Book excerpt: One of the fundamental elements of nursing practice is the transfer of patient information from one provider to another known as a bedside report. However, an unstructured bedside report that occurs away from the bedside, places patient safety, quality of patient care, and patient satisfaction in jeopardy. The purpose of this quality improvement (QI) project is to determine if the implementation of bedside reporting in an acute care pediatric unit will: decrease preventable errors reported through an event reporting system known as Safety Tracker, increase patient satisfaction in provider communication, and improve nurse accountability. The study included a convenience sample of 65 inpatient bedside nurses on a 30-bed, acute care pediatric unit in a large urban hospital. The participants were recruited on a voluntarily basis and informed consent was obtained prior to the study. Data were collected through pre-and post-questionnaires. Each questionnaire included, demographic data and two open-ended questions relating to the new Situation, Background, Assessment, Recommendation (SBAR) bedside handoff reporting tool. Education regarding the pre-and post-questionnaires and the new SBAR handoff reporting tool was provided by the primary investigator. Fifty-five participants, or (85%) completed both questionnaires for comparison. Results of this QI project indicated participants liked the consistency and structure of the new SBAR reporting tool; however, barriers identified were arousing and discussing sensitive information at the patient’s bedside. Patient satisfaction scores increased from 68.4% to 75%.

It is a time to review the plan of care, reconcile medications and infusions and answer questions. Traditionally, this has occurred in a central area such as a conference room or the nurses' station, away from the patient and family. Moving shift report to the bedside has been proven to improve communication between caregivers and the patient. Critical information can be exchanged and the bedside location allows for direct visualization of the patient along with verification of infusions, equipment settings and patient acuity. When this communication occurs at the bedside, patients and families are brought into the conversation and become active participants in the plan of care. Research has shown that conducting shift report at the bedside can significantly improve both patient and nurse satisfaction, leading to a more patient centered care approach, which in turn, may translate into better patient outcomes (Anderson and Mangiano, 2006). Bedside shift report (BSR), is a nursing intervention that may also assist with the Joint Commission directive to' involve both the patient and his or her family to develop a treatment plan that is tailored to the patient's unique needs. Staff should encourage the patient and family to ask questions throughout the course of care and provide opportunities for them to participate in care discussions' (The Joint Commission, 2010 pg. 18). To examine the effect of implementing BSR, a pilot study will be conducted in a 25-bed unit within an acute care hospital in the northeast United States. Presently, handoff report is conducted at the nurses' station; it is not uncommon for there to be questions later when the nurse actually visualizes the patient and the previous caregiver is gone. The process can also leave the patient unattended for up to an hour while report is taking place. This may result in delay of care or potentially, missed opportunities to avert crises. Based on the results of this pilot study, a decision will be made regarding hospital wide implementation.

Though the bedside nurses handover during acute care hospitalization lead to increase patient and family gratification, it also reduced the length of hospital stay and healthcare costs and nursing satisfaction without nurses' burnout/turnover.

With the use of Lewin09́s Change Theory an implementation, evaluation and dissemination plan has been established to support an Adult Medical Unit09́s change to bedside shift report. Each phase of the plan utilized evidence based best practice to support the change to bedside report. In implementing a standardized report process, this unit09́s goal was to improve patient safety, patient satisfaction, and nurse satisfaction. Surveys were conducted before and after implementation of the change along with real time audits to provide feedback to staff. Additional tools were provided to staff to further support their transition to bedside report. Through the literature review conducted, other near miss situations were noted that further supported the implementation of bedside shift report. These included medication errors, infiltrated intravenous sites, and incorrect laterality of scheduled surgeries. It is also noted that bedside report is supported by the Institute of Medicine and The Joint Commission who aim to support health care to be provided in the safest manner.

The proposed eight-group (four intervention groups and four comparison groups) research study will be analyzed using the Analysis of Covariance (ANCOVA). This grant could be applied to the American Nurses Foundation Nursing Grant Program, the Agency for Healthcare Research and Quality, or the National Institute of Nursing Research (NINR). The grant that will be chosen is the Agency for Healthcare Research and Quality’s (AHRQ) research grant.

Through many research studies, bedside reporting exhibited evidence-based practices that cultivate patient involvement by fostering patient/nurse trust/bonds, integrates patient in the care team, and promotes accountability for all and prioritizing care. The purpose of this project is to guide the nurse leader/nurse manager and staff nurses an educational plan consisting of visuals on bedside reporting. The implementation plan includes scripts to aid nurses for the optimal report. By visualizing the patient during report, nurses will be able to address patient's needs, perform safety scans and involve patient/family in the plan of care. In a study, there were increased nurses' satisfactions, accountability, patient involvement as well as decrease in 35% in falls and 50% decrease in medication errors. Pre/post implementation surveys are given to nurses in order to pinpoint barriers and revise the process. Nurse managers, administration and staff nurses are the recipients of disseminating evidence to enhance bedside reporting and continue ongoing evaluations.

The aim statement for the project is improving patient safety by ensuring the implementation of bedside shift report by 80% of nurses by August 2016. Kurt Lewin's change theory is going to be used to effect the change. There were in-services given to nurses on the floor and the project was also discussed in staff meetings. Flyers were posted around the units to help build up and maintain enthusiasm for the implementation of BSR. Implementation was initiated behind schedule due to conflict in schedules and bureaucratic hoops. The implementation was started on April 28, 2016 and so there are no exact numbers at this time because there is a two week trial period. But I have had conversations with the some nurses that have started the project since initiation. So far they seem to be very receptive of the change. Most of the patients have been neutral at this time but we'll see if that's going to be the case in two weeks. There are numerous publications that support the efficacy of bedside shift report in relation to patient care and safety. Publications from the Agency for healthcare research & quality (AHRQ), MEDSURG Nursing, American nurse today, Journal of Clinical Nursing and the Nursing journal are some of the articles that were used to investigate and develop ideas and policies for the project. Patient experience with the bedside shift report (BSR) will be tracked and the data collected will be used to improve the process in order to enhance the patient experience. Patient experience would include if the patient felt more engaged in their care, if they felt safer and had more confidence in the personnel helping out with the care. Also nurse satisfaction is another benchmark that would be measured by sending out surveys periodically in an effort to gauge the opinions of the staff members. The data collected from the surveys will be used to develop improvements to the process and increase the comfort level of the nurses in giving BSR. We would also track if the nurses feel more accountable for the care they delivered during the shift and if the accountability translates to nurses being more responsible in the care that they deliver. Periodic collection of data will be used to monitor patient satisfaction and nursing compliance. The data would be monitored by the appointed BSR champions because they would be able to monitor and collect data on compliance of the floor nurses in real time and compare it with data collected from patient satisfaction surveys. There are going to be bi-weekly meetings to analyze and improve the process by applying PDSA cycle. Analysis of the data can also help to recognize fallouts earlier and provide opportunity to make corrections. In conclusion, sustainability is what is important. One of the ways I recommend to ensure continuity of the project by making sure that the new employees, regardless of experience, are trained during their eight to twelve week floor orientation. After conducting a failure mode effect analysis coupled with experience, I concluded that continuity is also dependent on proper training of new employees. I concluded this because nursing is a high turnover profession and if the current nurses don't teach the new nurses before they leave the facility then the whole process will have to be restarted.

Ineffective communication during shift-to-shift reports can contribute to gaps in patient care and breaches in patient safety, including medication errors, falls, and sentinel events. The greatest risk of communication breakdown is during transitions in care. CONTEXT: St. Louise Regional Hospital, a small community hospital, consisting of an eight-bed medical-surgical intensive care unit lacked structure in how handoff should occur and had variances in shift handoffs. Observation of the shift handoff at the nurses' station revealed many communication gaps that have shown negative impacts on patient safety and outcomes. The improvement project described in this paper focused on evidence-based practices that support the benefits of bedside shift and implementations of standardized handoff tools. INTERVENTION: Intervention included an education in-service for unit managers, educational coordinators, and staff nurses to emphasize the important benefits of bedside shift report. MEASURES: The three components for evaluating improvement include outcome, process, and balancing measures to determine whether the improvement project has had the desired impact. The outcome measure expected to yield the following: direct observation of nurses during bedside report to calculate how often is being done, monitor for decrease in overtime due to more efficient shift-to-shift bedside reports, and a review of risk management reports observing for effect on the number and severity of medication errors. RESULTS: The outcome measure is to increase the nurses' compliance with bedside shift report in the intensive care unit to at least 80% within six months of implementation. CONCLUSION: With the proposed change, bedside shift report will addresses all the safety hazards by reducing adverse events, such as medical errors, patient falls at shift change, and sentinel events. Bedside shift report improves patient safety, enhances the quality of care, improves patient and nurse satisfaction, decrease unnecessary healthcare expenditure, and saves time.

Nurse's report feeling more informed about their patients while patients achieve a sense of relief and understanding about their plan of care when using bedside reporting (Sherman and others, 2013). The implementation of bedside reporting on a 24 bed inpatient unit concluded of a well-established plan which incorporated stakeholder education and preparation, guidelines, and pre and post patient and nurse surveys. Approval for the change in process was received from appropriate parties and the proposed change was introduced to all stakeholders. Meetings, guidelines, and educational material were made available to all nurses and stakeholders in conjunction with a support team to help encourage a seamless transition. To evaluate progress, affects, and issues with the new implementation, a nurse questionnaire was developed to assess feelings about the current method of reporting and the method of bedside reporting. To monitor the progress of bedside reporting, a questionnaire was developed to assess the impact on patient satisfaction. In conclusion, nurse reporting at the bedside has a positive impact on nurse communication and accountability. It also encourages patient participation in care, while becoming more informed and involved with their care.

Patient satisfaction scores obtained from patients after implementing beside shift report have shown higher scores when compared to patient satisfaction scores obtained from patients prior to implementing bedside shift report. Patient satisfaction scores are important to health care entities because they can affect many things including staff attitude, funding, and patient outcomes. Implementing bedside shift report can improve patient satisfaction scores. This intervention can improve patient satisfaction and patient safety, and potentially increase the incidence of positive patient outcomes.

Having a daily huddle with the nurses about the use of the SBAR communication tool, with a laminated card that will be handed out to each nurse to keep during change of shift report. There is a survey sent to patients at the end of their hospital stay: At the change of shift how often did the off-going nurse and on-coming nurse discussed your plan of care at the bedside? The first floor HCAPHS scores for Q1-20 52.4%, Q2-20 52.3%, Q3-20 56.4%, and Q4-20 72.3%. Reestablishing the current standard will have a positive impact on patient satisfaction scores, nurse satisfaction and patient safety.

Book excerpt: Background: The Joint Commission (TJC) evaluated sentinel events in healthcare and reported communication failure of care providers during patient handoffs as the most frequently cited root-cause. To improve patient outcomes and to avoid costly mistakes, it is recommended that the change of shift report should take place at the patient's bedside. Foreground: Studies regarding nursing shift reports indicate that information given is inconsistent among nurses. Despite attempts at creating standardized reporting sheets and implementing reporting methods to guide nurses, a lack of consistency remains, which can be a detriment to patient safety. Bedside reporting is recommended by TJC, but is not currently part of the nurse to nurse shift handoff report at a local, rural hospital. Framework: The Johns Hopkins Nursing Evidence-Based Practice Model was utilized as a guide to find the best evidence to implement and in translating the studies into practice. Method: Pre and post surveys were used to collect data on nursing staff perceptions on bedside reporting. After completing a pre survey, an education module on bedside reporting was shared with participants. Survey results obtained from nursing staff after participating in education were compared to the survey results of nursing perceptions before completing the education module. Descriptive data was obtained by comparing perceptions before and after education. Results: The findings show a positive change in nursing stadd perceptions on bedside reporting as evidenced by an improvement in pretest to posttest survey scores. Post education, there was an increase from 45% to 68% of the participants perceiving bedside reporting as more comprehensive than current reports held in a break-room. One-hundred percent of the participants perceive that bedside reporting was a beneficial form of communication, compared to eighty percent pre education. Conclusion: Educating the nursing staff on bedside reporting has led to an increase in buy-in. This can result in the success and sustainability of bedside reporting on the nursing unit, after full implementation. Tools, such as the ready-to-use Studer Group Bedside Reporting Tool Kit, can provide nurse managers with a valuable resource that may foster the support and sustainability of bedside reporting in their departments.

In regards to the health care industry more explicitly, change is not always the most comfortable experience, and thorough communication is not always the easiest thing to accomplish when time is against you. However, both change and the enhancement of communication are concepts that drive people and facilities to achieve greater outcomes in existing situations. The World Health Organization's (WHO) focus on patient safety and satisfaction has been a major motivation for change in health communication, and more particularly, clinical handover worldwide (World Health Organization, 2010). Based upon number of studies, research data collection and findings, it has been revealed that when changeof- shift report is given between nurses at a patient's bedside, satisfaction, safety, and quality communication are enhanced. Patients are able to be involved in their own care, report maintains itself to be strictly objective by nature with subjective mannerisms, and errors related to miscommunication diminish. Basically, with the implementation of one alteration (bedsidechange-of-shift report versus at the nurse's station), all parties involved in the interaction will benefit in a great way. The following information gathered and compiled in this capstone project creates a detailed picture of a problem-area in the health care industry, a proposed solution to the problem, and evidence-based literature to back the solution. By implementing a small change, from shifting the location of change-of-shift report from the nurse's station to the patient's bedside, the domino effect can create a large impact.
