Book excerpt: The purpose of the book is to explain the current state of the art use of calcium antagonists in hypertension and, specifically, hypertensive and diabetic kidney disease. The authors offer an up to date insight in the changing world of calcium channel blockers. Furthermore, scientific research has improved drug designs with better pharmacodynamic profile that allows once daily administration and twenty four hours antihypertensive effects. These achievements on effectiveness have been accompanied by reduced effects when compared with classical calcium antagonists. Most interestingly, this book examines new calcium channel blockers, which may effect kidney microcirculation. A comprehensive review of experimental and clinical facts directed to both basic research and clinical practice is also given in this book.

Book excerpt: The effect of calcium antagonists on heart muscle and blood circulation is the reason that they have found widespread clinical application for a number of years. Less well known, in contrast, is the effect this group of substances has on the kidneys, both on kidney cells and the blood flow through the kidneys. This effect was the subject of a workshop we organized in Tiibingen in June 1986. Different groups studied the effects of these substances, especially in animals, on the processing of calcium by the kidney cells and on blood flow. A possible explanation is that the calcium antagonists block the influx of calcium through special cell canals, especially the cells of the distal tubule. It is necessary to test whether there is a blockade or only a reduction in the passage of calcium. Our understanding of the effect of calcium antagonists is in large part based on the results of morphologic, physiologic, and pharmacologic studies of calcium in the kidneys. The particular processes involved in nephrocalcinosis are special objects of study with regard to calcium antagonists. This book presents the results of experimental studies of the effect of calcium antagonists on nephrocalcinosis and acute renal failure after ischemia. In this context, the clinician is particularly interested in the use of calcium antagonists to protect against the kidney in urolithiasis, in acute renal failure and during kidney transplantation. The book is thus of interest to urologists and nephrologists as well as pharmacologists. biochemists, physiologists, and others in research.

Book excerpt: Chronic kidney disease is one of the world's major public health problems, and the prevalence of kidney failure is rising steadily. Among the risk factors for a faster progression of renal disease are hypertension and proteinuria, many studies clearly demonstrating that hypertension is both a cause and consequence of chronic kidney disease. Namely, renal blood pressure regulation seems to be involved in five major pathophysiological mechanisms (all closely related to the renin-angiotensin system): Pressure-natriuresis, renal sympathetic nervous system, renal blood flow, intraglomerular pressure and tubuloglomerular feedback. This book reviews experimental data which form the basis of our current understanding of the association between hypertension and kidney diseases: The pathogenesis of increased blood pressure, the mechanisms by which systemic hypertension promotes progressive kidney failure, and the impact of antihypertensive agents on experimental renal mechanisms involved in hypertension. Furthermore, the role of angiotensin II receptor blockers in both the control of systemic blood pressure and the reduction of proteinuria is examined in an attempt to define optimal therapeutic strategies to prevent the otherwise inexorable deterioration of renal function in patients with chronic kidney disease.

Book excerpt: Calcium antagonists are now regarded as the most important advance in cardiac drug therapy since the advent of beta-adrenergic blocking agents. Acting ba sically as vasodilators-though with many other com plex mechanisms especially in the case of the anti arrhythmic calcium antagonists, these agents have grown in importance to become among the therapeutic agents of first choice for angina pectoris and hyper tension. The major aim of the present book is to present the clinician with the information needed for the practical use of calcium antagonists. What do all the numerous and often conflicting trials say? Do these agents really work? If so, which agent and in what dose? How do the three front runners, verapamil, nifedipine and diltia zem compare in the efficacy and side-effects with each other? How do the new second generation agents, now entering the North American market, slot in and com pare with the three first-liners? When the gloss is taken away from the advertisements, what is really left? The strong clinical bias of the present book should be complimented by further reading of books slanted towards fundamentals. One of the most important and recent of these is that by Dr Winifred Nayler (Calcium Antagonists, Academic Press, 1988). That book should be basic for essential background knowledge in the area of calcium antagonists. The important basic contribu tions of Fleckenstein deserve emphasis.

Book excerpt: Advances in itch research have elucidated differences between itch and pain but have also blurred the distinction between them. There is a long debate about how somatic sensations including touch, pain, itch, and temperature sensitivity are encoded by the nervous system. Research suggests that each sensory modality is processed along a fixed, direct-line communication system from the skin to the brain. Itch: Mechanisms and Treatment presents a timely update on all aspects of itch research and the clinical treatment of itch that accompanies many dermatological conditions including psoriasis, neuropathic itch, cutaneous t-cells lymphomas, and systemic diseases such as kidney and liver disease and cancer. Composed of contributions from distinguished researchers around the world, the book explores topics such as: Neuropathic itch Peripheral neuronal mechanism of itch The role of PAR-2 in neuroimmune communication and itch Mrgprs as itch receptors The role of interleukin-31 and oncostatin M in itch and neuroimmune communication Spinal coding of itch and pain Spinal microcircuits and the regulation of itch Examining new findings on cellular and molecular mechanisms, the book is a compendium of the most current research on itch, its prevalence in society, and the problems associated with treatment.
