Holding Government to Account draws on several unique sources of evidence, including interviews with senior officials from the National Audit Office and the civil service, as well as senior parliamentarians with experience of the NAO’s relationships with government and legislature. These interviews are supplemented by an analysis of previously unpublished manuscript material in the National Archives, examination of NAO reports and parliamentary and other reports focused on accountability. The book begins with a history of the National Audit Office in the context of the UK’s wider history. It then offers an overview of the constitutional, political and human legacies of the Exchequer and Audit Department, followed by a close examination of the National Audit Office’s leadership and decision-making from inception in 1984 through to the present. The authors conclude with an exploration of the way in which the meaning of public sector audit has evolved over time, in accordance with its wider political, ideological and material context. In doing so, they demonstrate that any question about the National Audit Office’s future and organisation is really a question about what democracy and good government mean in a modern bureaucratic state. The timetable is much tighter than for either High Speed 1 or Crossrail, despite the fact High Speed 2 is a much larger programme. Not allowing enough time for preparation undermines projects from the start. A rushed approach contributed to the failure of the InterCity West Coast franchise award. The Department has increased its High Speed rail team, but getting the right mix of skills is challenging and the Department lacks the commercial skills necessary to protect taxpayers' interest on a programme of this size

Book excerpt: The Department for Transport has approved expenditure of over £11 billion between 1998 and 2021 for the development of new and existing trunk roads and motorways by the Highways Agency, and just under £1.7 billion on major road schemes proposed and developed by local authorities in five year Local Transport Plans. Following on from a NAO report on this topic (HCP 321, session 2006-07; ISBN 9780102944600) published in March 2007, the Committee's report examines the steps taken by the Department for Transport and the Highways Agency to improve value for money and oversight of the roads programme and contracting methods and project management capability. By September 2006, the Agency's 36 completed schemes in the Targeted Programme of Improvement cost 40 per cent more than estimated initially, and for schemes still to be completed, latest forecasts indicate that final costs could be 27 per cent more than original estimates. The main causes for costs exceeding estimates are increases in construction costs, higher than forecast land prices and compensation to landowners, inflation and changes in the scope of the project. The report finds that the DfT has not been rigorous enough in its oversight of the Agency's delivery of major road schemes, allowing it too much latitude on delivery and cost plans, and has failed to monitor in-year expenditure against progress and delivery milestones. The Agency is overly reliant on consultants for project management expertise and needs to develop its in-house capability.

When this programme got the green light in 2007, we were supposed to get two aircraft carriers, available from 2016 and 2018, at a cost to the taxpayer of £3.65 billion. We are now on course to spend £5.5 billion and have no aircraft carrier capability for nearly a decade. The MOD rushed into a decision in the 2010 Strategic Defence and Security Review. Just 18 months later they were forced to admit they had got it wrong and revert to the original choice of aircraft. At the time of the SDSR the Department believed the cost of converting the carriers for the new aircraft would be between £500 million and £800 million. By May 2012 it had realised that the true cost would be as a high as £2 billion. Officials also made basic errors such as forgetting to include the costs of VAT and inflation. There are still concerns now. According to current plans, the early warning radar system essential for protecting the carrier will not be available for operation until 2022, two years after the first carrier and aircraft are delivered and initially operated. And the MOD does not yet have the funding to replace the shipping needed to support the new carrier. To avoid making the same mistakes again the MOD needs to start planning now for the next SDSR in 2015, including making sure that this time it has the right information on which to base decisions

The Programme is intended to improve accountability for functions currently carried out at arm's length from Ministers. The Cabinet Office says it is on track to make £2.6 billion of administrative savings by 2015. However there are substantial reservations about the robustness of this claim. Key concerns are that: there is a risk departments are claiming savings which are actually cuts to services, when they should be including only genuine savings arising from administrative reorganisations; estimates of transition costs such as redundancy and pension costs are incomplete; the savings estimate does not fully take account of the ongoing costs to other parts of government of taking on functions being transferred from abolished bodies and some departments have wrongly included wider savings from bodies being retained, rather than just administrative savings from bodies being abolished or substantially reformed. The Cabinet Office has accepted that its savings estimate needs to be reassessed and has undertaken to 'rebase' it. Focus now needs to be on managing the Programme effectively. Departments have decided on the form of individual reorganisations themselves without clear direction from the centre, leading in some cases to inconsistent treatment of bodies with similar functions. Furthermore, departments may not be getting the best value for money from the sale or transfer of assets of bodies being abolished

It has failed to exercise the robust financial management necessary to control its resources effectively in the long term, and there is a shortfall in planned expenditure against likely funding of up to £36 billion over the next ten years. The Department's consistent pattern of planned overspend demonstrates serious organisational failings and a dangerous culture of optimism. There are systemic failings: a tendency towards financial over-commitment, weaknesses in the financial planning processes and a division in responsibilities and accountability for financial stewardship. The Accounting Officer has not discharged his responsibility to ensure that expenditure represents value for money, and there is no explicit financial strategy linking funding to priorities. When financial savings have to be found there is then no clear basis for determining where cuts should be made. The appointment of a professionally qualified Finance Director is welcomed. The defence estate is valued at over £20 billion, and costs an estimated £2.9 billion per year to run. The built estate in the UK has been reduced by 4.3% between 1998 and 2008, achieving £3.4 billion in sale receipts. But more of the estate should have been released. The Department does not assess its estate against clear objective criteria. The Department does not collect centrally the information and data that would allow it to manage its estate in an effective way. It appears to lack urgency in its plans to improve its information base.

They are estimated, based on research covering the 2003-04 period, to cost society over £15 billion a year, £13.9 billion of which is due to drug-related crime. Fully one quarter of all problem drug users are hard-core offenders for whom drug treatment is ineffective and, indeed, whose offending has sharply increased after entering treatment. It is of particular concern that measures to cut problem drug use by young people are having limited effect. Preventing the young from descending into problem drug use is an essential part of bringing down the number of problem drug users in future. In 2008, the Government introduced a 10 year cross-departmental drug strategy to tackle problem drug use, which it defined as use of opiates (mainly heroin) and/or crack cocaine. The Home Office has overall responsibility for the strategy, with a number of other government departments and agencies, at national, regional and local levels, sharing responsibility for its delivery. Central and local government collectively spend £1.2 billion a year to deliver the measures set out in the strategy. Given the public money spent on the strategy and the cost to society it is unacceptable that there has not been sufficient evaluation of the programme of measures in the strategy and that it is not known if the strategy is directly reducing the overall cost of drug-related crimes. Following a recommendation made by the National Audit Office, the Home Office has agreed to produce an overall framework to evaluate and report on the value for money achieved from the strategy, with initial results from late 2011.

Too many diabetic people are developing complications because they are not receiving the straightforward care and support they need. The Department estimates that 24,000 people with diabetes die prematurely each year because their diabetes has not been managed effectively and it's estimated 80% of the costs of diabetes in the NHS are attributable to the treatment and management of serious, yet avoidable, diabetic complications. These costs will continue to increase as the number of diabetic people rises as will premature deaths. The National Service Framework for Diabetes, 2001 set out minimum standards for good diabetes care, including nine basic care processes which check for the early signs of avoidable diabetic complications as well as treatment targets for the management of blood glucose, blood pressure and cholesterol to minimise the risk of diabetic complications developing. Progress in delivering these standards of care and in achieving treatment targets has been poor. There is no strong national leadership, no effective accountability arrangements for commissioners, and no appropriate performance incentives for providers. The Committee saw no evidence that the Department will ensure that these issues are addressed effectively in the new NHS structure. Variation in the level of progress across the NHS also means that there is an unacceptable "postcode lottery" of care

The Ministry's own resource accounts for 2010-11 were delivered late and there were significant problems with the accounts produced by two of its major arm's length bodies, the Legal Services Commission and HM Courts Service's Trust Statement. The Ministry faces significant accounting challenges for the 2011-12 financial year, due to the required earlier publication of the accounts. The Ministry needs to break the cycle of continuing failure to produce accurate and timely accounts. It also faces considerable challenges in meeting its tough spending review commitments, but without a full understanding of its costs, the Ministry risks unnecessarily cutting frontline services, which are critical to the poorest in the community, rather than ensuring savings are achieved through genuine efficiencies. Maximising the income it obtains will help the Ministry and fine collection is improving, but it is being outpaced by the growth in fines outstanding. Excellent financial management is critical to the Ministry's future success as it seeks to achieve significant efficiency gains while coping with workload pressures, such as increases in the prison population, that are largely outside its control.

These programmes include: a Diamond Synchrotron (costing £383 million) to produce intense X-rays and shorter wavelength emissions for examining structures at molecular and atomic level; a new research ship, RRS James Cook, to conduct oceanographic and marine studies (a budget of £40 million); and a new Antarctic research station (with a budget of £34.7 million) for monitoring climate, ozone and space weather. Following on from a NAO report on this topic (HCP 153, session 2006-07, ISBN 9780102944198) published in January 2007, the Committee's report examines how large scientific facilities are delivered and the how their value is assessed. Findings include that the first two projects have been delivered largely to time and budget, but other projects still at an early stage are forecasting slippage, with five of the six most mature projects forecasting significant increases in the initial estimates for operating costs. Research Councils have not always sufficiently evaluated options for locating new scientific facilities and have had difficulty in attracting a sufficient number of bidders for contracts to help build new facilities. Performance indicators used by the Research Councils and project teams for monitoring the success of these facilities are not always sufficiently comprehensive or measurable, and if the UK is to maximise the value of these facilities, it needs to attract more young people into science to make good use of the research capacity now being built.

The living standards of vulnerable households will have been greatly improved by the installation of, for example, 810,000 new kitchens, 610,000 new bathrooms and 1,140,000 new central heating systems. There have also been wider benefits such as more tenant involvement in housing decisions and jobs created in deprived areas. The report welcomes the improvements made and the substantial progress towards the original target of all social housing being of a decent standard by December 2010. However, despite this progress, the target will not be met: 305,000 homes will still be non-decent at that date and the last of these will not be decent until 2018-19. The DCLG needs to do more to ensure that landlords can complete this outstanding work and that properties are not allowed to fall back into disrepair. It also needs to improve its financial control over this Programme. It is still not clear how much the Department itself has actually spent on the Programme, nor whether DCLG has obtained best value from the funds given to Arms Length Management Organisations (ALMOs). The Programme will have cost local authorities and Registered Social Landlords approximately £37 billion by 2010-11. The Department lacks some basic management information on the Programme, and needs to address these deficiencies in order to evaluate the impact properly.

Individual consumers lose around £6.6 billion every year because of the malpractices of traders. At least £4.8 billion is lost through malpractices which occur at a regional or national level, such as mass market scams, counterfeiting, and unscrupulous traders who operate over large geographical areas. The Department has overall responsibility for policy on consumer protection. However, the majority of enforcement work, from weights and measures testing to the prosecution of rogue traders, is carried out by local authority Trading Standards Services, each with jurisdiction in only its own local area. The Committee states, that the Department has limited understanding of the true cost of protecting consumers or of the success of existing interventions. There is no clear and complete information on how much enforcement activity actually costs. The approach to enforcing consumer protection has not kept pace with the changing nature of the problems it is intended to tackle, such as online shopping. Any changes the Department makes must deliver a system fit for the modern era. Responsibility for tackling regional and national instances of malpractice or rogue trading must be clearly designated.

Book excerpt: In March 2003, the Government signed three 30-year public private partnership (PPP) deals worth an estimated £15.7 billion for the maintenance and renewal of London Underground trains and stations, track and signalling, after a five-year procurement process which cost £455 million. Under the PPPs, London Underground retains responsibility for operations and safety, whilst three private sector infrastructure companies (infracos) maintain and renew the infrastructure. There is a built-in periodic review mechanism, untried in any other PPP arrangement, which enables the parties to respecify requirements and make changes to prices. Ownership of London Underground was transferred to Transport for London in July 2003, which reports to the Mayor of London, although the Department for Transport still retains an interest. Following on from two NAO reports (HCP 644, session 2003-04, ISBN 0102928339 and HCP 645, session 2003-04, ISBN 0102928320) published in June 2004, the Committee's report examines the rationale for the PPPs, the contract price and other costs, and the contract management arrangements.
