Diabetes is more prevalent in blacks compared to whites of all ages. The purpose of this project was to develop, implement, and evaluate a culturally competent, church-based diabetic self-management intervention program for African American women with type 2 diabetes. Twelve African American women with type 2 diabetes participated in a church-based education program. Pre- and post-tests were used to assess knowledge of diabetes. A three-month behavior diabetessurveyy was used to assess behavioral change after completing the education program. Post-test scores on the diabetes knowledge test changed from a baseline mean of 83.67 (SD= 10.48) to 97.08 (SD= 3.60). Theresultss indicate that with additional instruction and education, the participant's diabetic knowledge increased. Fifty percent (50%) of the participants made at least two behavioral changes on the 3-month behavioral survey. The project found that church-based interventions can be an important component of diabetes health education programs. Culturally competent approaches that integrate ethnic beliefs, values, and customs are beneficial when teaching diabetes education to African American women.

The purpose of this study was to assess diabetes knowledge and self-care activities and explore experiences of African-American women with type 2 diabetes mellitus (T2DM). This mixed method study used a quantitative correlational design and a qualitative descriptive design. The data was collected using surveys and semi-structured individual interviews and analyzed using SPSS 17.0 and Atlas ti 5.2. Data were transcribed verbatim and examined for emergent themes. The sample included fifty-two African-American women between the ages of 40-64 from the North Central Florida region. The mean age for the sample was 55 years. Approximately 55.8% of the participants had higher than the average knowledge for diabetes. Results showed no associations between the respondent's level of knowledge about diabetes and certain self-care activities. However there were positive significant correlations between knowledge about diabetes and the years of education (rho=.24) and some self-care activities namely (a) specific diet (r=.28) and (b) foot care (rho=.40).

My study was to explore why Blacks/African American women are not able to manage the disease compared to other demography. Prior research had indicated that Blacks/African American women are disproportionately affected by diabetes and have a higher incident of diabetes related complications. In the course of my investigation, I discovered that this population are denied access to affordable quality healthcare services and Medicaid insurance. During the studies, data were collected from among the demographic groups from churches, clinics, public libraries, city halls and other public places. Over one hundred and twenty participants responded to the survey questionnaire on Survey Monkey platform. The relationship between self-care efficacy and self-care knowledge, self-care efficacy and self-care management, self-care knowledge and self-care management were tested. The result indicated a moderate correlation, but statistically significantly relationship among the variable.

However, increasing rates of disease suggest that large segments of the population remain unaware of the impact of lifestyle practices on overall health and quality of life. This pilot feasibility study was designed to implement and evaluate a peer-educator delivered 8-week nutrition and lifestyle education intervention for African American women ages 45 years and over and at risk for Type 2 diabetes their church setting. This 2 x 3 repeated measures study (usual care (UC), intervention (INT); baseline, endpoint, follow-up) used a convenience sample of 41 women (average age = 61 ± 8.92 years) at risk for diabetes from two predominantly African American churches in Greensboro, NC (N = 21, UC; N = 20, INT). Significant changes in fiber intake occurred over time, with no between group differences (INT = 10.9 to 12.5 g; UC = 12.7 to 9.6 g, p = 0.031). Group by time differences occurred for total fat (p = 0.013), monounsaturated fat (p = 0.045), and percent of energy from fat (p = 0.022), with the intervention group significantly decreasing fat intake. When age and total number of chronic conditions were added as covariates, total carbohydrate (grams) differed over time (p = 0.025, age and 0.007, chronic conditions), with INT reporting an increase and UC reporting a decrease. Limiting or restricting high sugar foods and limiting portion sizes significantly differed between groups at endpoint and follow up (p = 0.04). UC received the same education handouts, but not the classes and also reported some significant dietary changes over time, suggesting the benefits of targeted messages for this audience. The peer educator consistently and accurately delivered the curriculum as designed. Attendance rates indicated high exposure to the intervention, and participants were engaged and willing to participate in the program. Book excerpt: Diabetes Mellitus was a disease that made its presence known in all races, ethnicities, ages, and genders, for it did not discriminate. African Americans were the second largest prevalence of diabetes following Alaskan Natives/Americans Indians. Through the implementation of the levels of disease prevention, African Americans were seen in the secondary or tertiary levels due to lack of access to resources located in the primary level. Thus, African Americans were at high risk for delayed diagnosis of type 2 diabetes when they were already experiencing complications of the disease. The purpose of this study was to examine knowledge and attitude differences among African Americans and Caucasians prior to and after attending a diabetes education program considered a "best practice" in the field of diabetes. Racial differences in knowledge and attitudes were assessed specifically among newly diagnosed patients with type 2 diabetes. Through the use of a medical chart review, a quasi-experimental design, patients (n = 100, 50 African Americans, 50 Caucasians) completed pre-tests and post-tests assessing knowledge and attitudes about diabetes before and after completion of a diabetes education program. Some significant findings included lower knowledge levels among African American patients on pre-test (p = 0.012). Patients who attended individual education sessions had significantly higher knowledge levels on their post-tests (p = 0.022). Although 63% of the patients were female (37% male), no significance was found when to sex was used as a variable in relation to knowledge and attitude scores among African Americans and Caucasians. Recommendations for further research would be to use the same pre-test, post-tests instrumentation for evaluation, however at another American Diabetes Association (ADA) recognized diabetes education program to assess these findings on a larger scale.

In the original form, the questionnaire was considered by some registered dietitians to be as useful as the tools presently used such as a diet diary or 24 hour recall. Responses from the participants were used to refine the questionnaire into a more culturally appropriate assessment tool. Although the instrument may not be appropriate for all African Americans currently living in North Carolina, it could be useful for those whose dietary choices include predominantly southern-style cooking, or "soul food."

In a rousing indictment of the idea that notions of biological race should drive scientific inquiry, Sweetness in the Blood provides an ethnographic picture of biotechnology’s framings of Type 2 diabetes risk and race and, importantly, offers a critical examination of the assumptions behind the recruitment of African American and African-descent populations for Type 2 diabetes research. James Doucet-Battle begins with a historical overview of how diabetes has been researched and framed racially over the past century, chronicling one company’s efforts to recruit African Americans to test their new diabetes risk-score algorithm with the aim of increasing the clinical and market value of the firm’s technology. He considers African American reticence about participation in biomedical research and examines race and health disparities in light of advances in genomic sequencing technology. Doucet-Battle concludes by emphasizing that genomic research into sub-Saharan ancestry in fact underlines the importance of analyzing gender before attempting to understand the notion of race. No disease reveals this more than Type 2 diabetes. Sweetness in the Blood challenges the notion that the best approach to understanding, managing, and curing Type 2 diabetes is through the lens of race. The current study aims were to create a culturally informed intervention for AAs with T2DM and determine its efficacy for increasing treatment adherence, diabetes knowledge, self-efficacy, problem-focused coping, and mindfulness, and reducing emotional distress and HbA1c levels. A focus group with AAs with T2DM was conducted to understand barriers to treatment adherence. Information provided was incorporated into the development of a culturally informed T2DM adherence intervention for AAs. AAs with T2DM experiencing adherence difficulty (N = 20) recruited from a Family Medicine Clinic were randomly assigned to the intervention group (IG, n = 12) and an education comparison group (ECG, n = 8). Measures of adherence, knowledge, self-efficacy, and psychological factors were administered pre- (T1), immediately post- (T2), and 3-months post-intervention (T3). HbA1c levels were abstracted from participants' medical records at T1 and T3. The sample was 50% female, 70% with some college, 50% single, 75% unemployed, with an average age of 50.8 years and T2DM diagnosis length of 6.9 years. No significant group differences were observed on T2 or T3 measures. A significant time effect on healthy dietary behavior adherence reflected increases from T1-T2-T3 [Wilks' Lambda = 0.47, F(2, 10) = 5.52, p = .02]. Within groups, the IG exhibited a significant increase in physical activity adherence [t(11) = -2.34, p = .04] and mindfulness [t(11) = -2.53, p = .03] and approached a significant decrease in depressive symptoms [t(4) = 2.31, p = .07] from T1-T2; and a significant increase in healthy dietary behavior adherence [t(7) = 2.76, p = .03] and significant reduction in HbA1c [t(8) = 2.55, p = .03] from T1-T3. The ECG demonstrated a significant increase from T1-T3 on diabetes-specific self-efficacy [t(4) = 2.83, p = .05]. While no between-group differences were observed, findings indicate this intervention's components hold promise for changing adherence behaviors for rural AAs with T2DM. Future research with a larger sample is warranted to better elucidate treatment effects and adding a long-term follow-up component would allow assessment of longitudinal effects.

Social influences are commonly associated with diabetes-related outcomes. Purpose: The purpose of the study was to assess whether and how socio-contextual factors influence self-management and emotional distress in urban African American adults living with type-2 diabetes, as well as to assess whether social support facilitate relatedness need fulfillment within this population. The study also sought to gain an understanding of the psychosocial support needs and preferences of urban African American adults living with type-2 diabetes. Methods: The descriptive study employed a mixed-methods research design. African American adults were recruited to take a one-time written survey. Hypotheses one and two were tested using hierarchical linear regression, hypothesis three was assessed using VI a simple linear model, and hypotheses four and five were evaluated using path analysis. Four gender-stratified follow-up focus groups were also conducted, to confirm quantitative findings and gain insight into the support needs and preferences of urban African American adults living with type-2 diabetes. Study Variables: Independent study variables included social support, autonomy support, basic psychological needs (autonomy, competence, relatedness), and autonomous regulation. Primary dependent variables were diabetes self-management and diabetes-related emotional distress. Results: 155 eligible individuals (67 male, 88 female) were included in the quantitative analytic sample. 23 individuals (10 male, 13 female) participated in follow-up focus groups, yielding a total of 6 hours of audio-recorded focus group data. Autonomy and competence significantly predicted self-management and emotional distress in hypotheses one and two, respectively. Competence was the strongest predictor of selfmanagement and emotional distress among sample participants, followed by autonomy, but relatedness was not observed to be a significant variable in the prediction of study outcomes. Social support was observed to significantly predict relatedness need fulfillment in a simple linear model for hypothesis three. Data did not support hypothesis four and five. Qualitative findings contextualized the quantitative study findings. Conclusions: Future support programs should facilitate basic need fulfillment in urban African American adults living with type-2 diabetes. Relatedness is especially important in this collectivistic cultural group.

One of the groups of Americans that suffer more than any other race from diabetes are African Americans. The literature review has linked lack of knowledge, poor eating habits, decreased physical activity, and lack of perception of risk associated complications related to diabetes as the cause for the increasing occurrence of type 2 diabetes among African Americans. The diabetes evidence-based practice (EBP) change project was designed and implemented to increase diabetes knowledge and reduce risk. A diabetes assessment questionnaire was used to assess participants' diabetes knowledge pre-and post-intervention during a 6-week diabetes education program. The EBP change project has demonstrated that the implementation of a culturally sensitive diabetes education program targeting African Americans with prediabetes was effective in increasing diabetes knowledge post intervention. Comparing pre (54.3%) and post (94.3%) questionnaire data, the intervention was successful and met the expected EBP change project benchmark. The presented data indicated that there was an increase of 40% on the post diabetes intervention assessment questionnaire deeming the EBP change project was successful in increasing diabetes knowledge. The purpose of this paper is to discuss the EBP change project that improved diabetes knowledge among African Americans with prediabetes. Diabetes is the leading cause of blindness, kidney failure, and non-traumatic lower limb amputations. African Americans are more likely to suffer from diabetes related complications than any other race. The purpose of this study was to test a six-week culturally tailored diabetes self-management program for African Americans. The diabetes self-management intervention was conceptualized as the foundation that supported the changes in diabetes knowledge, self-efficacy, symptom management, and self-management activities. Thus, the specific aim of this study was to increase knowledge about diabetes, self-efficacy, symptom management, and self-management activities in African American adults diagnosed with diabetes. A one-group pre- and post-test was the design used in this study. The six-week intervention focused on areas of diabetes self-management based on the American Diabetes Association (ADA) guidelines: (a) introduction to diabetes, (b) healthy eating, (c) being active, (d), medications, (e) glucose monitoring and complications, and (f) symptom management. An African American nurse led the intervention. Adult African Americans with diabetes who were age 18 years old and older were recruited from two predominantly black churches in the south-eastern part of the United States. Sessions were held at each church directly after the morning and evening worship services. A total of 32 participants were recruited and 28 participants completed the six-week program. Health history and biomarkers (BMI, HBA1C) were measured at baseline. The major concepts were measured at baseline and post intervention using the Spoken Knowledge in Low Literacy Diabetes (SKILLD) scale, Diabetes Empower Scale- Short Form (DES-SF), Diabetes Symptom Checklist- Revised (DSC-r) scale, and the Summary of Diabetes Self-Care Activities (SDSCA) scale. At baseline, many person had low diabetes knowledge, self-efficacy, symptom management, and diabetes self-management activities. Findings revealed statistically significant positive improvements in diabetes knowledge, self-efficacy, symptoms management, and diabetes self-management activities from pre-intervention to post- intervention. Study participants gained an awareness of the need or diabetes self-management education. Designing and implementing culturally tailored diabetes self-management programs in faith-based settings are practical and necessary to reduce the disparity in diabetes for African Americans. More diabetes self-management studies are needed that target, recruit, and retain an adequate representation of African Americans. Also, more research is needed regarding African American cultures and learning styles so that programs can modify learning objectives to meet their needs."--Abstract from author supplied metadata.

Book excerpt: The purpose of this study was to develop a theory grounded in data of how diabetic COGIC members combine their Afro-Theistic faith with their knowledge of type 2 diabetes mellitus (DM), through Afro-Theistic social constructs. With grounded theory as a methodology, this study unequivocally followed Corbin and Strauss (2007) processes of developing a grounded theory. The interview of only five participants provided enough thick and rich data to saturate five categories that emerged on the way to developing a grounded theory. The five categories that emerged were: (1) Faith, the most prevalent, (2) Fix and (3) Fatalism, both actions, and (4) Fear and (5)Frustration, both emotions. A Theory in 3-D emerged as a portrait of the grounded theory developed along with the actions, interactions and the emotions relative the subculture (COGIC) of African-Americans with type 2 diabetes mellitus (DM). This Theory in 3-D captured a theory of Substantive Living. The Theory in 3-D also morphed into a model called Permeated Learning. In the process of developing a theory concerning DM, this study traces the African-American culture back to slavery. The tracing of the African-American culture is in two areas, theism and health. Theism provided the source of African-Americans subcultures through the divisions of churches. Most of the health discussions relates to diabetes in the African-American culture, but a discussed recreational exercise of the African-American ancestors is at the end of chapter 5.
