Available in PDF, EPUB and Kindle. Book excerpt: The practice of autologous blood collection and transfusion has been steadily increasing. The concept is not new; however, its application has been limited because of lack of interest and willingness to make the extra effort required. Also there has been concern about the safety of the procedure. Homologous blood transfusion is easy and has allo wed us to salvage lives and perform procedures which would not have been feasible without the availability of whole blood or its components. Transfusion with homologous blood products is safe; however, com plications can and do occur. The transmission of AIDS by transfusion of homologous blood products has significantly increased patient and physician awareness of the dangers one faces. This potential complica tion has promoted autotransfusion. It is now commonly used and reques ted by patients and physicians. Blood for autotransfusion is collected pre or intraoperatively by phle botomy and retrieved intra and post operatively from sites of hemor rhage. Once collected, the blood is either stored or returned to the patient as whole or processed blood. To have a successful autotransfusion program requires anticipation of its needs and coordination and training of involved personel in the blood bank, emergency room, operating room and post-surgical care units. Pre deposit and processing of collected blood requires remission of donor criteria and labeling and storage procedures.

Book excerpt: Autotransfusion or autologous blood transfusion is an alternative to homologous transfusion. This work describes the various techniques for economising in the use of blood in surgery, including intentional normovolemic hemodilution which alters the blood's rheological properties. Programmed deferred autotransfusion as well as peroperative saving and recirculation of blood may be self sufficient and also avoids the risk of transmission of viral diseases such hepatitis and HIV infection as well as satisfying the wishes of religious sects such as Jehovah's Witnesses.

Available in PDF, EPUB and Kindle. Book excerpt: A functional autotransfusion unit will permit the use of a person's own blood for transfusion. By this means the possible nonavilability of homologous blood and other complications of homologous blood transfusions can be avoided. Under combat conditions, when homologous blood is not always available, salvage of the patient's blood will be highly desirable. A disposable autotransfusion unit using readily available plastic components and simple construction has been designed at Wilford Hall USAF Hospital. This unit has been tested under uncontaminated conditions in 20 dogs with 100 per cent survival and reported elsewhere. No evident sequelae have been encountered in any dog, although up to 1600 cc of blood have been given by autologous blood transfusion. The apparatus deserves more extensive evaluation under controlled clinical conditions. (Author).

Available in PDF, EPUB and Kindle. Book excerpt: Historically, 20% of all injured combatants die on the battlefield before they can be evacuated to a field hospital. Blood lossâ€"hemorrhageâ€"is the single major cause of death among those killed in action whose lives might otherwise be saved. Fluid resuscitation and the treatment of hypovolemia (the abnormally decreased volume of circulating fluid in the body) offer the greatest opportunity for reducing mortality and morbidity associated with battlefield casualties. In Fluid Resuscitation, a committee of experts assess current resuscitation fluids and protocols for the treatment of combat casualties and make recommendations for future research. Chapters focus on the pathophysiology of acute hemorrhagic shock, experience with and complications of fluid resuscitation, novel approaches to the treatment of shock, protocols of care at the site of injury, and future directions for research. The committee explicitly describes the similarities and differences between acute medical care during combat and civilian emergency trauma care. Fluid Resuscitation should help energize and focus research in both civilian and military emergency care and help save the lives of citizens and soldiers alike.
