Book excerpt: In clinical practice, all physicians have to deal with patients suffering from chronic kidney disease. Large numbers of these patients suffer from mild conditions, but nevertheless, they deserve special attention because inappropriate management may accelerate kidney function deterioration. On the other side, patients affected by end-stage renal disease require replacement therapies and often represent a challenge for clinicians due to their complex pathophysiology. Therefore, an adequate knowledge of the most important clinical and therapeutic aspects of renal failure is an essential requirement for every doctor, especially if we consider the increasing incidence and prevalence of this condition. The book summarises the most important etiopathogenetic and pathophysiologic aspects of chronic kidney disease and focuses the attention on important emerging topics: nutritional and dietary management, renoprotective interventions, new evidence about pathophysiological mechanisms provided by base research, innovations in pharmacological treatment, and strategies to improve patient’s quality of life. Data are reported in clear and concise language, supported by graphics, tables, and pictures that facilitate the comprehension of all the arguments.

Available in PDF, EPUB and Kindle. Book excerpt: Known worldwide, chronic kidney disease (CKD) is a disease that affects up to 4% of the population with increasing figures also in the developing countries. Life expectancy of patients affected by CKD is shortened compared to the overall population, and only a minority of patients reach end-stage renal disease (ESRD) with the need for dialysis or renal transplantation; death overtakes dialysis. In the 13 chapters, this book sheds light on the different aspects related to pathophysiology and clinical aspects of CKD, providing interesting insights into not only inflammation and cardiovascular risk but also the interplay of hormones and the functional aspects of endothelial function. In addition, chapters dealing with genetic aspects of polycystic kidney disease and also the clinical handling of patients with CKD and peritoneal dialysis will be beneficial for the open-minded reader.

Book excerpt: Abstract: In order to change the current state of chronic kidney disease knowledge and therapeutics, a fundamental improvement in the understanding of genetic and environmental causes of chronic kidney disease is essential. This article first provides an overview of the existing knowledge gaps in our understanding of the genetic and environmental causes of chronic kidney disease, as well as their interactions. The second part of the article formulates goals that should be achieved in order to close these gaps, along with suggested timelines and stakeholders that are to be involved. A better understanding of genetic and environmental factors and their interactions that influence kidney function in healthy and diseased conditions can provide novel insights into renal physiology and pathophysiology and result in the identification of novel therapeutic or preventive targets to tackle the global public health care problem of chronic kidney disease

Book excerpt: This book provides a comprehensive and systematic review of the latest findings in a wide spectrum of clinically important aspects of chronic kidney disease (CKD), focusing on clinical diagnosis and therapeutics. CKD is a global health problem with a rising morbidity and mortality. The last decade has seen significant improvements in determining the incidence, prevalence, and complications of CKD, mainly thanks to the definitions of CKD developed by the National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (K/DOQI). However, increased recognition of CKD has led to awareness of the limitations of its clinical diagnosis and treatment, which are essential to patients’ wellbeing. This book is of particular value not only to nephrologists, but also to general practitioners and residents with an interest in CKD. It offers a well-organized exposition of the current knowledge base. Compared with previously published books on kidney disease and CKD, it has a smaller number of more concise chapters. As a result, readers can easily obtain an overview of the most important topics in CKD. We hope that practitioners will gain as much from reading this practical guide to clinical management of CKD as we have from editing it.

Available in PDF, EPUB and Kindle. Book excerpt: This book systemically presents the latest research on renal fibrosis, covering all the major topics in the field, including the possible mechanisms, biomarkers, and strategies for prevention and treatment of chronic kidney disease (CKD). Due to its high prevalence, CKD represents a huge global economic and social burden. Irrespective of the initial causes, CKD progresses to end stage kidney disease (ESKD) due to renal fibrosis, which is characterized by glomerulosclerosis, tubule atrophy and atresia, and the excessive accumulation of extracellular matrix (ECM) in the kidney. Unfortunately, an estimated 1%-2% of the adult population living with CKD will need renal replacement therapy at some point as a result of ESKD. As such, strategies for preventing or slowing CKD progression to ESKD are of utmost importance, and studies aiming to understand the mechanisms of renal fibrosis have been the focus of intensive research. Recently, novel insights into the pathophysiological processes have furthered our understanding of the pathogenesis of renal fibrosis, and more importantly, promoted studies on the early diagnosis and treatment of CKD. This book draws lessons from the extensive, state-of-the-art research in this field, elaborating the new theories and new techniques to offer readers a detailed and comprehensive understanding of renal fibrosis and as well as inspiration for future research directions.

Available in PDF, EPUB and Kindle. Book excerpt: The present book contains the Proceedings of a two day Symposium on Uremic Toxins organized at the University of Ghent in Belgium. A series of guest lectures, free communications and posters have been presented. An international audience of 163 scientists from 16 nationalities listened to and discussed extensively a spectrum of topics brought forward by colleagues and researchers who worked for many years in the field of Uremic Toxins. There is a striking contrast between all the new dialysis strategies available in the work to "clean" the uremic patients and the almost non-progression of our knowledge on uremic toxins in the past decade. In this sense the symposium was felt by all participants as a new start for the research in the biochemical field of the definition of uremia. If the present volume would stimulate new work in this field in order to define uremia, or identify the uremic toxins, the purpose of the organizers would be maximally fulfilled.

Book excerpt: Associated with both acute kidney injury (AKI) and cardio-renal syndromes (CRS), new biomarkers represent both a popular area of investigation and a new opportunity for advancement of therapy. This book contains the resolutions of the most recent ADQI conferences on biomarkers in AKI (Dublin) and on cardio-renal syndromes (Venice). The first part answers specific questions about new biomarkers and their use and utility in AKI: What are the most suitable candidate molecules and physiologic measures, how solid and evidence based is the discovery phase? How can we incorporate the new biomarkers in the AKI conceptual model describing the evolution from susceptibility to insult, decreased GFR and organ death? Even if we have a positive biomarker pattern and we can identify patients at risk or patients with early or even subclinical AKI, how is this information affecting our clinical behavior and practice? The second part is dedicated to the appraisal of the current knowledge about the pathophysiological mechanisms involved in different forms of CRS: it contains contributions on the state-of-the-art knowledge and practice of CRS, particularly focusing on the pathophysiology of the five subtypes. Acute and chronic mechanisms of damage are explored in depth, with particular attention to the primacy of organ involvement and the subsequent pathways of organ crosstalk.Presenting the most recent research in the field of biomarkers, AKI and CRS, this publication is an important educational tool for advanced investigators and clinical experts, but also for students and fellows.

Book excerpt: Diabetes is a metabolic disorder of chronic hyperglycemia that causes numerous complications. The International Diabetes Federation and World Health Organization report that approximately 537 million people worldwide have diabetes in 2021, which may increase to 700 million by 2045. Diabetes and its complications significantly lower the quality of life of patients and cause more than 4 million deaths yearly. Diabetes and its complications also increase premature mortality, increasing society's economic burden. Numerous studies reveal that the chronic multisystem injury and dysfunction can be caused by diabetes, including cardiac and cerebral vascular lesions, nephropathy, retinopathy and neuropathy. In addition, diabetes usually co-occurs with other diseases that may have certain risk factors in common. For example, patients with diabetes are more likely to have hypertension, chronic obstructive pulmonary disease, or asthma. However, there is still a lack of knowledge on the relationship between diabetes and other potential comorbidities such as hepatitis infection, hearing impairment and periodontal disease. Furthermore, common risk factors contributing to the pathogenesis of diabetes and comorbidities are far from fully understood. Several comorbidities (e.g., cardiovascular and cerebrovascular diseases) raise the extra chances of death in patients with diabetes, but the association between diabetes and mortality of other diseases, such as endometrial or kidney cancer, is inconsistent. Further research is still needed to reveal the excess mortality associated with diabetic comorbidities.
