Book excerpt: Introduction : Avec l'évolution de la médecine et des lois de bioéthiques il est indispensable d'évaluer la capacité de décision des patients de réanimation. L'analgo-sédation et la préservation des capacités cognitives (sédation coopérative) sont à adapter à chaque patient. Nous avons cherché à identifier les facteurs associés aux altérations de la capacité cognitive des patients (évaluée par le score ACE), et notamment l'impact des molécules de sédations, ainsi qu'à l'évaluation subjective de la capacité de décision jugée par les cliniciens en charge du patient. Matériel et méthodes : L'étude DexCap est une étude observationnelle, prospective et multicentrique. Les patients inclus étaient calmes et coopérants (RASS [-1;0;+1]) et non-confus (CAM-ICU négatif). Le score ACE était réalisé par un interne ou un médecin indépendant puis le personnel en charge du patient était interrogé en aveugle grâce à une échelle de Lickert à 4 variables (capable, probablement capable, probablement incapable, incapable). Le critère de jugement principal était l'impact des différentes molécules sédatives reçues au cours des 72 heures précédentes sur la capacité de décision des patients évaluée par le score ACE. Les critères de jugement secondaires incluaient notamment la concordance entre le score et l'évaluation subjective du personnel en charge du patient, et l'identification des facteurs pouvant altérer la capacité de décision des patients. Résultats : 304 patients ont été inclus entre octobre 2017 et juin 2021. 87% ont obtenu un score ≥ à 56, notifiant une capacité de décision préservée ou faiblement altérée. En analyse univariée, l'administration isolée de Rémifentanil était associée à de meilleurs scores ACE (56,4% vs 75,%, p= 0,01).
