The Social Security Administration (SSA) operates programs that provide cash disability benefits to people with permanent impairments like hearing loss, if they can show that their impairments meet stringent SSA criteria and their earnings are below an SSA threshold. The National Research Council convened an expert committee at the request of the SSA to study the issues related to disability determination for people with hearing loss. This volume is the product of that study. Hearing Loss: Determining Eligibility for Social Security Benefits reviews current knowledge about hearing loss and its measurement and treatment, and provides an evaluation of the strengths and weaknesses of the current processes and criteria. It recommends changes to strengthen the disability determination process and ensure its reliability and fairness. The book addresses criteria for selection of pure tone and speech tests, guidelines for test administration, testing of hearing in noise, special issues related to testing children, and the difficulty of predicting work capacity from clinical hearing test results. Book excerpt: Short-term cumulative temporary threshold shift in noise-exposed Air Force personnel was investigated. Daily pure-tone audiometric tests were administered on five consecutive mornings to three groups of men. Twenty-seven flight-line men had noise exposure during the test week; 24 flight-line men had no noise exposure during test week; 52 men had non-flight-line jobs. Analysis of audiometric data showed that: (a) Most mean thresholds in the three groups on Tuesday through Friday were better than mean thresholds on Monday. Improvement in measured auditory sensitivity during the test week is interpreted as indicative of a learning effect. (b) Intra-subject variability in thresholds at 4000 cps and at 6000 cps was not related to threshold sensitivity. (c) No significant cumulative short-term temporary threshold shift occurred among the noise-exposed men during the five-day test period.

Book excerpt: Introduction: Previous research showing associations between normal hearing level, psychosocial variables, and growth rates in teenagers hypothesised that the associations observed may in part be driven by variations in the capacity to express IGF-1. The aim of this project will be to test this association directly. An observed correlation between IGF-1 levels and hearing, can potentially explain the background for poor Auditory Processing Disorder (APD) scores among children with normal hearing. IGF-1 plays an important role in maturation of the cochlea, which is the hearing organ in the inner ear. Aim: The aim of the research was to investigate the relationship between variations in hearing and APD with serum IGF-1 in teenagers between the age of around 14-16 years with normal hearing. Other variables, such as height and age were also correlated against hearing thresholds. Hypothesis: Variation in IGF-1 expression may have impact on variations in hearing and APD scores. Methods: Eight participants were recruited and underwent two separate sessions for the study, the first one for audiological assessment including pure tone audiometry, tympanometry, speech audiometry and four auditory processing assessments (dichotic digits test, frequency pattern test, random gap detection test and forward digit span test). The second part of the study was IGF-1 assessment, where the participants visited Labtest Clinics for the blood collection and the analysis of IGF-1. Results: All participants had normal hearing. There was a correlation seen between IGF-1 and the average hearing threshold in that participants with better normal hearing tended to have higher concentrations of serum IGF-1. There were no correlations between IGF-1 and hearing thresholds with APD scores or other variables such as height and age. Conclusion: Children with higher IGF-1 have better hearing thresholds, and neither hearing sensitivity nor IGF-1 concentration had an association with processing ability of sound.

Book excerpt: Vigilance performances consisting of auditory threshold, latency and false-positive responses measures were obtained from 24 Navy and civilian subjects during the course of six daily 48-minute monitoring sessions in which Ss pressed a microswitch to report single tones in signal trains of increasing intensity. Six signal densities from 2.5 to 120 signals per hour, and six intersignal intervals ranging from 0 to 108 seconds around a signal rate of one per minute were found to have no differential effect on auditory threshold. An improvement of 3.25 dB in signal/noise detection occurred when signal density was increased from 2.5 to 15 per hour. Higher rates were not additionally effective. Below the rate of 15/hour, response latency increased regularly with the slower rates, though there was no further improvement with higher signal densities. Thus a rate of about one signal every four minutes was the most efficient density. Time-on-watch analysis revealed large individual differences. An analysis of false-positive responding indicated that false alarms were unrelated to signal rate, intersignal variability, or listening session. (Author).
