Book excerpt: L'estimation de la vitesse de marche de confort (VMC) constitue le critère de référence habituel de la performance de marche après un Accident Vasculaire Cérébral. L'objectif de ce travail a été d'évaluer, avec le Locomètre, la pertinence des paramètres spatio-temporels de la marche en terme de validité et de reproductibilité, chez 20 sujets hémiplégiques. Les résultats montrent que la cadence et la longueur d'enjambée sont plus pertinentes que la VMC. Par ailleurs, l'analyse de la marche à vitesse rapide permet d'améliorer la qualité métrologique de l'ensemble des paramètres spatio-temporels, et paraît donc être une évaluation plus pertinente que la VMC chez les patients hémiplégiques. La perspective de ce travail est de compléter ce résultat par la mesure de la sensibilité au changement des paramètres.

Book excerpt: Le baro-biofeedback est une technique de rééducation consistant à révéler au patient de manière continue et instantanée des informations obtenues par des capteurs de pression. Ces informations, transmises au patient sous forme de signaux sensoriels ou sensitifs, permettent notammment de corriger l'asymétrie posturale. La récupération de la marche et des transferts étant un objectif prioritaire de la rééducation, le but de cette étude est d'évaluer les effets du baro-biofeedback sur les variables cinématiques de la marche de l'hémiplégique. La marche revêt différents aspects, son analyse cinématique consiste en l'étude des mouvements des différents segments de membres dans les trois plans de l'espace. L'étude a été menée chez 26 patients hémiplégiques; il s'agit d'un essai randomisé, avec groupe contrôlé. 13 patients ont été inclus dans le groupe dit "témoin" et recevaient cette même rééducation associée à une rééducation baro-biofeedback à l'aide de l'appareil BPM Monitor SMS Healthcare®. Les patients du groupe expérimental étaient significativement plus âgés. Les variables cinématiques de la marche ont été étudiées dès la reprise de la marche (JM) et 30 jours après (JM+30). Il s'agissait de l'amplitude articulaire des genoux et des hanches dans le plan sagittal, de l'amplitude des hanches et du bassin dans le plan frontal, de l'amplitude du bassin et des épaules dans le plan horizontal, des élévations de malléole et des EIAS sur l'axe vertical et pour terminer une analyse qualitative de la dissociation des ceintures. Les données ont été recuillies à l'aide du système 3D d'analyse du mouvement VICON 512 system®.Les résultats montrent une différence significative dans l'évolution de l'amplitude de hanche dans le plan sagittal du côté sain : dans le groupe expérimental l'amplitude diminue entre JM et JM+30 alors que dans le groupe témoin cette amplitude augmente. Par ailleurs, un effet groupe est noté pour l'amplitude des hanches dans le plan frontal du côté sain avec des valeurs inférieures dans le groupe expérimental quel que soit le temps. L'étude de la dissociation des ceintures montre que seul un patient du groupe expérimental présente ce croisement "physiologique" à JM+30. Notre étude ne semble donc pas en faveur d'une modification du schéma de marche par l'adjonction du baro-biofeedback à une rééducationn classique, cependant le petit nombre de sujets et la différence d'âge entre les deux groupes ne permettent pas d'exclure un effet positif de cette technique.
