However, the United States faces a shortage of affordable and accessible housing for low-income older adults and individuals living with disabilities. This shortage is expected to grow over the coming years given the population shifts leading to greater numbers of older adults and of individuals living with disabilities. Housing is a social determinant of health and has direct effects on health outcomes, but this relationship has not been thoroughly investigated. In December 2016, the National Academies of Sciences, Engineering, and Medicine convened a public workshop to better understand the importance of affordable and accessible housing for older adults and people with disabilities, the barriers to providing this housing, the design principles for making housing accessible for these individuals, and the features of programs and policies that successfully provide affordable and accessible housing that supports community living for older adults and people with disabilities. This publication summarizes the presentations and discussions from the workshop.

The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Affordable housing policy in the US has often been focused at the federal level where the laws and funding to build new affordable housing historically have been determined. However, as federal housing subsidies from the 1960s expire and federal funding continues to decline, local governments, tenants and advocates face the difficult challenge of trying to retain affordability amid increasing demand for housing in many American cities. Now, instead of amassing land, financing and sponsors, affordable housing stakeholders must understand the existing resident needs and have access to the market for affordable housing. Arguing for preservation as a way of acknowledging a basic right to the city, this book examines the ways that the broad range of stakeholders engage at the building and city levels. This book identifies the underlying challenges that enable or constrain preservation to demonstrate that effective preservation requires long-term relationships that engage residents, build trust and demonstrate a willingness to share power among residents, advocates and the government. Book excerpt: With U.S. health care costs projected to grow at an average rate of 5.5 percent per year from 2018 to 2027, or 0.8 percentage points faster than the gross domestic product, and reach nearly $6.0 trillion per year by 2027, policy makers and a wide range of stakeholders are searching for plausible actions the nation can take to slow this rise and keep health expenditures from consuming an ever greater portion of U.S. economic output. While health care services are essential to heath, there is growing recognition that social determinants of health are important influences on population health. Supporting this idea are estimates that while health care accounts for some 10 to 20 percent of the determinants of health, socioeconomic factors and factors related to the physical environment are estimated to account for up to 50 percent of the determinants of health. Challenges related to the social determinants of health at the individual level include housing insecurity and poor housing quality, food insecurity, limitations in access to transportation, and lack of social support. These social needs affect access to care and health care utilization as well as health outcomes. Health care systems have begun exploring ways to address non-medical, health-related social needs as a way to reduce health care costs. To explore the potential effect of addressing non-medical health-related social needs on improving population health and reducing health care spending in a value-driven health care delivery system, the National Academies of Science, Engineering, and Medicine held a full-day public workshop titled Investing in Interventions that Address Non-Medical, Health-Related Social Needs on April 26, 2019, in Washington, DC. The objectives of the workshop were to explore effective practices and the supporting evidence base for addressing the non-medical health-related social needs of individuals, such as housing and food insecurities; review assessments of return on investment (ROI) for payers, healthy systems, and communities; and identify gaps and opportunities for research and steps that could help to further the understanding of the ROI on addressing non-medical health-related social needs. This publication summarizes the presentations and discussions from the workshop.

She proposes a new strategy for producing decent, affordable housing for low-income people through non-profit community-based organizations. The potential of a new housing policy built on empowering community groups and low-income households is compelling. The production, rehabilitation, management and/or ownership by community-based organizations, with funding and technical assistance provided by a new type of public support system, not only would offer participants much-needed shelter, but also control over and security in their living environments. These qualities have been lacking in housing sponsored by the private for-profit sector as well as in previous subsidy programs. The author analyzes the limitations of both profit-oriented developers and public agencies as the primary vehicles for developing low- and middle-income housing. Promoting small-scale neighborhood organizations as better suited for delivering such services, she focuses on large multi-family projects and argues that our urban public housing stock represents an irreplaceable resource that is rapidly decaying to a point of no return.
