Book excerpt: Basic scientific background Breast cancer is one of the most common cancer and the most frequent cause of cancer death among women worldwide. Currently, subtyping breast cancers into hormone receptor (HR) positive, human epidermal growth factor receptor-2 overexpressing (HER2+), and triple negative breast cancer (TNBC) is the basis of diagnosing and treating this disease. The main treatment strategies for breast cancer include surgery, endocrine therapy, molecular targeted therapy, chemotherapy, radiotherapy, immunotherapy and gene therapy. However, resistance of breast cancer cells to chemotherapeutic agents, molecular targeted therapies and immunotherapy may occur either intrinsically or de nova, and is often ultimately responsible for treatment failure. Therefore, drug resistance poses a major challenge to breast cancer treatment. Current developments: Drug resistance in breast cancer is a complex clinical condition originating from a wide range of molecular alterations. The development of endocrine therapy resistance is believed to be associated with many cellular changes, such as ESR1 gene mutations, bypassing estrogen signaling pathway and altered tamoxifen metabolism. Meanwhile, changes in immune response, alternation of drug-binding property and downstream pathways are involved in the mechanisms of drug resistance in HER2+ breast cancer. In addition, resistance to chemotherapeutic agents predominantly arises from increased drug efflux and cross resistance. Current studies suggest that treatment strategies and therapeutics have to be designed specifically to each patient in different clinical situations. The use of modern genomic, proteomic and functional analytical techniques has contributed to identify novel genes and signaling networks involved in breast cancer drug resistance. Moreover, the use of high-throughput techniques in combination with bioinformatics and systems biology approaches has aided the interrogation of clinical samples and allowed the identification of molecular signatures and genotypes that predict responses to certain drugs. Despite much progress has been made in the field of breast cancer drug resistance, such as combination therapy and drug-loaded nanoparticles, the complexity and variability of drug resistance mechanism still inevitably lead to the continuous occurrence of drug resistance. Therefore, with the increasing amounts of anti-breast cancer agents, there are now unprecedented opportunities to understand and overcome drug resistance through further research into mechanisms and corresponding strategies, which will help achieve lasting disease control and bring survival benefits to patients with advanced cancer. Therefore, a better understanding of the molecular events in the metastatic process is critical. Several reports have described correlation of hyaluronan (HA) with initiation and progression of various types of epithelial cancers. The HA synthase (HAS) isoforms encode the enzymes that produce and deposit HA while the hyaluronidase (HYAL) genes code for enzymes that degrade HA and their expression is dysregulated in various tumors as a result of transcriptional and epigenetic changes that accompany progression. In this report, we examined the expression of HAS1, HAS2 and HAS3, HYAL1 and HYAL2 and LYVE1 in mammary tumor cell with different metastatic potential (4T1, highly metastatic; Cl66, moderately metastatic; Cl66M2, low metastatic). We observed increased expression of HAS1, HAS2 and HAS3 as well as HAYL 1 and HAYL2 in aggressive mammary tumor cells. Next we stably knocked-down HAS2 and HAS3 expression and overexpressed LYVE1 in 4T1 cells and analyzed cell proliferation, migration, tumor growth and metastasis. We observed that Has2/Has3 knock-down and LYVE1 overexpression modulated mammry tumor growth and spontaneous metastasis. Together, these data demonstrate an important role of HAS2 and LYVE1 in mammary tumor growth, angiogenesis, invasion, and metastasis.

The goal was to identify and begin characterizing the gene(s) responsible. Two approaches were proposed - (1) introduction of smaller pieces of chromosome 11 into cells with assessment of metastatic potential; and (2) identification of differentially expressed mRNA in metastasis suppressed cells. Progress using Approach #1 was not successful. Approach #2 was successful. We identified three novel cDNAs using differential display which were more highly expressed in the metastasis-suppressed neo11/435 hybrids. Moreover, one of those candidates, BRMS1, significantly suppressed metastasis in two human breast carcinoma cell lines when transfected and constitutively expressed. BRMS1 1 maps to 11q13, a site commonly involved in late-stage breast carcinoma. The mechanism of action appears to be novel, but was not determined during the funding period. In summary, we accomplished the stated ultimate objective for DAMD-17-96-1-6152. Results generated from DAMD-17- 96-1-6152 were used to successfully compete for funding from the National Cancer Institute to follow-up on these exciting findings.

The results presented in this thesis show that drug-resistant MCF-7/DOX cells survive high doses of radiation exposure better than MCF-7 cells. Moreover, the chemo- and radioresistance of MCF-7/DOX cells share common molecular mechanisms and loss of sensitivity to radiation in chemo-resistant cells may be explained by alterations in their DNA methylation profile. The results of experiments presented in this thesis may, therefore, serve as a first step for future analysis of tumour resistance to radio- and chemotherapy and for the development of novel epigenetic strategies for reversal of breast cancer resistance to cytotoxic treatment regimens.

Book excerpt: Breast cancer is the most common malignancy and second most common cause of cancer-related death in women, a clinical challenge exasperated by the lack of targeted therapies for metastasis, the most deadly component of breast cancer. Although the molecular mechanisms underpinning the dissemination of primary breast cancer cells to distant tissues remain incompletely understood, long noncoding RNAs (lncRNAs) have emerged as potent regulators of breast cancer development and progression, including the metastatic spread of disease.Through in silico and biological analyses, we identified a novel lncRNA, BMP/OP-Responsive Gene (BORG), whose expression directly correlates with aggressive breast cancer phenotypes, as well as with metastatic competence and disease recurrence in multiple clinical breast cancer patient cohorts. Mechanistically, BORG elicits the metastatic outgrowth of latent breast cancer cells by promoting the localization and transcriptional repressive activity of TRIM28, which binds BORG and induces substantial alterations in carcinoma proliferation and survival. Moreover, inhibiting BORG expression in metastatic breast cancer cells impedes their metastatic colonization of the lungs of mice, implying that BORG acts as a novel driver of the genetic and epigenetic alterations that underlie the acquisition of metastatic and recurrent phenotypes by breast cancer cells.Interestingly, the BORG-dependent transcriptome demonstrates enhanced signaling through survival and viability pathways, as well as decreased activation of cell death pathways. Accordingly, we determined that BORG expression is broadly promoted by environmental and chemotherapeutic stresses, a transcriptional response that facilitates the survival of breast cancer cells. These effects rely upon a BORG-induced induction of NF- B signaling as part of a feed-forward NF- B signaling loop, as well as its ability to bind and activate RPA1.Taken together, our mechanistic insights into BORG-mediated metastasis and chemoresistance offer the foundation necessary to develop therapies that target the molecular drivers vital to metastatic outgrowth of disseminated breast cancer cells, particularly those persisting in a dormant state.
