Pharmaceutical interventions for brain trauma, in general, met with limited success and are often compounded by serious side effects. Recent research and technology on stem cell transplantation have opened up a novel means for ONS repair. This study aims to investigate the therapeutic potential of neural differentiation of stem cells from umbilical cord blood for neural repair following TBI. The present results have shown an in vitro differentiation of stem cells into neural lineages. These cells expressed neural markers nestin, Map2, GFAP and ONPase as detected by immunohistochemistry. Animals that received neural stem cells showed improved neurological functions when compared with control groups. This study has shown the differential and therapeutic potential of stem cells from umbilical cord blood which may be used as an alternative or a complementary therapy for traumatic brain injury.

Cell therapy, however, remains the most compelling treatment for neurodegenerative diseases, disorders, and injuries, including Parkinson’s disease, Huntington’s disease, traumatic brain injury, and stroke, which is expanded upon in more detail in Chapter 1 by Snyder and colleagues. Cell therapy is also unique in that it is the only therapeutic strategy that strives to replace lost, damaged, or dysfunctional cells with healthy ones. This repair and replacement may be due to an administration of exogenous cells itself or the activation of the body’s own endogenous reparative cells by a trophic, immune, or inflammatory response to cell transplantation. However, the precise mechanism of how cell therapy works remains elusive and is c- tinuing to be investigated in terms of molecular and cellular responses, in particular. Book excerpt: Stroke and traumatic brain injury (TBI) are leading causes of death and long-term disability worldwide. Despite the socioeconomic burden of these neural injuries, there are no restorative therapies. Induced pluripotent stem cell-derived neural stem cells (iNSCs) may be the critically needed therapy to restore neurological function after brain injury. Preclinical studies demonstrate that iNSCs are a multimodal therapeutic that improves recovery after neural injury. However, the enthusiasm of these results has been stifled by the hundreds of therapeutics shown to be effective in preclinical neural injury models but have failed in clinical trials, thus suggesting a lack of translatability of preclinical animal models to human patients. Compared to the rodent, the sole preclinical model iNSC therapy has been tested in, the pig possesses striking similarities in brain anatomy and physiology compared to the human and may be an ideal animal model to bridge the gap between preclinical rodent models and human patients. However, injury models and outcome measures are not well characterized in the pig, and more studies in the pig are needed to determine if large animals truly carry greater predictive power of clinical efficacy. The objectives of these studies were to 1) develop a translational pig model to study TBI pathophysiology and recovery mechanisms, and 2) assess whether iNSC therapy improves recovery in a pig stroke model. These studies strengthen current preclinical therapeutic development strategies for neural injury by improving the translational capacity of animal models and provide further insight into the potential of iNSC therapy for human patients.
