Book excerpt: There hasn’t been a better time to be a drug developer for immune-based therapies than the past couple of decades. We have seen an explosion in immune-based therapies for cancer, autoimmune and infectious diseases, metabolic diseases and diseases and disorders of the nervous system. The modalities of these immune-based therapies span small molecules, biologics, and gene and cell therapeutic approaches. Significant advances have been made in optimizing drug design for its specificity for the target, characterizing the mode of action in in vitro assays, and ensuring safety and manufacturability. However, an area of challenge that remains is identifying animal models for evaluating efficacy and pharmacokinetics/pharmacodynamics relationship that are predictive of drug effects in humans. Discussion on this topic is warranted as examples of failures of translation from animal models to humans provide us an opportunity to learn more about human biology.

Book excerpt: Recycling is an act of collecting and processing items that would otherwise be discarded as waste in order to create a new product. Recycled material is being used in an increasing number of today’s products. Waste management is primarily concerned with a wide range of wastes, including industrial, biological, household, municipal, organic, biomedical, and radioactive wastes. Human activity, such as the mining and processing of basic resources, generates waste and poses health problems that can emerge both indirectly and directly. Waste mismanagement is a serious problem on an individual and a governmental level. Nowadays, the waste disposal business is struggling to adapt to globalized consumerism, a system in which things are manufactured on one continent, purchased and used on another, and disposed of on yet another. Therefore, remediation is often subject to a variety of legal criteria, but it can also be based on evaluations of human health and environmental concerns in cases where no statutory standards exist or when standards are advisory. This book discusses recycling strategies and technologies to find solutions to waste management. Chapters address such topics as biodegradable waste, the circular economy, managing industrial and nuclear waste, and much more.

This book was released on 2012-12-20 with total page 159 pages. Available in PDF, EPUB and Kindle. Book excerpt: In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment.

Book excerpt: On March 19, 2014, the National Academies of Sciences, Engineering, and Medicine held a workshop on the topic of the sharing of data from environmental health research. Experts in the field of environmental health agree that there are benefits to sharing research data, but questions remain regarding how to effectively make these data available. The sharing of data derived from human subjects-making them both transparent and accessible to others-raises a host of ethical, scientific, and process questions that are not always present in other areas of science, such as physics, geology, or chemistry. The workshop participants explored key concerns, principles, and obstacles to the responsible sharing of data used in support of environmental health research and policy making while focusing on protecting the privacy of human subjects and addressing the concerns of the research community. Principles and Obstacles for Sharing Data from Environmental Health Research summarizes the presentations and discussions from the workshop.

Book excerpt: The Institute of Medicine study Crossing the Quality Chasm (2001) recommended that an interdisciplinary summit be held to further reform of health professions education in order to enhance quality and patient safety. Health Professions Education: A Bridge to Quality is the follow up to that summit, held in June 2002, where 150 participants across disciplines and occupations developed ideas about how to integrate a core set of competencies into health professions education. These core competencies include patient-centered care, interdisciplinary teams, evidence-based practice, quality improvement, and informatics. This book recommends a mix of approaches to health education improvement, including those related to oversight processes, the training environment, research, public reporting, and leadership. Educators, administrators, and health professionals can use this book to help achieve an approach to education that better prepares clinicians to meet both the needs of patients and the requirements of a changing health care system.

This book was released on 2012-03-21 with total page 78 pages. Available in PDF, EPUB and Kindle. Book excerpt: The "homeland" security mission of the Department of Homeland Security (DHS) is paradoxical: Its mission space is uniquely focused on the domestic consequences of security threats, but these threats may be international in origin, organization, and implementation. The DHS is responsible for the domestic security implications of threats to the United States posed, in part, through the global networks of which the United States is a part. While the security of the U.S. air transportation network could be increased if it were isolated from connections to the larger international network, doing so would be a highly destructive step for the entire fabric of global commerce and the free movement of people. Instead, the U.S. government, led by DHS, is taking a leadership role in the process of protecting the global networks in which the United States participates. These numerous networks are both real (e.g., civil air transport, international ocean shipping, postal services, international air freight) and virtual (the Internet, international financial payments system), and they have become vital elements of the U.S. economy and civil society. Export Control Challenges Associated with Securing the Homeland found that outdated regulations are not uniquely responsible for the problems that export controls post to DHS, although they are certainly an integral part of the picture. This report also explains that the source of these problems lies within a policy process that has yet to take into account the unique mission of DHS relative to export controls. Export Control Challenges Associated with Securing the Homeland explains the need by the Department of Defense and State to recognize the international nature of DHS's vital statutory mission, the need to further develop internal processes at DHS to meet export control requirements and implement export control policies, as well as the need to reform the export control interagency process in ways that enable DHS to work through the U.S. export control process to cooperate with its foreign counterparts.

Book excerpt: The "VOLGA" conferences, hosted in odd-numbered years by the Department of Theoretical and Experimental Reactor Physics of the Moscow Engineering Physics Institute (MEPhI), are some of the most prestigious technical meetings held in Russia. Traditionally, these conferences present the opportunity for reactor physicists from around the world to gather at MEPhI's holiday camp on the banks of the Volga river (near Tver) to exchange ideas and explore innovative concepts related to nuclear power development. In 1997, NATO became involved in the "VOLGA" meetings for the first time by co-sponsoring "VOLGA97" as an advanced research workshop. This workshop broke with tradition a bit in that the venue was moved from MEPhI's holiday camp to a location nearer Moscow. The workshop program was effectively organized in order to cover a broad range of topics relating to the theme of the meeting. Generally, the papers concerned safety related questions associated with utilizing both weapons-grade and reactor-grade plutonium in the nuclear fuel cycle, including facility requirements, licensing issues, proliferation risks, and a variety of advanced concepts for alternative fuel cycles. The program contained a total of ninety-nine papers presented in five days of sessions.
