Passive immunotherapy using heteroantisera for the treatment of cancer in animals and humans has been studied for over 50 years. Attempts have been made to treat animal tumors with sera from immunized syngeneic, allogeneic, or xenogeneic animals. A number of studies of passive immunotherapy using heterologous antisera in humans have also been performed. These studies have generally been attempted in patients with large tumor burdens, and as would be expected, results have been transient at best. A wide variety of solid tumors as well as leukemias and lym phomas have been treated with antisera raised in sheep, horses, rabbits, and goats. Problems such as anaphylaxis, serum sick ness, and severe cytopenias have been encountered with these antisera. There are a number of potential mechanisms by which unconju gated antibodies might be cytotoxic to tumor cells. Antibodies bound to the cell surface membrane of tumor cells may lead to cell lysis by complement-dependent or antibody-dependent cellu lar cytotoxicity. Circulating tumor cells bound by antibody may be more susceptible to phagocytosis by the reticuloendothelial system. Antibody bound to the cell surface membrane of tumor cells may enhance immunogenicity of the tumor cell leading to activation of the host's immune system.

Book excerpt: The foundation for targeted therapy of cancers driven by members of the ErbB oncoprotein family was established initially by the demonstration that ectodomain binding monoclonal antibodies (mAb) could disable the protein kinase encoded by the HER2/neu oncogene. Homomeric and heteromeric erbB kinases play critical roles in the development of cancer and in the spread of early lesions. In particular, antibodies targeting the p185erbB2/neu receptor provide major clinical benefits in the treatment of breast cancer and also stomach cancer. As suggested by our study with oncogenic neu transgenic mice, anti-p185erbB2/neu antibodies are also effective in preventing the tissue hyperplasia that precedes tumorigenesis, tumor growth and the dissemination of ErbB2/neu kinase-positive cells into other tissues. As a therapeutic principle, “reversion of phenotype” for established tumors and “prevention” of tumorigenesis and spread can explain the basis for the benefits invoked by therapeutic and adjuvant therapies for breast cancer patients after cancers are surgically removed. These emerging principles being enlightened by ongoing studies of monoclonal antibody therapy will continue to provide guidance for the development of new targeted therapies for resistant tumors that arise after treatment.

Book excerpt: Cancer is one of the main global health problems. Most cancer patients die from metastases rather than from the primary tumor. Therefore, it is of utmost importance to develop novel treatment options to prevent and treat metastases. A promising treatment option for cancer is immunotherapy. Immunotherapy aims to stimulate the patient’s own immune system to destroy the tumors. There are several types of immunotherapy, including antibody therapy. Antibodies have both direct effects and indirect effects. Antibodies bind tumor cells to reduce their growth (direct effect). Simultaneously, antibodies can bind immune cells, including macrophages, natural killer (NK) cells and neutrophils, creating a bridge between these cells and tumor cells. In this way, the immune cells are stimulated to eliminate the tumor cells (indirect effect). In my thesis I investigated multiple strategies to enhance the efficacy of antibody therapy. In Part I of my thesis I focused on the prevention of metastases. In Chapter 2 we discuss the process through which tumor cells detach from the primary tumor in the intestine and grow out as metastasis in the liver. As tumor cells detach from the tumor and enter the blood circulation, they are referred to as circulating tumor cells. It has been described that surgical removal of tumors in the intestine results in an increased number of circulating tumor cells. This increases the chance to develop metastases. Therefore, elimination of circulating tumor cells represents a promising treatment option to prevent development of metastases. In Chapter 3 we investigated if, for this purpose, we could use antibodies against the molecule EGFR that is highly expressed on tumor cells compared to healthy cells. We demonstrated that treatment with anti-EGFR antibodies stimulates macrophages to eat tumor cells. Therefore, we advise to treat patients with anti-EGFR antibodies prior to surgical removal of tumors in the intestine.

The idea behind this breakthrough is to stimulate the patient's own immune system to recognize the cancer cells and destroy them. In this therapy, the antibodies are known to be powerful medications to activate the immune system in different ways. They circulate throughout the body until they discover a substance that body recognize as alien i.e. antigen and bind to them. Similarly, cancer cells often have molecules on their surface known as tumor-associated antigens. The researchers can design many clones of the antibody that only target a certain antigen type such as one found on tumors or cancer cells. Then, these are used as an effective drug for treating cancer. Thus, the antigen specific antibody interactions play a vital role in cancer immunotherapy. In this study, we propose a dynamic model to represent the population of antigens and antibodies in cancer patients; in particular we focus on the antigen-specific-antibody interactions to elicit an immune response that leads to the death of cancer cells. We formulate a terminal control problem where the schedule and doses of these antibodies are considered as control variables. The objective functional has been formulated as a measure of antigen population at the end of the treatment period. Pontryagin minimum principle (PMP) has been used to obtain the optimal control policies. For illustration, a series of numerical results is presented showing the effectiveness of immune therapy for cancer treatment corresponding to the different scenarios, choices of parameters and treatment periods. The results indicate that the control doses are followed by the emergence of antigen population. This approach would be potentially applicable to determine and prescribe the optimal doses and schedules for cancer patients.
