Real-world examples in data interpretation prepare readers for the OSCE paper in the primary exam, and for the structured vivas in the final exam for the Fellowship of the Royal College of Anaesthetists. Emphasizes the type of data that anaesthetists encounter in general anaesthesia, the respiratory system, cardiology, orthopaedics and trauma, intensive care, paediatrics, pain, obstetrics, and neurosurgery. Presents questions with complete answers.

Book excerpt: The anesthesia and intensive care in infants and children have assumed positions of primary importance in contemporary medicine. In these delicate sectors of medicine, clinical research activities must be continually supported by interdisciplinary collaborations. Neonatal and pediatric intensive care requires that all involved physicians, including the surgeon and anesthesiologist, be willing to work together as a team. However, coordination of the various pathophysiological and clinical aspects of neonatal and pediatric critical medicine is highly complex. A complete and current review of the anaethesia and intensive care of infants and children must include discussions of morbidity rates, so as to guide the clincians in choosing the diagnostic approach, as well as of monotoring techniques appropriate to support the therapeutic decision. Recent progress in intensive care medicine for infants and children has been significant.

Book excerpt: Recent technological innovations - influenced primarily by the development of more sophisticated, faster and cheaper computer systems - permitted also the evolution of more affordable systems for Patient Data Management, so called PDM-Systems. The experience of the authors, on one of the first PDMS installation sites in Europe, shows that the purchase of such a system is not an easy task, since accurate data are not available in a comparable format. Therefore the first part of the book is devoted to a comparison of already installed, commercially distributed bedside based PDMS with regard to their specifications, functions and performance. The methods included a questionnaire with detailed questions for the vendors to answer and a "table of functions” comparing the most important functions which should be included in a PDMS. With that list the different systems (which were all in clinical use) were checked for the availability and the way of use of these functions. To evaluate variations in the systems performance an "information retrieval test” was designed and executed. In the second part the different vendors, whose systems were included in the study, were to describe the systems from their viewpoints. The third part contains papers describing the users’ experiences. The fourth and last part shows how to use PDMS-data for scientific and therapeutic purposes including two papers on clinical expert systems. Thus, this book provides valuable information for clinicians and hospital managers who have to decide on the purchase of a Patient Data Management System.

Book excerpt: In modern medicine the most functional methodologies are taking an increasing importance in spreading informations, rendering it credible while at the same time using reliable data to focus interaction between basic science and clinical medicine. Critical care medicine embraces these needs and more than any other discipline thrives and develops thanks to interdisciplinary contact. APICE 2004 has been organised to provide precise answers to these issues. In particular, considerable emphasis has been given to the reviews regarding the most important aspects - or the most significant clinical developments - in the sectors involving variety of functions: neurological, respiratory and cardiovascular, gastrointestinal, metabolism and perfusion; trauma infections, sepsis and organ failure; perioperative medicine and life support techniques; information technology dedicated to clinical medicine, but also as a means of information and education. The contributing authors are all part of leading research groups at the international level in the various sectors presented in the volume.

Book excerpt: Innovation, training and research are the pillars that support a process deriving from basic science and multi-professional/multidisciplinary interventions. The APICE 2012 yearbook deals with several innovations for optimising prevention and management measures for the critically ill, by integrating diagnostic procedures with pharmacological and technological options. Peri- and postoperative managements as well as treatment of surgical infections and of pain, new and old artificial ventilation strategies are some of the most relevant topics the volume deal with in this new edition. The volume focuses also on the improvement standards and quality of care; on the expanding concept of clinical governance and professionalism and on the importance of ethical principles for establishing a process of patient-centered and evidence-based care.

Book excerpt: There is a tendency of an increasing number of signals and derived variables to be incorporated in the monitoring of patients during anesthesia and in intensive care units. The addition of new signals hardly ever leads to thedeletion of other signals. This is probably based on a feeling of insecurity. We must realize that each new signal that is being monitored brings along its cost, in terms of risk to the patient, investment and time. It is therefore essential to assess the relative contribution of this new signal to the quality of the monitoring process; i. e. given the set of signals already in use, what is the improvement when a new signal is added? Beyond a certain point the addition of new information leads to new uncertainty and degrades the result (Ream, 1981) In the diagnostic process, it is possible to evaluate "result" in an objective, qualitative way. The changes in the sensitivity and specificity of the diagnosis as a result of the addition or deletion of a certain variable can be calculated on the basis of false negative, false positive, correct negative and false negative scores. Different methods for multiple regression analysis have been implemented on computers (Gelsema, 1981) which can support such decision processes. In monitoring, the situation is much more complex. Many definitions of monitoring have been given; the common denominator is that monitoring is a continuous diagnostic process based upon a (semi)continuous flow of information. This makes simple assessment methods useless.

Book excerpt: In April of 1991, 425 partICIpants from 18 countries met in Hamamatsu in Japan for the 6th International Symposium on Computing in Anesthesia and Intensive Care (lSCAIC). The meeting was one of the most spectacular academic and fruitful in the history of ISCAIC. We had four days of fascinating presentations and discussions covering many areas of technology in Anesthesia and intensive care. New technologies were presented and old technology reexamined. The measures of success of the meeting were the excellent research material in oral and poster presentations, and state of the art reviews of the latest issues by distinguished worldwide key speakers. It must be sure that the meeting was most effective to promote and disseminate up-to-date information in these fields across the participating countries. The aim of this book is to record the exciting achievements of the meeting and extend them further among our colleagues. We hope the readers of this book will share the same excitation as well as the latest information in this speciality. Finally we would like to extend our deepest gratitude to all participants and others for the contribution to the compilation of this book. Kazuyuki Ikeda, M.D.

Book excerpt: Designed for candidates sitting the primary FRCA examination, this book brings together exam questions from recent years and structures them into six practice papers. The format of 90 questions per paper echoes the exam itself. Following each paper a scoring chart and detailed explanations of answers are provided. The questions cover physiology, pharmacology, physics, clinical measurement and statistics as they appear in the primary FRCA. There are questions on all recently added exam topics, and those that now carry stronger emphasis and more weight such as resuscitation, sepsis and trauma. The latest drugs, equipment, monitoring techniques and safety procedures are referenced. Trainees will find this an invaluable tool for exam preparation, whether sitting the FRCA in the UK, through the London College external examiners in many Asian and African countries, or similar exams for anaesthetists in Australasia and North America.

Selected using a rigorous methodology, the studies cover topics ranging from pain medicine, critical care, cardiothoracic anesthesiology to general anesthesiology. For each study, a concise summary is presented with an emphasis on the results and limitations of the study, and its implications for practice. Brief information on other relevant studies is provided, and an illustrative clinical case concludes the review. This book is a must-read for health care professionals in anesthesiology and pain medicine, and anyone who wants to learn more about the data behind clinical practice in anesthesiology, pain medicine, critical care and its broad subspecialties.
