Book excerpt: This edited volume is a definitive text on adaptive clinical trial designs from creation and customization to utilization. As this book covers the full spectrum of topics involved in the adaptive designs arena, it will serve as a valuable reference for researchers working in industry, government and academia. The target audience is anyone involved in the planning and execution of clinical trials, in particular, statisticians, clinicians, pharmacometricians, clinical operation specialists, drug supply managers, and infrastructure providers. In spite of the increased efficiency of adaptive trials in saving costs and time, ultimately getting drugs to patients sooner, their adoption in clinical development is still relatively low. One of the chief reasons is the higher complexity of adaptive design trials as compared to traditional trials. Barriers to the use of clinical trials with adaptive features include the concerns about the integrity of study design and conduct, the risk of regulatory non-acceptance, the need for an advanced infrastructure for complex randomization and clinical supply scenarios, change management for process and behavior modifications, extensive resource requirements for the planning and design of adaptive trials and the potential to relegate key decision makings to outside entities. There have been limited publications that address these practical considerations and recommend best practices and solutions. This book fills this publication gap, providing guidance on practical considerations for adaptive trial design and implementation. The book comprises three parts: Part I focuses on practical considerations from a design perspective, whereas Part II delineates practical considerations related to the implementation of adaptive trials. Putting it all together, Part III presents four illustrative case studies ranging from description and discussion of specific adaptive trial design considerations to the logistic and regulatory issues faced in trial implementation. Bringing together the expertise of leading key opinion leaders from pharmaceutical industry, academia, and regulatory agencies, this book provides a balanced and comprehensive coverage of practical considerations for adaptive trial design and implementation.

Available in PDF, EPUB and Kindle. Book excerpt: Due to population aging, calcific aortic valve disease (CAVD) has become the most common heart valve disease in Western countries. No therapies exist to slow this disease progression, and surgical valve replacement is the only effective treatment. Calcific Aortic Valve Disease covers the contemporary understanding of basic valve biology and the mechanisms of CAVD, provides novel insights into the genetics, proteomics, and metabolomics of CAVD, depicts new strategies in heart valve tissue engineering and regenerative medicine, and explores current treatment approaches. As we are on the verge of understanding the mechanisms of CAVD, we hope that this book will enable readers to comprehend our current knowledge and focus on the possibility of preventing disease progression in the future.

Available in PDF, EPUB and Kindle. Book excerpt: The Department of Cardiology at The University of Texas MD Anderson Cancer Center was established on September, 1, 2000. In the past 15 years, we have evaluated and treated more than 10,000 cancer patients with cancer ther¬apy-related cardiovascular complications. Three years ago, we initiated the MD Anderson Practice (MAP) project to distillate our practice patterns into al¬gorithms to be shared with the onco-cardiology community. Because cancer is often an exclusion criterion for cardiology studies, purely evidence-based man¬agement of cancer therapy-related cardiovascular complications is not possible. With this vacuum of knowledge, various "guidelines" have proliferated that are either misleading or difficult to practice. In this manual, we present 16 MAPs that have been extensively reviewed by the cardiologists at MD Anderson. These MAPs should be considered our best practices rather than "guidelines." These MAPs will be updated frequently to reflect advances in the field. This manual con¬sists of MAPs, figures, and tables. We hope you will find these materials useful to your practice and provide us with feedback to improve these MAPs.

Available in PDF, EPUB and Kindle. Book excerpt: In recent years there has been considerable interest in the diagnosis and understanding of ventricular repolarisation, particularly the QT interval prolongation and abnormal T and T/U wave morphology associated with torsades de pointes. Advances in ion channel cloning have greatly improved our understanding of the role of ionic channels in mediating cardiac repolarisation. Unfortunately, it is increasingly recognised that a number of drugs, both those associated with altering repolarisation, and others for non-cardiac conditions can increase the propensity for polymorphic ventricular tachycardia, syncope and even ventricular fibrillation and sudden death. In this volume, arrhythmia specialists from St. George’s Hospital Medical School, London discuss the mechanisms behind QT prolongation and torsades de pointes. They focus particularly on the risk of individual cardiac and non-cardiac drugs in provoking long QT syndrome, providing a comprehensive review which will be useful for all electrophysiologists treating polymorphic ventricular tachycardias, and will expose important regulatory issues for pharmaceutical authorities and for the wider medical community.

Book excerpt: This is a detailed clinical compilation on sudden cardiac death causes. From the various geographical regions of the world comes different clinical perspectives toward the SCD problem that shed light on the predictors and estimated prevalence in genetic inheritance worldwide. For many centuries, the mysterious deaths of prominent professional sportsmen have preoccupied the attention of the media and medical professionals. In some cases, it was possible to obtain the medical histories of the victims, data on the presence of some specific diseases, and conditions or potential symptoms preceding the fatal episode. Various rare inherited syndromes that are associated with congenital heart disease are focused on in this study, and the interrelationship between ventricular premature beats, sustained/non-sustained ventricular tachycardias and ventricular fibrillations are discussed. Cardio-oncology is an emerging field where more and more patients face consequences from radiotherapy and chemotherapy because these individuals become more prone to SCD. Among the most notable developments, three-dimensional echocardiography and myocardial strain imaging are worth noting and might inexpensively improve the current risk stratification process. Recently, a strain that reflects the change of length and thickness of myocardial fibres has been incorporated into the routine clinical practice. The risk of sudden cardiac death is not the same in patients with Chagas disease, and newly predicted factors are to blame. The prevalence of SCD, hypertrophic cardiomyopathy and the accurate risk are extremely challenging, so much so that they recently became a source of argument between European and American experts. This is a must-have book for those interested in a clinical approach to the SCD phenomenon.
