Available in PDF, EPUB and Kindle. Book excerpt: Doctors Pellet, Cannoni and Pech have joined forces to write a masterly work on the subject of otoneurosurgery and tumors of the base of the skull. The anatomic illustrations, derived from their meticulous dissections, are of excellent quality and a credit to the famed tradition of French neuroanatomists of the past. It is evident that the operative techniques described by these surgeons indicate that they have spent a great deal of time in the dissecting-room and that they have completely mastered the temporal bone. A perfect knowledge of the fundamental anatomy of the petrous bone is essential before embarking on the difficult surgical procedures in this region of the brain and base of the skull. Reading this work makes it abundantly clear that the results of this type of sur gery are vastly improved when neurosurgeons and otologists combine their skills. Each of these specialists brings his unique experience and entire knowledge to the operation for the greater benefit of the patient. We wish to congratulate Doctors Pellet, Cannoni and Pech on the publication of this excellent work. March 1990 William F. House, MD William E. Available in PDF, EPUB and Kindle. Book excerpt: No special field of surgery dealing with the cranial nerves exists today. This is not surprising in view of the characteristics of this group of morphologically and topo graphically heterogenous nerves. Morphologically we must differentiate between central nerves (I, II and VIII) and the so-called peripheral nerves (nn. III to VII and IX to XII), in which post-lesion rgeneration is quite different. Anatomo-topographi cally we must consider an intracranial and an extracranial part of each cranial nerve. For practical reasons at operation, further subdivisions of the intracranial course of cranial nerves are to be distinguished in the anterior, middle and posterior cranial fossae as well as within the petrous bone. This underscores the extensive tasks awaiting surgeons operating in the ventral part of the brain and facial skull as well as in the more dorsal part of the skull and neck. This very wide field cannot be covered by a single surgical discipline alone. In our opinion, considerable progress has been made in surgery of the cranial nerves only in recent years. This may be explained by the increased mastery of microsurgical techniques by all surgeons in terested in the surgery of the base of the skull as well as with the initiation of more interdisciplinary consultation and jointly performed operations. Possibilities of fu ture development can be discerned in the text. The base of the skull separating the extra-and intracranial part of cranial nerves should not be a barrier but a connect ing link.

Available in PDF, EPUB and Kindle. Book excerpt: Idiopathic Hemifacial Spasm constitutes a severe disability both from a cosmetic and psychological point of view and functionally for the daily life. Until recently this peculiar disease had remained enigmatous and was resistant to any kinds of treatments. Over the last two decades important advances have been made in the comprehension of its mechanisms and the efficacy of its treatment. Botulinic toxin revealed to be helpful although its effects are most often transient. Microvascular decompression – suppressing the neuro-vascular conflict almost constantly found at the root exit zone of the facial nerve from the brain stem – has been recognized as the curative treatment of this disease. A wide range of specialists, namely neurologists, ophthalmologists, ENT-surgeons, clinical neurophysiologists, neuroradiologists and of course neurosurgeons are involved in the diagnosis and the management of Hemifacial Spasm and will be interested in this multidisciplinary review of the topic.

Book excerpt: Diseases of the Sinuses: A Comprehensive Textbook of Diagnosis and Treatment, 2nd Edition, offers the definitive source of information about the basic science of the sinuses and the clinical approach to sinusitis. Since the widely praised publication of the first edition, understanding of sinus disease has changed dramatically, mainly as a result of recent developments and new discoveries in the field of immunology. This updated and expanded edition is divided into sections addressing, separately, the pathogenesis, clinical presentation, medical and surgical management of acute and chronic rhinosinusitis. Special entities such as autoimmune-related sinusitis, allergy and sinusitis, and aspirin-exacerbated respiratory disease are discussed in separate chapters. The role of immunodeficiency is also addressed. The management section has been fully updated to incorporate new medical modalities and surgical procedures. Developed by a distinguished group of international experts who share their expertise and insights from years of collective experience in treating sinus diseases, the book will appeal to anyone who has an interest in sinus disease, including both physicians and allied health professionals. Internists, pediatricians, allergists, otolaryngologists and infectious disease specialists will find the book to be an invaluable, comprehensive reference. Physician assistants and nurse practitioners who work with specialists who treat sinus disease will also benefit from the book.

Book excerpt: Dr. McCarthy and his colleagues at NYU have developed techniques of distracting various components of the craniofacial skeleton. These techniques hold considerable promise for reconstructive surgery, allowing the surgeon to make precise changes in the lengthening and shape of the new craniofacial bone. This is the first volume of its kind, bringing together the pioneers of these revolutionary techniques: an international group of experts present their clinical and laboratory experience with distraction of the cranial vault, orbit, midface and zygoma, and mandible, as well as the principles of distraction, biomechanics, and the physiology of bone healing and remodelling. With two hundred illustrations, this is a must for all reconstructive plastic, maxillofacial, and oral surgeons.

Book excerpt: State-of-the-art book provides hands-on guidance on the latest endothelial keratoplasty techniques A significant number of eye disorders result from deficiencies in the endothelial layer of the cornea. Endothelial keratoplasty (EK) helps spare patients from months of refractive adjustments, lengthy recovery, complications, and poor outcomes associated with full-thickness penetrating keratoplasty. This newer methodology is based on more selective transplantation of cells and is less invasive, resulting in quicker recovery and more efficacious outcomes. Contributors are world-renowned pioneers who are front and center in implementing innovative, sight-restoring techniques that continue to change the ophthalmic surgery landscape. In the first section, a solid foundation is laid with discussion of anatomy, the history of EK, clinical aspects of endothelial cell dysfunction, and clinico-pathology. Subsequent sections cover surgical procedures, special topics, surgical outcomes, complications, and future trends. Readers will gain invaluable insights from clinical pearls throughout the book, gleaned from hands-on experience by leading experts in the field of ocular surgery. Key Features: A large compendium of high-quality videos demonstrate cutting-edge approaches Richly illustrated with 400 images that further delineate anatomy and techniques Anterior segment surgery advances including Descemet's stripping endothelial keratoplasty (DSEK), predescemet's endothelial keratoplasty (PDEK), ultrathin DSEK, Descemet's membrane endothelial keratoplasty (DMEK), and trocar anterior chamber maintainer Treatment of conditions such as pseudophakic bullous keratopathy and Fuchs' dystrophy Management of complicated cases including glued IOL, cataract extraction & IOL implantation, glaucoma, failed grafts, and scarred corneas This book is a stellar resource on the surgical management of patients with corneal disease. It will benefit both trainees and practicing ophthalmologists - and is a must-have for specialists who perform corneal surgery.
