Dans les chapitres suivants ils expliquent ensuite comment ces trois systèmes influent sur la performance du secteur pharmaceutique au cours des étapes successives de la sélection de l'achat de la distribution et de la prescription de médicaments. Pour chacune de ces étapes ils décrivent les objectifs d'une politique pharmaceutique nationale, les moyens d'atteindre ces objectifs et les structures ainsi que les dispositions organisationnelles nécessaires à cette fin. Le chapitre consacré à la sélection traite des stratégies qui permettent de supprimer les médicaments sans intérêt thérapeutique de réduire le nombre des médicaments et d'accroître l'efficacité des médicaments disponibles. Il présente aussi des méthodes pour mesurer les coûts et l'efficacité des médicaments et les illustre par des exemples tirés de différents pays. Le chapitre 2 explique les avantages et les inconvénients de trois stratégies courantes d'acquisition : la confiance aveugle la défiance systématique et la coopération. Les expériences nationales servent à démontrer que certaines stratégies d'achat peuvent avoir des répercussions immédiates et à plus long terme sur le coût des médicaments. La distribution fait l'objet du chapitre 3 où sont examinés les aspects de l'acessibilité géographique physique et économique des médicaments. Une attention particulière est portée aux avantages et aux inconvénients comparatifs de certaines stratégies visant à financer la consommation de médicaments et à prévenir les ruptures de stock les surplus et les pertes de médicaments.
