Book excerpt: En France, plus de 17% des personnes de plus de 75 ans seraient atteintes par la maladie d'Alzheimer ou un syndrome apparenté. Il n'existe actuellement aucun traitement permettant de guérir cette maladie. Quelques médicaments peuvent apparemment retarder l'évolution de la maladie mais leur action est inconstamment efficace et transitoire et ils ont des effets secondaires, ce qui a été à l'origine de controverses portant notamment sur leur justification économique. Les psychotropes sont très largement employés dans la prise en charge des symptômes comportementaux et psychologiques des démences, alors que certaines de ces molécules sont à éviter en raison de leurs effets secondaires chez ces patients fragiles. Nous avons mis en place une étude prospective afin d'évaluer les pratiques de prescription des médicaments anti-Alzheimer et des psychotropes chez les sujets âgés déments. Parmi les 200 patients de l'étude, 54,5% étaient traités par un ou plusieurs médicaments de la démence. Deux tiers des patients étaient traités par un médicament de la démence depuis plus d'un an, alors que l'efficacité à long terme de ces médicaments est mal documentée. 8,7% des prescriptions d'anticholinestérasique et 8,0% des prescriptions de mémantine étaient inappropriées par insuffisance de preuve d'efficacité pour les indications où ils étaient utilisés. 13,8% de patients traités recevaient une association d'un anticholinestérasique avec la mémantine alors que ces associations sont actuellement hors AMM. 81,0% des patients prenaient au moins un médicament psychotrope, et certains patients avaient jusqu'à 5 psychotropes, avec un nombre moyen de 2 psychotropes par patient. 18,3% des antidépresseurs étaient prescrits à une posologie plus faible que celle recommandée par l'AMM de ces médicaments. 32,0% des patients étaient traités par au moins une benzodiazépine ou une molécule apparentée. La posologie maximale de ces molécules n'était pas toujours respectée. 30,0% des patients prenaient au moins un neuroleptique. Des dépassements de posologies ont été constatés pour certaines phénothiazines, molécules dont l'utilisation n'est pourtant pas recommandée dans cette population de patients. Il n'a pas été constaté de différence significative entre le nombre de neuroleptiques prescrits chez les patients traités par un médicament de la démence et ceux qui l'étaient pas. La prescription d'un médicament anti-Alzheimer n'a donc pas entraîné de diminution de la prescription de neuroleptiques alors que cet argument est parfois avancé pour justifier de prolonger tut médicament anti-Alzheimer.

However, it was rediscovered in the 1970s under the influential work of the Genevan School. In particular, Piaget’s work on conservation, including conservation of number, profoundly influenced developmental psychologists who, working first in the Piagetian theoretical framework, began to discover a broader set of topics in mathematical cognition. In developmental psychology, the field continued to expand and covered a wide range of topics. During the same period, however, no such evolution occurred in neuropsychology, and except for some studies around the time of publication, very little had been published on acalculia and number processing disorders. However, a more general theoretical evolution occurred in neuropsychology, mainly due to increasing collaboration between clinical and experimental neuropsychologists, on the one hand and cognitive psychologists on the other. The objective of this book was to promote an evolution in the neuropsychology of calculation and number processing deficits and thus to introduce clinical and experimental neuropsychologists, as well as developmental and cognitive psychologists, to recent research and theoretical approaches that are of particular interest for the neuropsychological approach to mathematical cognition.
