Book excerpt: Behavior therapists often "desensitize" clients by exposing them to an anxiety-eliciting stimulus such as a phobic object. The premise is that repeated exposures will eventually reduce or extinguish the fear. This process is called therapeutic exposure. Many therapists would agree that therapeutic exposure is an effective treatment for several anxiety disorders. However, the "directness" with which therapeutic exposure should be encouraged for a client is much debated. Many feel that direct therapeutic exposure, more commonly known as flooding or implosive therapy, causes clients an excessive amount of anxiety, and may therefore produce long-lasting and harmful side effects. In response to these concerns, one well-known behavior therapy technique, systematic desensitization, was designed to expose clients to imagined fear stimuli slowly and under relaxing conditions so as not to elicit anxiety. In the first chapter of this book, we show that these concerns are based on false assumptions. When used properly, direct therapeutic exposure is not harmful. In Chapters 2 and 3, we review the literature on the process and outcome of flooding and implosive therapy from a practical, clinical perspective. This literature shows that these treatments are effective and efficient for certain psychiatric disorders. In Chapter 4, we describe the treatments we offer our clients and show how learning theory can be used to help develop the procedure and content of therapy. Chapter 5 provides case examples with transcripts of typical therapy sessions. This book can be used as a handbook for therapists or students.

Time and research have shown that the techniques of behavior ther apy have been more successful than its models have been explanatory. Yet progress has been substantial in both realms, as the following volume makes patently clear. In 1980 an inventory of this progress was catalogued at an NIMH-sponsored workshop. What both that workshop and this volume clearly show is that the behav ior therapy researcher need no longer suffer the epithet "overly simplistic." One of the major strengths of this volume is its elucidation of the complexities that now attend the study of the anxiety disorders, particularly from a behav ioral point of view. The researcher at times appears almost to be buried in a landslide of complexities regarding this topic. The concept of anxiety itself has undergone a differentiation to a level of complexity that poses problems for both the conceptualization and the treat ment of anxiety disorders. In virtually one voice, the several authors of this volume argue the multidimensional nature of anxiety. The "lump" view of anx iety has given way to the three-channel view of anxiety. An investigator's future research career could well be secured just by spending time puzzling out the significance of the low intercorrelations among the channels.

Our capacity seemingly to duplicate sights, sounds, and other sensory experiences through some form of central brain process continues to remain a mysterious, alma st miraculous skill. Because imagery is so much a private experience, experimental psychologists found it hard to measure and turned their attentian to observable behaviors that could easily be studied in ani maIs as well as in humans. Psychoanalysts and others working with the emotionally disturbed continued to take imagery informatian se riously in the form of dream reports, transferenee fantasies, and as indications of hallucinations or delusions. On the whole, however, they emphasized the maladaptive aspects of the phenomena, the dis tortions and defensiveness or the "regressive" qualities of daydreams and sequences of images. The present volume grows out of a long series of investigations by the senior author that have suggested that daydreaming and the stream of consciousness are not simply manifestations in adult life of persist ing phenomena of childhood.
