Book excerpt: Candida auris is a recently emerged fungal pathogen that is a growing source of nosocomial infections globally. C. auris colonizes and persists on human skin, facilitating rapid spread of this fungus that causes invasive disease with high mortality rates. As the threat of C. auris grows, it is increasingly important to understand how it proliferates in the skin microenvironment, with a goal of identifying ways to reduce skin colonization and spread. Here, we use in vitro and ex vivo skin models to study C. auris colonization. Our first study focuses on the efficacy of the antiseptic chlorhexidine against C. auris on skin. We show that, despite strong activity against C. auris in vitro, chlorhexidine is minimally effective in decolonizing C. auris from porcine skin. We find that the activity of chlorhexidine can be augmented when combined with some essential oils and their active components, suggesting ways to improve current treatment options. In our next study, we demonstrate that C. auris grows to high burdens on human skin ex vivo, similar to our porcine skin studies. We show that other closely related Candida spp. do not grow as well as C. auris in the skin microenvironment. The use of human skin provides another option for skin colonization studies and confirms the relevance of our ex vivo skin models to clinical observations. In our final study, we investigate the role of potassium transport in skin colonization. By systematically removing components from our synthetic sweat medium, we found that K+ is critical for growth in the skin microenvironment. We disrupted genes involved in K+ uptake, including TRK1, a predicted potassium uniporter. We found trk1[delta] to exhibit a growth defect on skin, which was reversed by supplementing with a high concentration of extracellular potassium, and had defects in potassium uptake compared to WT. We further show that trk1[delta] has physiological disruptions, such as decreased intracellular pH and increased sensitivity to high concentrations of Na+ compared to WT. Overall, we demonstrate the utility of skin models for studying C. auris and present new information about potassium transport and skin colonization. Future studies focused on further uncovering the mechanisms of potassium transport as they relate to skin colonization will be critical in the search for new treatments and methods to prevent C. auris skin colonization.

Book excerpt: Fungal pathogens are on the rise. Annually more than one billion people are affected by superficial, allergic and mucosal fungal infections. However, of highest concern are the life-threatening invasive fungal infections. An increasing at-risk population together with inaccurate diagnostics and limited treatment options led to a dramatic increase in these life-threatening fungal infections and exceptionally high mortality rates, resulting in over 1.6 million deaths caused by fungi each year. Candida species are particularly prevalent in hospital settings and account for more than 750.000 invasive infections each year. This makes Candida one of the most critical groups of fungal pathogens. The last decades an epidemiological shift has been taking place from Candida albicans as the primary cause of invasive Candida infections towards other, often drug resistant, species. Especially Candida auris showed the world the ability of fungi to rapidly emerge as a significant threat to human health. More insights into the ecological mechanisms, phenotypic traits and genomic elements that contribute to the emergence of Candida pathogens, such as C. auris, are essential to both expose current and future pathogens and develop strategies to combat them. In this thesis C. auris and emerging relatives within the Candida haemulonii species complex are characterized using genomic and phenotypic approaches. Pathogenic species are compared to their closest non-pathogenic siblings, exposing specific traits that could contribute to their virulence and emergence in the nosocomial environment. In addition, ways to better understand the epidemiology and improve diagnostic tools for C. auris and the C. haemulonii complex were investigated."--
