However, although dental implant can achieve a fair rate of survival, still, some of them fail. The risk for implant failures is not evenly distributed among patients, which indicates that there are biological, mechanical, or behavioral risk factors that can cause implant failure. Among biological risk factors, drugs taken by patients are of special interest, due to the fact that most implant patients are elder adults that are often polymedicated.The success of implants relies mainly on osseointegration, a direct functional and structural interlocking between implants and bone. The process of osseointegration around implants is similar to the biological events occurring during bone repair and fracture healing. Dysregulation of bone metabolism is known to have a negative impact on bone healing and implant osseointegration. Many of the patients receiving implant treatments are taking different kinds of medications. Some drugs interfere with biological processes involved in bone metabolism.We hypothesize that medications interfering with bone metabolism could influence implant survival. The aim of our research is to identify risk associated with implant failure. Some example of such drugs are antihypertensive medications, selective serotonin reuptake inhibitors (SSRIs), and proton pump inhibitors (PPIs), etc., which are commonly prescribed for patients suffering from highly prevalent conditions such as hypertension, depression, and gastric acid reflux, respectively. The impact of these drugs on osseointegration is unexplored and important to know whether they could affect bone, osseointegration, and dental implants. In fact, our research is looking into the relationship between pharmacology, osseointegration and dental implants. It also may help to open the door for new pharmacological innovation that might improve the success of osseointegrated implants, and avoid unnecessary complications. This research project is of great relevance in health care science in general, and in dentistry in particular, as well as for dental implant patients taking certain medications. Accordingly, we believe that this project is of great importance to healthcare researchers, practitioner and patients.The three retrospective cohort studies included a cohort of patients with osseointegrated dental implant placement at the East Coast Oral Surgery clinic in Moncton, Canada, from January 2007 to September 2015. Kaplan-Meier estimator was used to describe the hazard function of dental implant failure in terms of the use of SSRIs, antihypertensive drugs, and PPIs, respectively. Multilevel mixed effects parametric survival analyses were used to test the association between medication exposure and risk of implant failure adjusting for potential confounders. The findings suggest that treatment with SSRIs or PPIs may be associated with an increased risk of osseointegrated dental implant failure, whereas antihypertensive drugs usage is associated with higher survival rate of dental implants.The publication of "Selective Serotonin Reuptake Inhibitors and the Risk of Osseointegrated Implant Failure: A Cohort Study" has been the most-read article of 2014 in the Journal of Dental Research. It is estimated that every year, over two million implants are placed throughout the world. However, dental implants may exhibit immediate or delayed complications that could lead to implant failure. The aim of this study was to explore the factors coincidental with removal of failed implants at the UCSF Postgraduate Periodontology Clinic (PGPC) by reviewing electronic health records (EHR) for the last 19 years.Methods: EHR data from Axium were searched for the dental code D6100 (implant removal) from January 2001 - April 2019. Furthermore, code D7140 (simple tooth removal) was used to include cases that were suspected to be misclassified. Inclusion criteria was focused on patients who had dental implant(s) removed and had sufficient records for analysis. Patient and surgical data were gathered from dental records and analyzed. Exclusion criteria focused on patient data that was incomplete or not accessible.Results: Upon review of 3,426 simple tooth extractions and 81 implant explantations, it was determined that 114 implants were explanted from 88 patients over 19 years. Fifteen patients were removed from the data analysis based on exclusion criteria. Analysis was performed on 73 patients who had a total of 96 implants removed. Of these, 59% of patients had periodontal disease and 32% were smokers. Amongst the medical conditions reviewed, 32% of patients reported cardiovascular disease, 23% had a bone disorder, and 18% had emotional disorders. In terms of the implants, 41 implants were placed in grafted bone; 54% being allograft. Out of 96 implants removed, 58 were late failures and 33 implants failed before loading. Furthermore, we examined the reasons for tooth extractions at sites with subsequent implant failure and found that 6 teeth had endodontic treatment and 3/6 had periapical abscesses. Forty-five percent of cases that had a late failure did not report regular recall and maintenance appointments.Conclusion: Implant failure was coincidental with a history of periodontal disease and other risk factors, such as cardiovascular disease and emotional disorders. A lack of consistent maintenance recalls was frequently found in cases with late implant failure. Not every patient is a good candidate for implant therapy and risk factors have to be carefully evaluated and presented to the patient. This research highlights the need for a systematic evaluation of patients planning to receive implants and the need to inform them of possible complications and risks of implant treatment.

However, some failures are seen during the first year after implant placement. Previous studies have been related the history of periodontitis, smoking habit and a poor oral hygiene with a higher risk to develop periimplantitis. However, more research is necessary in this regard to identify and consequently control all the conditions that can affect the long-term success of implant therapy.The aim of this study was to establish a standardized method to evaluate the long-term results of MISu00ae implants (V3 and C1), in terms of survival, and to assess the risk factors associated with biological and technical complications. We aim to study the results of different surgical scenarios and patient-related factors and, therefore, identify their influence for the long-term stability of the treatment. A prospective cohort study with five hundred implants (C1 and V3 MISu00ae) was conducted in patients older than 18 years old scheduled for implant therapy. Each patient was carefully informed about the objectives of the study and the informed consent was signed. All the surgeries were performed by postgraduate students at the UIC (International University of Catalonia). Patient-related and surgical related factors of each patient were saved in our database to test their influence in the long-term survival of the implants After 1-year of follow-up, 4.2% of 501 implants had failed; which were considered early failures; without osseointegration of the implant. No prosthetic or biologic complications were found. History of periodontal disease was significantly related with higher implant failure (OR:3.00).

Book excerpt: While the systemic health consequences of silicone gel-filled breast implants have been explored in several large studies, little attention has been placed on factors associated with implant failure (defined as silicone gel on the outside of the implant and commonly referred to as implant rupture). This study explores woman-specific and implant-specific characteristics potentially associated with silicone breast implant failure. The study population consists of 441 women (807 implants) who received MRI examination to diagnose the integrity of their implants (i.e., failed versus intact) between 1992 and 1995 at the UCSD MRI Institute and who responded to a mailed survey in 1996 (79% response rate). Information included the woman's demographic characteristics and personal factors. Medical records provided information about the patient's implants and her implant surgery. Multivariate analysis using the Generalized Estimating Equation (GEE) approach was conducted in order to control for confounding and simultaneously take into account the correlation between factors from two implants within the same woman. Thirty percent of the implants had failed. Meme ME polyurethane-coated implants were identified as having a much greater risk of failure than other implants (86% compared to 27%), and failure occurred in this sub-group at a much younger implant age. Among non-Meme ME implants, increasing age and having had a previous implant were strong predictors of implant failure. Implants placed in the sub-muscular position were at higher risk of failure compared to sub-glandular implants, and smaller implants were more likely to fail than larger ones. Wearing a bra during exercise (always or most of the time) appeared to increase the risk of failure if the implants were sub-glandular or hard. Hardness was a risk factor for implant failure, but only among women who wore a bra during exercise. The risk of failure for a "typical" patient with a 10-year old implant was estimated to be 17%. The results of this study offer guidance to women with breast implants and their health care providers who are questioning their risk of silicone breast implant failure
