Furthermore, the incidence of this tumor is slowly but steadily increasing while our means of treating it remain ineffec tive in distressingly many patients. Breast cancer thus represents an increasing threat to public health. The clinician responsible for the management of patients with this distressing disease has found himself, in recent years more than ever before, in a very awkward state of uncertainty. Many of his tradi tional views or practices have been questioned. The new concepts challenging his position are based on evidence at times so esoteric that, being often unable to comprehend it, he may subconsciously tend to distrust it. To his further dismay, clinicians and investigators of equal caliber and repute in this field disagree so widely and on so many important issues that he can no longer refer to an authoritative source for guidance; his horizons are therefore blurred and the stage is set for confusion. Much of what underlies the current controversy stems from our ignorance concerning the etiology of breast cancer and our inability to comprehend the many eccentricities of its clinical behavior.

Rapid advances in the field of molecular biology have been bewildering for those involved in its study and management. “Molecular Pathology of Breast Cancer” aims to close this knowledge gap by discussing comprehensively the evolution, biological basis and clinical applications with a focus on the “what, when, and how” of the most significant molecular markers known to date. These markers are evaluated in the context of genomic, transcriptomic and proteomic profiles, which is integral to the practice of precision medicine. The application of next generation sequencing (NGS) has provided new insights in the regulation of genomic and transcriptomic structure and function. Alterations in DNA such as mutations and single nucleotide polymorphisms (SNPs) have been correlated with outcomes and provide for novel therapeutic approaches. These NGS analyses have also revealed the extensive contributions of epigenetic mechanisms such as histone modifications, non-coding RNA and alternative splicing. All of these changes together contribute to alterations in proteome. Newer assays that allow greater stability and analytical consistency are emerging. These alterations in tumor profiles can be also now detected by imaging techniques. The heterogeneity of both tumor and tumor microenvironment, an inevitable reality, is discussed in detail with particular focus on cancer stem cells and immune signaling. A chapter is dedicated to the emerging technology of “liquid biopsy”, which opens a novel approach for “continuous” monitoring of cancer that might be superior to conventional diagnostics, “Molecular Pathology of Breast Cancer” provides a quick and easy, not to mention essential, tour for clinicians, pathologists and scientists who are seeking to understand the integration of molecular biology into the diagnosis, prognosis and management of breast cancer.

In keeping with present concerns and in line with current trends, the theme was "Diet and Breast Cancer. " This proceedings volume is comprised of chapters from the platform presentations of the two day conference and abstracts from the poster session held at the end of the first day. Experimentally, there is sufficient evidence to support a relationship between dietary fat and the risk of breast cancer. A meta-analysis was provided by data from 114 experiments with over 10,000 animals, divided into groups fed ad libitum on diets with different levels or sources of fat, or different levels of energy restriction. This exercise suggested that linoleic acid was a major determinant of mammary tumor development but that other fatty acids also enhanced mammary tumor development in animals. However, as mentioned by several speakers, results from epidemiological studies often are conflicting, thus leading to confusion among both health professionals and the public. Surveys of specific populations which have migrated from countries with low breast cancer rates to those with higher rates are often some of the most compelling studies with respect to a high fat diet-breast cancer association. Nonetheless, various cohort and prospective studies, some quite large, did not appear to show a relationship between consumption of fat (any type) and breast cancer.
