Book excerpt: Nous avons entrepris d'analyser les accidents du travail par agression ayant fait l'objet d'une déclaration, sur 3 années consécutives de 2007 à 2009 inclus. Cette étude avait pour but d'évaluer les conséquences sur la santé et sur le travail de ces agressions, ainsi que la qualité de la prise en charge, d'identifier les mesures de prévention en place ou à promouvoir. Les objectifs secondaires étaient de mettre en évidence une éventuelle sous-déclaration de ces aggressions et d'analyser la perception de la violence. Un questionnaire a été envoyé aux 96 personnes concernées par les déclarations. Suite au retour de 44 questionnaires, des entretiens semi-directifs ont été réalisés auprès de 21 personnes ayant accepté d'y participer. Sur le risque violence : il apparaît que plus d'un tiers des répondants sont exposés quotidiennement à la violence verbale au cours de leur travail. Près de 90% avaient déjà été victimes d'agressions non déclarées en accident du travail. Sur les conséquences de l'agression : 52,2% des répondants ont constaté des répercussions psychologiques initialement marquées, évoluant généralement vers la résolution. La prise en charge psychologique a été considérée insuffisante ou inexistante par 71,4%. Alors que la formation à la gestion de la violence a été jugée nécessaire par la majeure partie (90,9%) des répondants, seul un tiers (31,8%) en a bénéficié. Il apparait nécessaire de prendre en compte la violence au sein de l'hôpital, phénomène difficile à évaluer et sous-estimé. Les mesures de prévention, comme l'accès aux formations à la gestion de la violence, ainsi que la prise en charge psychologique, méritent d'être améliorées.

Dans un premier temps a été réalisée une évaluation rétrospective de la souffrance au travail parmi 123 patients pris en charge à la Consultation de pathologie professionnelle du Centre hospitalier de Cherbourg, déclarant avoir été victimes d’une situation de harcèlement moral présumé. Une étude prospective à l’aide d’un questionnaire téléphonique destiné à mettre en évidence d’éventuels facteurs pronostiques concernant le devenir professionnel de ces salariés a ensuite été réalisée. Ce travail nous a permis de mettre en évidence les conséquences importantes d’une situation de harcèlement moral au travail, tant sur la santé de la victime que sur sa vie professionnelle. Le pronostic socio-professionnel de telles situations est en effet sombre puisque, dans 92,4 % des cas, nous avons constaté que la principale conséquence de la situation de harcèlement avait été la perte de l’emploi occupé. La prise en charge des salariés en situation de souffrance du travail doit donc être réalisée de la manière la plus complète, et le médecin du travail y tient un rôle primordial.

Psychosocial Interventions for Mental and Substance Use Disorders details the reasons for the gap between what is known to be effective and current practice and offers recommendations for how best to address this gap by applying a framework that can be used to establish standards for psychosocial interventions. The framework described in Psychosocial Interventions for Mental and Substance Use Disorders can be used to chart a path toward the ultimate goal of improving the outcomes. The framework highlights the need to (1) support research to strengthen the evidence base on the efficacy and effectiveness of psychosocial interventions; (2) based on this evidence, identify the key elements that drive an intervention's effect; (3) conduct systematic reviews to inform clinical guidelines that incorporate these key elements; (4) using the findings of these systematic reviews, develop quality measures - measures of the structure, process, and outcomes of interventions; and (5) establish methods for successfully implementing and sustaining these interventions in regular practice including the training of providers of these interventions.
