Available in PDF, EPUB and Kindle. Book excerpt: Arterial hypertension, coronary heart disease and heart fail ure are the commonest cardiovascular conditions to present in clinical practice. Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

Available in PDF, EPUB and Kindle. Book excerpt: Stroke is a major cause of death and the major cause of adult neurological disability in most of the world. Despite its importance on a population basis, research into the genetics of stroke has lagged behind that of many other disorders. However, the situation is now changing. Anincreasing number of single gene disorders causing stroke are being described, and there is growing evidence that polygenic factors are important in the risk of apparently "sporadic" stroke.Stroke Genetics provides an up-to-date review of the area, suitable for clinicians treating stroke patients, and both clinical and non-clinical researchers in the field of cerebrovascular disease. The full range of monogenic stroke disorders causing cerebrovascular disease, including ischaemicstroke, intracerebral haemorrhage, aneurysms and arteriovenous malformations, are covered. For each, clinical features, diagnosis, and genetics are described. Increasing evidence suggest that genetic factors are also important for the much more common multifactorial stroke; this evidence isreviewed along with the results of genetic studies in this area. Optimal and novel strategies for investigating multifactorial stroke, including the use of intermediate phenotypes such as intima-media thickness and MRI detected small vessel disease are reviewed. The book concludes by describing apractical approach to investigating patients with stroke for underlying genetic disorders. Also included is a list of useful websites.

Book excerpt: This volume is devoted to atherosclerosis, hypertension, and diabetes, three of the most important disease conditions in the world today. Nutritional intervention, cholesterol lowering agents, lipids themselves, particularly oxidized LDL, protein modification by ADP-ribose, bone marrow study, endothelial cell dysfunction, angiotensin, and the role of infection and inflammation are all discussed in the context of atherosclerotic cardiovascular disease. The hypertension section focuses on factors that may be responsible for high blood pressure, such as genetic predisposition, vascular hyperplasia and remodeling, insulin resistance, neurological aspects such as hypothalamic peptides. Also discussed are the possible contributions of the cellular function of the endothelium, nutrition, kidney dysfunction, leptin, and the brain. Novel routes of drug delivery for treatment of hypertension is also a focus. The risk factors and mechanisms responsible for diabetic vascular and cardiac dysfunction are discussed. Lipid profile changes and fibrinolysis in diabetic patients is detailed, along with adipogenesis, diabetic cardiomyopathy, energy metabolism in the diabetic heart, vanadate as an alternative to insulin, insulin resistance mechanisms, and neurotransmitters as targets for the prevention of cardiovascular disease and diabetes.

Book excerpt: Arteriosclerosis And Hypertension: With Chapters On Blood Pressure This book is a result of an effort made by us towards making a contribution to the preservation and repair of original classic literature. In an attempt to preserve, improve and recreate the original content, we have worked towards: 1. Type-setting & Reformatting: The complete work has been re-designed via professional layout, formatting and type-setting tools to re-create the same edition with rich typography, graphics, high quality images, and table elements, giving our readers the feel of holding a 'fresh and newly' reprinted and/or revised edition, as opposed to other scanned & printed (Optical Character Recognition - OCR) reproductions. 2. Correction of imperfections: As the work was re-created from the scratch, therefore, it was vetted to rectify certain conventional norms with regard to typographical mistakes, hyphenations, punctuations, blurred images, missing content/pages, and/or other related subject matters, upon our consideration. Every attempt was made to rectify the imperfections related to omitted constructs in the original edition via other references. However, a few of such imperfections which could not be rectified due to intentional\unintentional omission of content in the original edition, were inherited and preserved from the original work to maintain the authenticity and construct, relevant to the work. We believe that this work holds historical, cultural and/or intellectual importance in the literary works community, therefore despite the oddities, we accounted the work for print as a part of our continuing effort towards preservation of literary work and our contribution towards the development of the society as a whole, driven by our beliefs. We are grateful to our readers for putting their faith in us and accepting our imperfections with regard to preservation of the historical content. HAPPY READING!

Available in PDF, EPUB and Kindle. Book excerpt: Abstract: Atherosclerosis and hypertension, common causes of death in adults, may originate in childhood. Genetic, environmental, and nutritional factors may contribute to the disorders, but may also have preventive value in children. Investigations concerning the earliest origins of these disorders are presented, with a focus on those areas of research which represent potential medical strategies for prevention of atherosclerosis and hypertension. The predictive value of nutrition, obesity, cigarette smoking, plasma lipids and lipoproteins, serum cholesterol, heredity, blood pressure, and other risk factors is examined. Sociological and cultural dietary studies, such as the design of lipid profiles in children, are described; these are aimed at reducing cardiovascular risks in certain populations by fat-modified diets. The effect of socioeconomic status on the development of obesity in children is evaluated.
