Book excerpt: En 2021 en France, les « maladies psychiatriques » (avec en première ligne les troubles anxio-dépressifs) représentent 140/0 des dépenses totales de santé, le premier poste de dépenses de l'Assurance maladie et sont responsables de 40 0/0 de l'absentéisme au travail. Les patients en souffrance psychologique représentent près de 10 0/0 des consultations de médecine générale. En effet, le généraliste est au centre de la prise en charge, puisque la majorité de ces patients ne consultera pas de spécialiste. Il n'a pas été retrouvé de littérature portant sur le ressenti des médecins généralistes dans la prise en charge de ces patients. Méthode : Il s'agissait d'une étude qualitative avec entretiens individuels semi-structurés et analyse inspirée de la théorisation ancrée. Les médecins étaient contactés par téléphone ou email, leurs coordonnées étaient retrouvées sur le site de l'Ordre des médecins. Les entretiens avaient lieu en présentiel (au cabinet) ou en visioconférence, ils étaient intégralement enregistrés sur dictaphone puis retranscrits mot à mot. Une analyse ouverte puis axiale a été réalisée à l'aide du logiciel Nvivo. Résultats : Le médecin généraliste est un acteur clé du système de santé : sa position de premier interlocuteur pour accéder au parcours de soins, sa proximité physique du lieu de résidence du patient, la relation au long cours médecin-patient en font le premier interlocuteur pour les patients anxio dépressifs. Il a un rôle de dépistage de la souffrance psychologique, et accompagne le patient à son rythme, consultation après consultation. Cet échange non centré sur le corps vient renforcer et enrichir la relation médecin-patient. Cependant, il est confronté à de nombreuses difficultés limitant la qualité des soins : la carence ressentie des généralistes dans la formation à la psychothérapie, le non remboursement des psychologues, la pénurie de médecins (généralistes et psychiatres) entraînant une surcharge de travail, les résistances des patients, et parfois un sentiment d'inefficacité pour le généraliste. Conclusion : Les médecins généralistes s'estiment plutôt à l'aise dans la prise en soins des patients anxio dépressifs, qui leur est facilitée par la bonne communication médecin-patient et la relation au long cours. Cependant, ils font face à de nombreuses difficultés qui limitent la qualité des soins, et qui risquent de s'exacerber au vu de l'importante augmentation de la prévalence des troubles anxio-dépressifs et des limites actuelles de l'offre de soins psychiatriques.

L'espérance de vie des patients souffrant de ces troubles est sensiblement diminuée car la prévalence des pathologies somatiques est plus forte dans cette population. Cependant, dans certains territoires en France, médecins généralistes et psychiatres font face à des difficultés dans le parcours de soins de leurs patients, souvent liées à un défaut de communication entre ces professionnels. OBJECTIF Le but était d'évaluer les difficultés rencontrées par les médecins généralistes et les psychiatres dans le département du Puy-De-Dôme et les moyens mis en œuvre par ces derniers pour les surmonter. MÉTHODE Il s'agit d'une étude qualitative réalisée par une approche inspirée de la théorisation ancrée à partir d'entretiens semi-directifs conduits auprès de 15 médecins généralistes et 15 psychiatres du Puy-De-Dôme. Les professionnels ont été inclus en faisant varier les critères de sexe, d'âge et de modalités d'exercices. RÉSULTATS Les médecins généralistes et les psychiatres interrogés ont affirmé avoir conscience des enjeux. Ils disaient s'investir dans la prise en charge de leurs patients par la rédaction de courriers. Les psychiatres reconnaissaient ne pas en faire suffisamment et parfois ne pas recevoir d'informations détaillées de la part des généralistes. Ils ont mis en avant une méconnaissance des attentes de chacun voire des perceptions différentes de la pathologie mentale. Pour faire face aux difficultés, ils ont encouragé le dialogue et les rencontres interprofessionnelles dans différents cadres afin de mieux définir les rôles de chacun dans la prise en charge. Le travail en réseau et la coordination dans le cadre du parcours de soins, ainsi que la responsabilisation du patient et son accompagnement ont été mis en avant. CONCLUSION Le développement du dialogue entre psychiatres et MG permettrait de résoudre certaines difficultés du parcours de soins. L'utilisation de messageries informatiques sécurisées, un numéro de téléphone d'avis et d'orientation en psychiatrie pourraient faire gagner du temps à l'échange entre les professionnels.

Nelly Oudshoorn explains why it is that, although the technical feasibility of male contraceptives was demonstrated as early as the 1970s, there is, to date, no male pill. Ever since the idea of hormonal contraceptives for men was introduced, scientists, feminists, journalists, and pharmaceutical entrepreneurs have questioned whether men and women would accept a new male contraceptive if one were available. Providing a richly detailed examination of the cultural, scientific, and policy work around the male pill from the 1960s through the 1990s, Oudshoorn advances work at the intersection of gender studies and the sociology of technology. Oudshoorn emphasizes that the introduction of contraceptives for men depends to a great extent on changing ideas about reproductive responsibility. Initial interest in the male pill, she shows, came from outside the scientific community: from the governments of China and India, which were interested in population control, and from Western feminists, who wanted the responsibilities and health risks associated with contraception shared more equally between the sexes. She documents how in the 1970s, the World Health Organization took the lead in investigating male contraceptives by coordinating an unprecedented, worldwide research network. She chronicles how the search for a male pill required significant reorganization of drug-testing standards and protocols and of the family-planning infrastructure—including founding special clinics for men, creating separate spaces for men within existing clinics, enrolling new professionals, and defining new categories of patients. The Male Pill is ultimately a story as much about the design of masculinities in the last decades of the twentieth century as it is about the development of safe and effective technologies.

Book excerpt: In the late 1950s, like tens of thousands of young men of his generation, Pierre Bourdieu, having recently passed the agrégation in philosophy, found himself immersed in the Algerian war. Motivated by an impulse that, as he himself says, ‘was civic rather than political’, nothing seemed more important to him than to understand the Algerian situation and provide the elements that would enable others to come to an informed judgement about it. In extremely tough conditions and along with a small group of students, Bourdieu undertook a series of studies across an Algeria that was tightly patrolled by the army, leading him to discover the shocking reality of the resettlement camps and to analyse the mechanisms of destruction of Algerian society of which they were emblematic. To achieve the objectives he had set himself, Bourdieu had to carry out a genuine intellectual conversion, acquiring an ethnographic understanding of Algerian society, learning sociological analysis at a breakneck pace and inventing new instruments - both theoretical and empirical - that would enable him to understand the relations of domination specific to colonialism. These new tools also enabled him to analyse the nature of the crisis that the war had both produced and manifested. This unique volume brings together the first texts written by Bourdieu in the midst of the Algerian conflict, as well as later writings and interviews in which he returns to the topic of Algeria and the decisive role it played in the development of his work.

Book excerpt: Much orthodox economic theory is based on assumptions which are treated as self-evident: supply and demand are regarded as independent entities, the individual is assumed to be a rational agent who knows his interests and how to make decisions corresponding to them, and so on. But one has only to examine an economic transaction closely, as Pierre Bourdieu does here for the buying and selling of houses, to see that these abstract assumptions cannot explain what happens in reality. As Bourdieu shows, the market is constructed by the state, which can decide, for example, whether to promote private housing or collective provision. And the individuals involved in the transaction are immersed in symbolic constructions which constitute, in a strong sense, the value of houses, neighbourhoods and towns. The abstract and illusory nature of the assumptions of orthodox economic theory has been criticised by some economists, but Bourdieu argues that we must go further. Supply, demand, the market and even the buyer and seller are products of a process of social construction, and so-called ‘economic' processes can be adequately described only by calling on sociological methods. Instead of seeing the two disciplines in antagonistic terms, it is time to recognize that sociology and economics are in fact part of a single discipline, the object of which is the analysis of social facts, of which economic transactions are in the end merely one aspect. This brilliant study by the most original sociologist of post-war France will be essential reading for students and scholars of sociology, economics, anthropology and related disciplines.
