Available in PDF, EPUB and Kindle. Book excerpt: Most women who die from cervical cancer, particularly in developing countries, are in the prime of their life. They may be raising children, caring for their family, and contributing to the social and economic life of their town or village. Their death is both a personal tragedy, and a sad and unnecessary loss to their family and their community. Unnecessary, because there is compelling evidence, as this Guide makes clear, that cervical cancer is one of the most preventable and treatable forms of cancer, as long as it is detected early and managed effectively. Unfortunately, the majority of women in developing countries still do not have access to cervical cancer prevention programmes. The consequence is that, often, cervical cancer is not detected until it is too late to be cured. An urgent effort is required if this situation is to be corrected. This Guide is intended to help those responsible for providing services aimed at reducing the burden posed by cervical cancer for women, communities and health systems. It focuses on the knowledge and skills needed by health care providers, at different levels of care.

Book excerpt: This volume reviews the evidence for a causal link between sexually transmitted infection with human papillomavirus (HPV) and the occurrence of cervical cancer, from a variety of different angles. Epidemiological studies and clinical, pathological, and cytological aspects of HPV infection are reviewed. Modern methods for analyzing HPV-DNA types by molecular biological techniques are described, and the statistical problems to be overcome in epidemiological work are explained. The volume was prepared by a broad team of experts from around the world, who met in Copenhagen in March 1988 to reach a consensus on the present state of understanding and to establish directions for future work.

It may be present with vaginal bleeding but symptoms may be absent until the cancer is in advanced stages, which has made cervical cancer the focus of intense screening efforts. Most scientific studies point to human papillomavirus (HPV) infection responsible for 90% of the cases of cervical cancer. There are 7 most common types of HPV - 16, 18, 31, 33, 42, 52 and 58. Types 16 and 18 being the most common cause of the cancer. Treatment is with surgery (including local exicision) in early stages and chemotherapy and radiotherapy in advanced stages of the disease. This book presents cutting edge research in this field. This involves, programs for cancer screening, alternative approaches to cervical cancer prevention, HPV/Co-Infections and Host Genetic Profiles, Small Cell Carcinoma of the Uterine Cervix, Indicators of HPV-induced carcinogenesis, functional genomics as a tool for understanding cervical cancer, histone deacetylase inhibitors, Chinese women and cervical cancer and cervical cancer in Northeastern Thailand.

Book excerpt: Globally, cervical cancer is the second leading cause of death for women with cancer. This is a tragedy in part because cervical cancer affects women usually a decade sooner than other cancers when those women are in their reproductive years and in the prime years in the labour force. While screening with the Pap test or other more recently evaluated tests have resulted in low cervical cancer rates in high resource countries, the lack of screening options in low resource countries means that many women present with symptoms reflective of advanced disease and so the chance of survival is low even with aggressive treatment. In this book, we have an international spectrum of authors from both high and low resource settings. We review the risk factors that lead to cervical cancer. We discuss novel screening techniques including HPV testing. We present the capacity for HPV vaccines to markedly reduce cervical cancer rates. Planning and implementing an organised cervical screening program and the assessment of quality indicators as a means of evaluation is outlined. In this book, we discuss novel operative options in early stage invasive cervical cancer as a means of preserving fertility. Also, we discuss the role of neoadjuvant chemotherapy as a means of down staging the disease. The role of surgically staging women to define more advanced disease is outlined including options for management of metastatic and/or recurrent disease with chemotherapy and targeted therapies. The special situation of women found to be pregnant during the diagnosis of pre-invasive or invasive cervical cancer; and the diagnosis and management principles are reviewed. Just as poliomyelitis has almost been eliminated in the lifetime of one generation, we look forward to the elimination of oncogenic HPV and its deadly ramifications particularly for women exposed to this virus.

Available in PDF, EPUB and Kindle. Book excerpt: Catching Cancer introduces readers to the investigators who created a medical revolution—a new way of looking at cancer and its causes. Featuring interviews with notable scientists such as Harald zur Hausen, Barry Marshall, Robin Warren, and others, the book tells the story of their struggles, their frustrations, and finally the breakthroughs that helped form some of the most profound changes in the way we view cancer. Claudia Cornwall takes readers inside the lab to reveal the long and winding path to discoveries that have changed and continue to alter the course of medical approaches to one of the most confounding diseases mankind has known. She tells the stories of families who have benefited from this new knowledge, of the researchers who made the revolution happen, and the breakthroughs that continue to change our lives. For years, we’ve thought cancer was the result of lifestyle choices, environmental factors, or genetic mutations. But pioneering scientists have begun to change that picture. We now know that infections cause 20 percent of cancers, including liver, stomach, and cervical cancer, which together kill almost 1.8 million people every year. While the idea that you can catch cancer may sound unsettling, it is actually good news. It means antibiotics and vaccines can be used to combat this most dreaded disease. With this understanding, we have new methods of preventing cancer, and perhaps we may be able to look forward to a day when we will no more fear cancer than we do polio or rubella.

Book excerpt: This book provides primary care clinicians, researchers, and educators with a guide that helps facilitate comprehensive, evidenced-based healthcare of women and gender diverse populations. Many primary care training programs in the United States lack formalized training in women’s health, or if they do, the allotted time for teaching is sparse. This book addresses this learning gap with a solid framework for any program or individual interested in learning about or teaching women’s health. It can serve as a quick in-the-clinic reference between patients, or be used to steer curricular efforts in medical training programs, particularly tailored to internal medicine, family medicine, gynecology, nursing, and advanced practice provider programs. Organized to cover essential topics in women’s health and gender based care, this text is divided into eight sections: Foundations of Women's Health and Gender Based Medicine, Gynecologic Health and Disease, Breast Health and Disease, Common Medical Conditions, Chronic Pain Disorders, Mental Health and Trauma, Care of Selected Populations (care of female veterans and gender diverse patients), and Obstetric Medicine. Using the Maintenance of Certification (MOC) and American Board of Internal Medicine blueprints for examination development, authors provide evidence-based reviews with several challenge questions and annotated answers at the end of each chapter. The epidemiology, pathophysiology, evaluation, diagnosis, treatment, and prognosis of all disease processes are detailed in each chapter. Learning objectives, summary points, certain exam techniques, clinical pearls, diagrams, and images are added to enhance reader’s engagement and understanding of the material. Written by experts in the field, Sex and Gender-Based Women's Health is designed to guide all providers, regardless of training discipline or seniority, through comprehensive outpatient women’s health and gender diverse care.

She was a poor Southern tobacco farmer who worked the same land as her slave ancestors, yet her cells—taken without her knowledge—became one of the most important tools in medicine: The first “immortal” human cells grown in culture, which are still alive today, though she has been dead for more than sixty years. HeLa cells were vital for developing the polio vaccine; uncovered secrets of cancer, viruses, and the atom bomb’s effects; helped lead to important advances like in vitro fertilization, cloning, and gene mapping; and have been bought and sold by the billions. Yet Henrietta Lacks remains virtually unknown, buried in an unmarked grave. Henrietta’s family did not learn of her “immortality” until more than twenty years after her death, when scientists investigating HeLa began using her husband and children in research without informed consent. And though the cells had launched a multimillion-dollar industry that sells human biological materials, her family never saw any of the profits. As Rebecca Skloot so brilliantly shows, the story of the Lacks family—past and present—is inextricably connected to the dark history of experimentation on African Americans, the birth of bioethics, and the legal battles over whether we control the stuff we are made of. Over the decade it took to uncover this story, Rebecca became enmeshed in the lives of the Lacks family—especially Henrietta’s daughter Deborah. Deborah was consumed with questions: Had scientists cloned her mother? Had they killed her to harvest her cells? And if her mother was so important to medicine, why couldn’t her children afford health insurance? Intimate in feeling, astonishing in scope, and impossible to put down, The Immortal Life of Henrietta Lacks captures the beauty and drama of scientific discovery, as well as its human consequences.
