé une étude ancillaire de la base de données ECOGEN, constituée à partir d'une étude transversale, multicentrique et nationale conduite auprès de128 cabinets de médecine générale de France métropolitaine. Le lieu d'exercice des médecins généralistes a été classé en 3 catégories selon une classification dérivée de celle de l'Insee : zone rurale, grappe urbaine et zone urbaine. Les motifs de consultation, les résultats de consultation et les procédures de soins étaient classés selon la Classification internationale des soins primaires (CISP-2). La base de données comprenait 20613 consultations. Le nombre annuel moyen de consultations par médecin généraliste était plus élevé (0,0001) dans les zones rurales, avec une durée de consultation plus courte (

Book excerpt: Introduction : peu de thèses définissent le milieu d'exercice des médecins généralistes interrogés. Ces milieux sont implicitement déterminés par la perception des médecins. L'INSEE (Institut National de la Statistique et des Études Économiques) définit une commune rurale comme une commune n'appartenant pas à une unité urbaine. L'INSEE propose d'autres définitions comme la définition de rural dans les travaux sur les bassins de vie en se basant sur des critères européens. L'objectif principal de cette étude est de déterminer des critères pour définir une pratique rurale ou urbaine selon la perception de médecins généralistes d'Aquitaine. Secondairement, de comparer les données de l'INSEE avec les perceptions des médecins interrogés, définir le milieu semi-rural et créer un outil d'aide à la détermination du milieu d'exercice des médecins généralistes. Matériel et Méthode : nous avions réalisé une étude qualitative par focus groupe et analysé des données par le logiciel NVivo®. Les 3 focus groupes avaient été réalisés en milieu rural, semi-rural et urbain entre Février et Mars 2017. Résultats : nous avions atteint la saturation des données. La perception des médecins interrogés permettait de définir leur type de pratique selon les critères suivants: distance des urgences, activité plus ou moins élargie à tous les aspects des soins premiers, densité de population, typologie du territoire, catégorie socio-professionnelle des patients facilement déterminée par l'appartenance aux différentes caisses principales de sécurité sociale. Il y avait plusieurs discordances entre les perceptions des médecins quant à leur lieu d'installation et les définitions de l'INSEE. Conclusion : la définition du type de pratique est plurifactorielle. A partir des critères déterminés, nous avons créé un outil d'aide à la détermination du milieu d'exercice. Aucune définition de l'INSEE n'est parfaite sur ce sujet. Par défaut, il faudrait utiliser celle des bassins de vie urbains, intermédiaires et ruraux selon la nouvelle typologie européenne.
