Book excerpt: The decision of Harvey Cushing to leave general surgery and concentrate on the infant field of central nervous system surgery was in retrospect a landmark in the history of neurosurgery. His concentrated work, and also that of his colleague Walter Dandy, originated with the desires of both pioneers to understand surgical anatomy and neurophysiology. The fundamental knowledge and surgical techni ques that they provided became the standard of excellence for several generations of neurosurgeons; so much so that the general belief was that the surgical techniques could not be improved upon. Twenty-five to thirty years ago microtechniques began to appear in a few surgical research centers, they were then gradually applied to clinical neurosurgery and have contributed to a new level of understanding in surgical anatomy and neurophysiology. We are now fortunate to have a new standard of morbidity and mortality in the surgical treatment of intrathecal aneurysms, angiomas, and tumors. It has been said that microneurosurgery was reaching its limits, especially when treating lesions in and around the cavernous sinus and skull base; those lesions notorious for involvement of the dural and extradural compartments, with a tendency to infiltrate adjacent nerves and blood vessels. The dangers of uncontrollable hemorrhage from the basal sinuses and post-operative CSF rhinorrhea appeared unsurmountable. The lateral aspects of the petro-clival region have been of interest to a few pioneering ENT surgeons and neurosurgeons but the cavernous sinus in most respects has remained the final unconquered summit.

It is only a short time ago that the major concepts of the anatomy of the so-called cavernous sinus were described in detail. Surgical interventions in this region are very complex, they are time-consuming and require an extensive back ground of experience in surgery of the cranial base. Pioneer anatomical studies of the parasellar region done by Taptas, and the daring direct operative approach introduced by Parkinson pro moted the development of modern neuroradiological intervention proce dures, which were initiated by Serbinenko and further refined by Debrun, Vifiuela and others. The technique of the detachable balloon catheter stimulated surgeons to proceed with the direct operative approach to lesions of the parasellar region. Today, it is hard to imagine a successful man agement of vascular pathologies of this region without a complementary use of the two techniques.

Book excerpt: As in the acclaimed prior editions, Endoscopic Sinus Surgery, Fourth Edition focuses strictly on anatomy, 3D reconstruction, and step-by-step surgical techniques. Written by Peter-John Wormald, the richly illustrated text details anatomy and operative treatment of sinonasal conditions such as nasal polyposis, chronic rhinosinusitis (CRS), Samter's triad, eosinophilic mucus CRS, exophthalmos, acute orbital hemorrhage, orbital subperiosteal abscess, and a wide array of tumors. The fourth edition reflects simplified anatomical nomenclature published in 2016 by Wormald et al, including reclassification of cells in the frontal recess and the extent of surgery of frontal sinus surgery (EFSS). This enables clearer understanding of impacted anatomy, most notably the frontal sinus drainage pathways. New and revised evidence-based procedures include the mega-antrostomy and pre-lacrimal approach to the maxillary sinus and use of anterior based pedicled flaps for frontal drillout. Refinements in other surgical techniques include endoscopic medial maxillectomy and resection of sinonasal neoplasms. Anatomy and surgical approaches elucidated through 1,000 images including CTs, illustrations, and diagrams Exquisite dissections by Rowan Valentine, who worked in the anatomy laboratory of the late Albert L. Rhoton Jr., internationally renowned author, educator, brain anatomist, and neurosurgeon Delineation of variations in frontal recess anatomy and difficult regions adjacent to the sinuses provides important surgical guidance Seventy operative videos detail potential anatomical variations seen in the frontal sinuses, ancillary procedures such as DCR, orbital decompression, CSF leak closure, and skull base surgical approaches This classic reference is a must-have for otolaryngologist-head and neck surgeons, as well as residents and fellows seeking a robust tutorial on the latest functional endoscopic sinus surgery (FESS) techniques. It enables 3D understanding of diseased anatomy, associated surgical treatment decision making, and safer surgery.

Book excerpt: The sellar region and paranasal sinuses constitute the anatomical sections of the skull base in which pathological entities warrant interdisciplinary management. Processes originating in the paranasal sinuses can reach and involve the skull base in and around the sella, sometimes not respecting the natural dural boundary. On the other hand, lesions involving the sellar block, such as pituitary adenomas and meningiomas, can also extend downwards into the paranasal sinuses. The orbit and cavernous sinus may be subject to involvement and infiltration by both paranasal and sellar pathology. The advancement and new achievements of modern diagnostic procedures, such as high-resolution CT, three-dimensional reconstruc tion, MRI, and MRI angiography, as well as the detailed selective angiographic protocols and endovascular techniques, have increased the possibilities for surgical management of this type of pathology with extra- and intracranial involvement. Long-standing and intense inter disciplinary work has led to sophisticated operative approaches which for benign tumors allow total excision with preservation of structures and function, and for some malignant lesions permit an en bloc resec tion via a combined intracranial-extracranial approach. This volume reflects the work and scientific exchange which took place during the IV International Congress of the Skull Base Study Group, held in Hanover. Leading authorities in the basic sciences including anatomy joined with diagnosticians, clinicians, and surgeons from different fields to evaluate the state of the art of this topic in skull base surgery.

Available in PDF, EPUB and Kindle. Book excerpt: Recent years have seen remarkable advances in neurosurgery, and these advances have increasingly illuminated the importance of the role of the venous system. Until now, the main focus of neurosurgery has been directed to arteries such as those feeding tumors, but it is now clear that some lesions that had been considered contusional hemorrhage caused by surgical manipulation were in fact hemorrhagic infarctions resulting from impaired venous circulation. In addition, the concept of venous hypertension has emerged as a mechanism of neurological impairment in patients with arteriovenous malformations in the brain or spinal cord. Great strides have also been made in neuroradiological diagnosis with the introduction of magnetic resonance imaging. For example, the exact location of tumors and the degree of invasion of sinuses can now be determined prior to surgery. Thus, new operative techniques are being developed that benefit from such detailed information. This book presents material from a wide range of fields, including embryology, anatomy, pathophysiology, neuroradiology, diagnosis, and treatment. It is hoped that the reviews of pioneering work on the intracranial venous system presented here will prove invaluable for further development of new operative techniques.

Book excerpt: THE DEFINING WORK IN NEUROSURGERY, REISSUED FOR A NEW GENERATION OF TECHNICAL EXCELLENCE Cranial Anatomy and Surgical Approaches is the master work of the legendary neurosurgeon Albert L. Rhoton, Jr. -- a distillation of 40 years of work to improve safety, accuracy, and gentleness in the medical specialty the author helped shape. Newly reissued and featuring more than 2000 full-color illustrations, this definitive text on the microsurgical anatomy of the brain remains an essential tool for the education and enrichment of neurosurgeons at any career stage. It fulfils its author's hopes to make, in his words, the "delicate, fateful, and awesome" procedures of neurosurgery more gentle, accurate, and safe. Across three sections, Cranial Anatomy and Surgical Approaches details the safest approaches to brain surgery, including: · Micro-operative techniques and instrument selection · Microsurgical anatomy and approaches to the supratentorial area and anterior cranial base, including chapters on aneurysms, the lateral and third ventricles, cavernous sinus and sella. · Anatomy and approaches to the posterior cranial fossa and posterior cranial base, including chapters on the fourth ventricle, tentorial incisura, foramen magnum, temporal bone, and jugular foramen · Supra- and infratentorial areas, including chapters on the cerebrum and cerebellum and their arteries and veins
