Available in PDF, EPUB and Kindle. Book excerpt: In 1985, volume 74 of the Springer-Verlag Handbook of Experimental Phar macology, under the editorship of H. -H. Frey and D. Janz, appeared. In this volume the then available data on the topic of antiepileptic drugs were col lated and analysed. Over the intervening years knowledge in this area has grown progressively. More new antiepileptic drugs than the total number of agents that were in common use 15 years ago have in the interval either come on to the market or are about to do so. As well, further agents are at a fairly advanced stage of development, whilst the already established drugs have by and large held their places in clinical practice. Knowledge of epileptogenesis has advanced considerably. The mechanisms of action of antiepileptic drugs at the molecular level and in various animal models of epileptic seizures and of the epileptic state are much better understood than they were previously. As well, more information is available concerning the natural history of human epilepsy, and this knowledge is important in making optimal use of the various agents that are now available. Therefore, it has seemed appropriate at this stage in the evolution of knowledge to produce a second volume dealing with Antiepileptic Drugs in the Handbook of Experimental Pharmacology series.

More than half of epilepsies start before the age of 20 years and nearly 25% of them are refractory. Two problems arise: - It is essential to treat them so that brain development continues as normally as possible. - There are few drugs available. This work outlines 6 molecules in detail that are specifically used in epileptic encephalopathies. Every aspect of these molecules is discussed, including drug development, indication, efficacy, cost, etc. Drafted by international experts in the field of epileptology, this book provides all the necessary information on orphan drugs and their clinical use.

Available in PDF, EPUB and Kindle. Book excerpt: Blood level determinations of antiepileptic drugs are not available to the majority of physicians who prescribe these medications for the treatment of epileptic patients, yet rapid accurate methods for quantitative assay of these drugs have been developed. Knowledge of the level of the drug in the blood is essential in the treatment of those patients who develop symptoms and signs of intoxication, show unexpected toxicity on multiple antiepileptic drugs, or totally fail to respond to therapeutic doses of a drug. An objective documentary review of the literature is given for an understanding of the relationship of blood level determinations to the variations in absorption, metabolism, excretion, distribution, degree of clinical effectiveness, drug interaction, and toxic side effects. Comprehensive tables of the blood level values that have been published to date for the major antiepileptic drugs are presented. The development of laboratory methods for the quantitative assay of each antiepileptic drug is reviewed in an historical perspective, leading from bromides to ethosuximide and gravimetric analysis to gas-liquid chromatography. Inclusive tables for methodology are also given.

Book excerpt: Since the fortuitous discovery of its anticonvulsant activIty in 1962, valproate has established itself worldwide as a major antiepileptic drug against several types of epileptic seizures. Clinical experience with valpro ate has continued to grow in recent years, including use of valproate for diseases other than epilepsy, for example in bipolar disorders and migraine. In this volume on valproate emphasis is placed on the scientific back ground leading to the discovery of val pro ate, its subsequent pharmacologi cal and toxicological characterization, and its clinical development into one of the most widely and successfully used anti epileptic drugs, a real mile stone in drug therapy. The current state of knowledge of valproate will be reviewed by experts in the field, including new hypotheses about its mecha nisms of action, its metabolism into pharmacologically active metabolites, its unique distribution characteristics, its unwanted hepatotoxic and terato genic adverse effects and its various clinical uses. Furthermore, the wide variety of available pharmaceutical formulations of valproate, including novel controlled-release formulations, will be outlined. The monograph is aimed at a broad readership, particularly neurologists, psychiatrists and basic scientists working in the field of epilepsy research. Because the monograph also deals with structure-activity relationships of valproate as well as of its metabolites and analogs, the book should also serve for rese archers working in medicinal chemistry, particularly in the pharmaceutical industry.

Book excerpt: "He who is faithfully analysing ... epi lepsy is doing far more than studying epilepsy" Hughlings Jackson Modifying this well-known statement by Jackson, one could say today: "He who is faithfully analysing anti-epileptic drugs is doing far more than studying anti-epileptic drugs". For these drugs not only serve to prevent epileptic fits and thus advance the treatment of epilepsy, they are also effective in the treatment of cardiac arrhythmias and trigeminal neuralgia. Furthermore, clinical pharmacologists consider anti-epi leptic drugs as model drugs in pharmacokinetics and pharmocodynamics, since reliable methods are available for their determination and their effects and side effects can be defined. The methods of estimating of drugs in body fluids provide a tool that enables us to throw light on many obscure relationships in pharmaceutical treatment. Now that we can study the pharmacokinetics and interaction of drugs in man, many hypotheses based on clinical experience alone may well be eliminated or corroborated. The grow ing body of knowledge will make us more careful about the administration of drugs in combination. Now that we can study how biological parameters interfere with drug action, we may perhaps proceed to the scientific analysis of many clinical observations that suggest the importance of such factors as age, sex, menstrual cycle, pregnancy, fever, diet, stress, sport, climate, and altitude.
