Book excerpt: The Republic of Therapy tells the story of the global response to the HIV epidemic from the perspective of community organizers, activists, and people living with HIV in West Africa. Drawing on his experiences as a physician and anthropologist in Burkina Faso and Côte d’Ivoire, Vinh-Kim Nguyen focuses on the period between 1994, when effective antiretroviral treatments for HIV were discovered, and 2000, when the global health community acknowledged a right to treatment, making the drugs more available. During the intervening years, when antiretrovirals were scarce in Africa, triage decisions were made determining who would receive lifesaving treatment. Nguyen explains how those decisions altered social relations in West Africa. In 1994, anxious to “break the silence” and “put a face to the epidemic,” international agencies unwittingly created a market in which stories about being HIV positive could be bartered for access to limited medical resources. Being able to talk about oneself became a matter of life or death. Tracing the cultural and political logic of triage back to colonial classification systems, Nguyen shows how it persists in contemporary attempts to design, fund, and implement mass treatment programs in the developing world. He argues that as an enactment of decisions about who may live, triage constitutes a partial, mobile form of sovereignty: what might be called therapeutic sovereignty.

Every renal unit needs to have a reference text of this kind and urine microscopy is an important diagnostic aid in nephrology. After the 'Historical Introduction' by J Stewart Cameron, chapter 1 descibes the methods used to prepare and analyse the urine samples. Chapter 2, which is the most important section of the book, describes with the help of 249 illustrations (over 200 in full colour) the elements of the urinary sediment. Chapter 3 deals with the urinary sediment of the normal subject, while chapter 4 deals with the urinary changes which are observed in the main diseases of the kidney and urinary tract. Chapter 5 describes the changes of the urinary sediment caused by drugs. Chapter 6 is about the interpretation of the urinary findings shown in the previous chapters. Chapter 7 covers the new systems to analyse the urinary sediments. An appendix shows how to adjust the microscope to analyse the urine specimens. The primary audience is practising nephrologists and nephrologists in training, and also those in internal medicine rotating through the renal unit who perform routinely the analysis of the urinary sediment. Clinical pathologists would also find this an attractive and useful book. The illustrations are outstanding and additional clinical information has been added in this new edition. In just over a decade the public and official response to the disease has resulted in the development of a whole network of organizations devoted to the study, containment, and practical treatment of AIDS. In this important and original analysis of AIDS policy, Virginia Berridge examines the speed and nature of the official (and unofficial) response to this new and critical historical event. The policy reaction in Britain passed through three stages. From 1981-1986 the outbreak of a new contagious disease led to public alarm and social stigmatization, with a lack of scientific certainty about the nature of the disorder. AIDS was a new and open policy area - there were no established departmental, local, or health authority mechanisms for dealing with the problem. This was a period of policy development from below, with relatively little official action and many voluntary initiatives behind the scenes. This phase was succeeded in 1986-1987 by a brief stage of quasi-wartime emergency, in which national politicians and senior civil servants intervened, and a high-level political response emerged. The response was a liberal one of `safe sex' and harm minimization rather than draconian notification or isolation of carriers. The author demonstrates that despite the `Thatcher revolution'in government in the 1980s, crisis could still stimulate a consensual response. The current period of `normalization' of the disease sees panic levels subsiding as the rate of growth slows and the fear of the unknown recedes. Official institutions have been established and formal procedures adopted and reviewed; paid professionals have replaced the earlier volunteers. The 1990s have seen change in the liberal consensus towards a harsher response and the partial repoliticization of AIDS. In this fascinating and scholarly account, Virginia Berridge analyses a remarkable period in contemporary British history, and exposes the reaction of the British political and medical elites, and of the British public to one of the most challenging issues of this century.

While the motivation behind creating an HIV narrative is sometimes the disseminate facts about HIV and STIs, far more often it is the sexy ratings a show can receive by including a “taboo” or controversial topic. As a result, while some education is provided to audiences, far more shows are found only perpetuating misinformation and stereotypes. As a result viewers, young populations especially, continue to believe they are not at risk. HIV on TV: Popular Culture’s Epidemic offers a discussion of how HIV has permeated popular culture. News broadcasts, movies, television shows, even music lyrics have imbedded messages about HIV. Examining over 35-years of the HIV evolution on television this book offers a critical lens for examining how medial topics, specifically HIV, are covered in the media. Cutting across three common genres (news, drama, and comedy), characterizations, contexts, and themes are critically analyzed to uncover what each genre has contributed to audience’s understanding of risk, and what it is like to live with HIV. In total, the book offers three perspectives of the lessons presented about HIV. First, is the view from the screen; asking what the characters themselves say to the viewer. Second the book shares results from interviews with viewers themselves who have recalled seeing the shows mentioned. Finally, the book offers thoughts and reflections from writers, producers, and actors involved in the narratives. Providing the greatest insight, an interview with Daniel Franzese (Mean Girls, Road to Recovery, Looking) offers his experience in playing multiple roles as someone living with HIV and a challenge to media writers to be cautious in how they choose to write HIV into a storyline.
