Book excerpt: Excerpt from Biographical Clinics Patients, Vol. 3: Essays Concerning the Influence of Visual Function Pathologic and Physiologic Upon the Health of Patients The examination of 7,166 school children in New York City in 1905 showed that 1,273, or more than 17 percent, were suffering from defective eyesight of the worst or most evident kinds. A much larger proportion have smaller defects, and these added to the more manifest ones make a total of over 33 percent which had defects interfering with the pursuit of their studies. But these examinations took note only of the manifest errors of refraction or disease, and the kinds of eyestrain which have the worst reflex effects upon the general health are those caused by the low errors not manifest and which may be temporarily overcome by morbid effort. It is therefore abundantly demonstrated that almost everything urged by refractionists as to the pernicious influence of eyestrain is more than true. A Pott's fracture is an eversion fracture. It is always an eversion fracture, never an inversion fracture. What do we mean by an eversion fracture? It is a fracture of the lower end of the fibula that is produced by the foot turning outward when the injury is sustained. What hap pens when the foot turns outward? The external surface of the astragalus presses against the tip of the external malleolus, and the fibula breaks primarily. If the interosseous ligament is strong, the fibula breaks within the first 2% inches above the tip of the external malleolus, otherwise only the tip of the fibula is broken off on a line with the tibio - astragaloid articulation, as in the case before us today.
