Book excerpt: A concise and current treatment of the subject of nuclear power safety, this work addresses itself to such issues of public concern as: radioactivity in routine effluents and its effect on human health and the environment, serious reactor accidents and their consequences, transportation accidents involving radioactive waste, the disposal of radioactive waste, particularly high-level wastes, and the possible theft of special nuclear materials and their fabrication into a weapon by terrorists. The implementation of the defense-in-depth concept of nuclear power safety is also discussed. Of interest to all undergraduate and graduate students of nuclear engineering, this work assumes a basic understanding of scientific and engineering principles and some familiarity with nuclear power reactors

Book excerpt: This Safety Guide provides recommendations on the structure and content of the safety analysis report to be submitted by the operating organization to the regulatory body for authorization of the siting, construction, commissioning, operation and decommissioning of a nuclear power plant. It is intended to facilitate both the development of the safety analysis report by the operating organization and the checking of its completeness and adequacy by the regulatory body. The publication is a revision of IAEA Safety Standards Series No. GS-G-4.1, Format and Content of the Safety Analysis Report for Nuclear Power Plants, which it supersedes. The revision reflects feedback experience from the Fukushima Daiichi accident and the subsequent stress tests performed. It also describes good practices and experience from the use of safety analysis reports for newly built nuclear power plants in different States and informs on recent progress made in approaches to safety assessment.

Book excerpt: This report was prepared in response to the recommendation of the IAEA General Conference in 1992 that the Director General develop a process to provide a common basis on which the acceptable level of safety of all operating nuclear power plants built to earlier standards can be judged. This followed a recommendation made at the IAEA Conference on 'The Safety of Nuclear Power: Strategy for the Future' held in September 1991. The report also provides a basis for allocating responsibilities for, and deciding on the best approach to, making a safety assessment, and for selecting criteria to adopt in deciding on what corrective action to take.

Book excerpt: The March 11, 2011, Great East Japan Earthquake and tsunami sparked a humanitarian disaster in northeastern Japan. They were responsible for more than 15,900 deaths and 2,600 missing persons as well as physical infrastructure damages exceeding $200 billion. The earthquake and tsunami also initiated a severe nuclear accident at the Fukushima Daiichi Nuclear Power Station. Three of the six reactors at the plant sustained severe core damage and released hydrogen and radioactive materials. Explosion of the released hydrogen damaged three reactor buildings and impeded onsite emergency response efforts. The accident prompted widespread evacuations of local populations, large economic losses, and the eventual shutdown of all nuclear power plants in Japan. "Lessons Learned from the Fukushima Nuclear Accident for Improving Safety and Security of U.S. Nuclear Plants" is a study of the Fukushima Daiichi accident. This report examines the causes of the crisis, the performance of safety systems at the plant, and the responses of its operators following the earthquake and tsunami. The report then considers the lessons that can be learned and their implications for U.S. safety and storage of spent nuclear fuel and high-level waste, commercial nuclear reactor safety and security regulations, and design improvements. "Lessons Learned" makes recommendations to improve plant systems, resources, and operator training to enable effective ad hoc responses to severe accidents. This report's recommendations to incorporate modern risk concepts into safety regulations and improve the nuclear safety culture will help the industry prepare for events that could challenge the design of plant structures and lead to a loss of critical safety functions. In providing a broad-scope, high-level examination of the accident, "Lessons Learned" is meant to complement earlier evaluations by industry and regulators. This in-depth review will be an essential resource for the nuclear power industry, policy makers, and anyone interested in the state of U.S. preparedness and response in the face of crisis situations.

Book excerpt: The nuclear industry and the U.S. Nuclear Regulatory Commission (USNRC) have been working for several years on the development of an adequate process to guide the replacement of aging analog monitoring and control instrumentation in nuclear power plants with modern digital instrumentation without introducing off-setting safety problems. This book identifies criteria for the USNRC's review and acceptance of digital applications in nuclear power plants. It focuses on eight areas: software quality assurance, common-mode software failure potential, systems aspects of digital instrumentation and control technology, human factors and human-machine interfaces, safety and reliability assessment methods, dedication of commercial off-the-shelf hardware and software, the case-by-case licensing process, and the adequacy of technical infrastructure.

Book excerpt: Unforgettable congressional hearings in 1978 revealed that fallout from American nuclear weapons testing in the 1950s had overexposed hundreds of soldiers and other citizens to radiation. Faith in governmental integrity was shaken, and many people have assumed that such overexposure caused great damage. Yet important questions remain--the most controversial being: did the radiation overexposure in fact cause the cancers and birth defects for which it has been blamed? Elements of Controversy is the result of a decade of exhaustive research in AEC documentary records and the full clinical and epidemiological literature on radiation effects. More concerned with uncovering the historical story than with assigning blame, Barton Hacker concludes that every precaution was taken by the AEC to avoid harming test participants or bystanders. And, he points out, the biomedical literature suggests that these precautions worked. Yet top officials in Washington--for whom the success of nuclear weapons was of overriding importance--had asserted that testing involved no risks at all. Discrepancies between unverifiable government claims and the revelations that some actual risk was present explain the origins and angry persistence of the controversies, Hacker argues. The Department of Energy delayed publication of Hacker's study for five years, and while his controversial book is sure to draw objections from both sides of the radiation-hazard debates, it will provide a much-needed guide to understanding their polemics. Book excerpt: This Safety Guide is intended primarily for use with land based stationary thermal nuclear power plants but it may, in parts, have a wider applicability to other nuclear facilities. It provides recommendations and guidance on the possible format and content of a SAR in support of a request to the State regulatory body for authorization to construct and or operate a nuclear power plant. As such, it contains recommendations on meeting the requirements of Safety guide GS-R-1 "Legal and governmental infrastructure for nuclear, radioactive waste and transport safety" (2000, ISBN 9201008007)
