Book excerpt: One of the most puzzling and striking features of many of the genetically determined progressive neuromuscular diseases such as the spinal muscular atrophies and the muscular dystrophies is that muscular wasting and weak ness in these cases is curiously selective, at least in the early stages, pick ing out certain skeletal muscles and sparing others. The diagnosis of these conditions has largely depended in the past upon the recognition of specific patterns of involvement of individual muscles and muscle groups, taken along with information derived from the mode of inheritance within the in dividual family and the results of special investigations. The investigations of most value have proved to be serum enzyme studies, electromyography and related techniques, and muscle biopsy. The advent of CT scanning has, however, introduced a new dimension; as the authors of this interesting monograph have clearly demonstrated, it is now possible, using the whole body scanner, to define patterns of muscular atrophy in the limbs and trunk much more precisely than by any other method. Not only does this techni que demonstrate which muscles are involved, but the changes in relative density provide useful information about the severity of the process and about the progress of the disease if the studies are performed serially. This monograph is pleasantly written and most attractively illustrated.

Book excerpt: This Open Access book presents practical approaches to managing patients affected by various rheumatological diseases, allowing readers to gain a better understanding of the various clinical expressions and problems experienced by these patients. Discussing rheumatology from an organ systems perspective, it highlights the importance ofdetailed musculoskeletal examinations when treating patients affected by rheumatological diseases. The book first explores the latest diagnostic approaches and offers key tips for accurate musculoskeletal examinations before addressing the various treatment modalities, with a particular focus on the most common joints involved in rheumatoid arthritis: the wrists and the metacarpophalangeal joints (2nd and 3rd). Featuring easy-to-understand flow diagrams and explaining the common medical problems associated with rheumatic disease, such as shortness of breath and anemia, it is not only a valuable resource to rheumatologists, but will also appeal to medical students, junior residents, and primary healthcare physicians.

Available in PDF, EPUB and Kindle. Book excerpt: Polymyositis and Dermatomyositis provides extensive information regarding Polymyositis and Dermatomyositis (PM/DM), which is described as a heterogeneous disease complex. This book is divided into four sections: Part I (Clinical Features) covers the classification of PM/DM, details of the clinical presentation, and the disease's association with the other connective tissue disorders and malignancies. Part II (Etiology and Mechanisms) covers advances in the immunopathology and viral etiology of PM/DM along with a frequently recognized entity: inclusion body myositis. Part III (Diagnosis and Treatment) covers the histologic, muscle enzyme histochemical, electron microscopic, and resin histology features of PM/DM along with those electromyographic features that could help make a more accurate diagnosis. Part IV (Overview) summarizes the issues that may not have been clear and highlights differing and unsettled views or present available data. This text is directed to clinicians in private practice or in academic institutions concerned with PM/DM patients, including neurologists, rheumatologists, pediatricians, dermatologists, physiatrists, and neuromuscular investigators. This book is intended as well for neuromuscular pathologists who interpret muscle biopsy specimens and electromyographers who perform EMG studies to help determine the clinical diagnosis. Researchers in immunology and immunopathology of neuromuscular diseases will find discussions in this book invaluable.

Book excerpt: Muscle weakness with ageing is almost inevitable, generally beginning to manifest beyond the age of 40, and is usually unstoppable. It can lead to reduced mobility, increased risk of falling, injury, and even death. But “you’re just getting old” is not a sufficient diagnosis. Specific causes of neuromuscular symptoms may explain progressive muscle weakness, and should be investigated for potential treatment. Muscle Ageing, Inclusion-Body Myositis and Myopathies explores the clinical and pathological expression of muscle weakness in aging persons. Case studies demonstrate how physicians can more accurately diagnose weakening elderly patients and make better management decisions. It also explores sporadic inclusion-body myositis and hereditary inclusion-body myopathies. The former, the most common progressive muscle disease in the over 50s, is frequently under-diagnosed and, with the increasing population of aged individuals, is presenting a greater challenge. This disease of muscle has pathological similarities with the well-known Alzheimer and Parkinson brain diseases. Edited and written by a leading international cast of authors, Muscle Ageing, Inclusion-Body Myositis and Myopathies provides a state-of-the-art guide to ageing-associated neuromuscular disorders. It should be in the hands of all those involved in the care of aging and muscle-weakened patients. Book excerpt: The muscular dystrophies are an important group of inherited disorders. They are characterized by muscle wasting and weakness, but vary considerably in their clinical manifestations and severity. This text reviews our understanding of the most important of these disorders. In many instances, the genes and protein products responsible for the dystrophies have been identified and it is now possible to establish a precise diagnosis, detect preclinical cases, identify carriers and offer prenatal dianostic testing. The book goes on to describe the opportunities for management of the symptoms through respiratory care, physiotherapy and surgical correction of contracture, and examines the potential, in the future, for effective treatment utilizing the new techniques of gene and cell therapy. Professor Emery has invited chapters from the leading international experts in the field providing a unique insight into the current situation and the hopes for the future.
