It discusses the purpose, primary tasks, and attributes of the Army medical evacuation system. It also defines the differences between medical evacuation and casualty evacuation. Chapter 2 discusses the employment of medical evacuation resources and the coordination and synchronization required to effectively execute medical evacuation operations. This includes the medical evacuation request process, consideration for evacuation missions, support planning considerations, and evacuation in specific environments. Chapter 3 describes the mission, function and capabilities of medical evacuation units and elements as specified in the unit's table of organization and equipment. It also discusses the mission command headquarters to which they are assigned. Chapter 4 discusses the factors that establish the evacuation policy and the impact of the evacuation policy on Army Health System support. Chapter 5 provides insight and considerations into developing the operational and tactical medical evacuation plan that supports the combatant commander's mission. Chapter 6 describes the medical regulating system designed to ensure the efficient and safe movement of regulated patients to the appropriate military treatment facility by the most effective means. It also discusses the multi-Service responsibility and assets used to conduct this mission. Appendix A provides a summary of the Geneva Conventions and The Law of War. Appendix B provides an example of a medical evacuation plan as part of an operations order. Appendix C provides an example of the 9-line medical evacuation request format. For the Army, CASEVAC involves the unregulated movement of casualties using predesignated or opportune tactical or logistic aircraft and vehicles. These vehicles/rotary-wing aircraft are not staffed with medical personnel for en route care (unless augmentation is planned for in the operation plan). These vehicles/aircraft do not have organic medical equipment. If the combat medic is not available to provide care en route, the combat lifesaver (CLS) may accompany the casualties to monitor their conditions. Casualty evacuation is oftentimes the first step in a process that moves a wounded or injured Soldier from the point of wounding into the multifaceted Army Health System. Casualty evacuation can be accomplished by a variety of transportation platforms. These methods must be trained and practiced and include manual carries, litter evacuation, and the use of nonmedical vehicles. In order to determine the appropriate evacuation method, the Soldier executing the movement of the casualty must be able to determine the most effective mode of movement available to them to best meet their operational needs and the needs of the casualty. The movement of a casualty begins the evacuation chain which clears the operational area of casualties and moves the casualty through successively enhanced roles of medical care. Once the casualty is in the medical evacuation system, they receive en route medical care and emergency medical intervention, if required; this enhances the Soldier's prognosis and can reduce long-term disability. Upon arrival at an MTF the patient continues through the Army Health System until he is treated and released or continues through the Army Health System for more definitive care in the continental U.S.

Medical directors or designated supervising physicians should endorse these guidelines as a baseline, appropriately adjust components as needed, and responsibly manage individual unit medical missions within the scope of practice of their Critical Care Flight Paramedics, Enroute Critical Care Nurses, and advanced practice aeromedical providers. The medication section of this manual is provided for information purposes only. CCFPs may administer medications only as listed in the guidelines unless their medical director and/or supervising physician orders a deviation. Other medications may be added, so long as the unit supervising physician and/or medical director approves them. This manual also serves as a reference for physicians providing medical direction and clinical oversight to the CCFP. Treatment direction, which is more appropriate to the patient’s condition than the guideline, should be provided by the physician as long as the CCFP scope of practice is not exceeded. Any medical guideline that is out of date or has been found to cause further harm will be updated or deleted immediately. The Medical Evacuation Concepts and Capabilities Division (MECCD) serves as the managing editor of the SMOG and are responsible for content updates, managing the formal review process, and identifying review committee members for the annual review. The Standard Medical Operating Guidelines are intended to provide medical procedural guidance and is in compliment to other Department of Defense and Department of the Army policies, regulatory and doctrinal guidance. OVERVIEW Clinical Practice Guidelines (CPGs) are the backbone of the system-wide JTS Performance Improvement program. The CPGs compiled from DoDTR data and used by healthcare providers worldwide are largely responsible for the decreased Case Fatality Rate for the wars in Iraq and Afghanistan. CPGs are evidence-based and developed with experts in the military and civilian communities, deployed clinicians, Service trauma/surgical consultants, JTS leadership and formerly deployed Trauma Directors and Coordinators. JTS has a formalized process for developing, reviewing, updating, and approving CPGs. The guidelines are developed and implemented by clinical subject matter experts in response to needs identified in the military area of responsibility. CPGs were developed originally for U.S. Central Command. However, collaborative efforts are ongoing with the other Combatant Commands to customize CPGs to their COCOMs. INTRODUCTION TO THE JOINT TRAUMA SYSTEM (JTS) The Joint Trauma System (JTS) is the Department of Defense (DoD) authority for the military’s trauma care system. The vision of the Joint Trauma System is that every Soldier, Sailor, Marine and Airman injured on the battlefield will have the optimum chance for survival and maximum potential for functional recovery. To achieve this vision, in 2006, the JTS implemented programs for data -driven trauma system development and improvement in addition to the collection of trauma data. As part of its data collection efforts, the JTS maintains a registry of trauma patients who received care at medical treatment facilities (MTFs). Since 2007, this registry – known as the DoD Trauma Registry (DoDTR) – has documented demographic, injury, treatment, and outcomes data for all trauma patients admitted to any DoD MTF, regardless of whether the injury occurred during on-going military operations, and is the largest military trauma data source in the world. Development of the DoDTR began during the early years of the Global War on Terror (GWoT) when the need to systematically improve trauma care for combat wounded resulted in the impromptu creation of a demonstration registry, known then as the Combat Trauma Registry (CTR). The CTR was constructed by the Center for AMEDD Strategic Studies (CASS); trauma-related information was initially abstracted into it from paper medical records received from trauma nurse coordinators (TNCs) at Landstuhl Regional Medical Center (LRMC) in Germany. Shortly after the demonstration program started, the Army Surgeon General approved its transition to an operational mode, leading to the formation of the Joint Theater Trauma System (JTTS) and, eventually, the Joint Trauma System (JTS).

Variances will exist between what is included in this publication and the actual modified TOEs of deployed units. These variances may be due to updates of areas of concentration (AOCs), military occupational specialties (MOSs), and military grades and modifications made to unit's TOE which result in the unit's modified TOE or updates to the base TOEs reflecting Total Army Analysis findings and judgments. Where significant differences exist in the base TOEs of the BCTs, an explanation of these differences is provided. This publication applies to the Active Army, Army National Guard/Army National Guard of the United States, and the United States Army Reserve, unless otherwise stated. Army Techniques Publication ATP 4-02.42 Army Health System Support to Stability and Defense Support of Civil Authorities Tasks June 2014 examines the various situations in which Army medical personnel may be required to provide support for stability and DSCA tasks. While the Department of Defense (DOD) is not a provider of first resort in disasters, requests for support from U.S. forces may be required when military-unique capabilities (such as lift capability, engineering, or deployable medical support) exist that can expedite relief efforts during urgent, life-threatening situations. This manual is a two-part publication. Part one of the ATP discusses AHS support to stability tasks and part two covers medical support to DSCA tasks. Part two of the manual, which provides a separate discussion of AHS support to DSCA tasks, the National Response Framework (NRF), National Disaster Recovery Framework, and the DOD's role in the National Disaster Medical System (NDMS). Army Techniques Publication 4-02.42 consists of seven chapters- Chapter 1 provides an overview of stability in operations, the primary stability tasks, national and DOD-level guidance, and the importance of ensuring that AHS support to stability tasks is regionally focused and conducted in consonance with the combatant commander's theater engagement strategy. Chapter 2 discusses the Department of State's Post-Conflict Reconstruction Essential Tasks matrix as it relates to the Army primary stability tasks. The chapter also provides the doctrinal description for three of the five Army stability tasks, medical aspects of the supporting initial and transformational response tasks, and corresponding health service support (HSS) and force health protection (FHP) considerations for each task. Chapter 3 discusses the employment of AHS assets in support of the primary stability tasks, the medical aspects of building partner capacity, the role of civil affairs, legal considerations, and AHS support to operations with a stability focus. Chapter 4 provides medical planning considerations for AHS support to joint operations and stability tasks to include transition and end state considerations. Chapter 5 provides a brief overview of the primary DSCA tasks, national and DOD-level guidance. This chapter also provides a brief description of medical aspects of the NRF, National Disaster Recovery Framework, and the NDMS. Chapter 6 provides legal considerations that may apply when providing medical support to DSCA tasks and a discussion of the support provided for each of the 10 medical functions. Chapter 7 describes the interorganizational and interagency coordination required in support of DSCA tasks, the process for requesting DOD assistance for support to civil authorities, some of the NDMS medical resources that may be employed during a disaster, as well as some of the participating organizations that may be involved in the relief effort.

Book excerpt: The Army Health System (AHS) is a complex system of interrelated and interdependent systems which provides a continuum of medical treatment from the POI or wounding through successive roles of health care to definitive, rehabilitative, and convalescent care in the CONUS, as required. Medical evacuation is the system which provides the vital linkage between the roles of care necessary to sustain the patient during transport. This is accomplished by providing en route medical care and emergency medical intervention, if required, and to enhance the individual's prognosis and to reduce long-term disability. Medical evacuation occurs at the tactical, operational, and strategic levels and requires the synchronization and integration of service component medical evacuation resources and procedures with the DOD worldwide evacuation system operated by the United States Transportation Command (USTRANSCOM). Army medical evacuation is a multifaceted mission accomplished by a combination of dedicated ground and air evacuation platforms synchronized to provide direct support (DS), general support (GS), and area support within the joint operations area (JOA). At the tactical level, organic or DS medical evacuation resources locate, acquire, treat, and evacuate Soldiers from the POI or wounding to an appropriate MTF where they are stabilized, prioritized, and, if required, prepared for further evacuation to an MTF capable of providing required essential care within the JOA. Although the most recognized mission of Army medical evacuation assets is the evacuation and provision of en route medical care to battlefield wounded, the essential and vital functions of medical evacuation resources encompass many additional missions and tasks that support the JHSS system. Medical evacuation resources are used to transfer patients between MTFs within the JOA and from MTFs to United States Air Force (USAF) mobile aeromedical staging facilities (MASFs) or aeromedical staging facilities (ASFs); emergency movement of Class VIII, blood and blood products, medical personnel and equipment; and serve as messengers in medical channels. Medical regulating provides the interface with the DOD worldwide medical evacuation system by determining the patient's destination (the MTF best suited to provide the required care) and scheduling the means to transport the patient with the required en route medical care. Formal medical regulating begins at Role 3, however technological advances in information management (IM)/information technology (IT) are permitting this capability to be used at Role 1 and Role 2 MTFs in some situations.

It will be necessary for a recovery support activity to contact the higher, lower, or adjacent headquarters (both sustainment and operational) to coordinate support, report status, request technical assistance, or request additional resources. This manual will present the roles and mission of the various recovery organizations so that proper coordination can be conducted. The guidelines in this publication should be followed as closely as possible within the constraints and restrictions of the tactical situation. ATP 4-31, Recovery and Battle Damage Assessment and Repair (BDAR), is the revision of FM 4-30.31, Recovery and Battle Damage Assessment and Repair. ATP 4-31 provides an overview of the battlefield recovery, and battle damage assessment and repair for the fundamental purpose of returning combat assets to the battlefield as soon as possible. It also explains the difference between recovery operations with its different types and methods and the battle assessment and repairs. This publication also reviews the rigging procedures and the utilization of the mechanical advantage to accomplish the mission. Overviews the recovery methods, techniques and the safety precautions associated with each recovery operation. Major changes from FM 4-30.31 include an improved hand and arm signals for day and night recovery operations. ATP 4-31 is comprised of four chapters: Chapter 1 discusses the battlefield recovery with its various types and battle damage assessment and repairs; it discusses the various types of recovery and the responsibility of the owning units. Chapter 2 explains the rigging methods and techniques, how to take advantage of the mechanical advantage during various rigging configurations with equipment readily available. Chapter 3 covers huge varieties of recovery techniques for different obstacles, overturned, and mired situations and emphasizes safety during the recovery operations. Chapter 4 covers the various improvised repair procedures in order to rapidly return disabled equipment to operational condition in wartime by expediently repairing, bypassing, and restoring minimum function to essential systems.

Army Health System resources (personnel and equipment) are organic to a variety of organizations within the brigade combat teams (BCTs). The numbers of personnel, medical equipment, and unit capabilities for providing organic AHS support also varies depending upon the parent formation/organization. It is essential for AHS planners to understand how AHS resources are arrayed across the battlefield and the capabilities and limitations of the various medical assets that are used across the range of military operations in support of the warfighting functions in the conduct of unified land operations and in pursuit of decisive action in any operational environment (OE). To facilitate this understanding, this publication uses the infantry brigade combat team (IBCT) base table of organization and equipment (TOE) for illustrative purposes only. Variances will exist between what is included in this publication and the actual modified TOEs of deployed units. These variances may be due to updates of areas of concentration (AOCs), military occupational specialties (MOSs), and military grades and modifications made to unit's TOE which result in the unit's modified TOE or updates to the base TOEs reflecting Total Army Analysis findings and judgments. Where significant differences exist in the base TOEs of the BCTs, an explanation of these differences is provided.
