Book excerpt: Type 1 Diabetes (T1DM) is a life-long chronic illness often diagnosed during childhood or adolescence which requires individuals to adhere to a complex treatment regimen. Unfortunately, adherence is especially poor during adolescence. One factor which may influence adherence is treatment responsibility, or determining whether the parent or adolescent assumes treatment responsibility. Although there are no clear guidelines regarding when to transition responsibility, knowing whether the adolescent has the cognitive capacity or executive functions (EF) to successfully manage their treatment is essential. Parental stress has also been associated with treatment responsibility, suggesting that greater levels of parent responsibility are associated with greater levels of reported parental stress. However, most studies have failed to examine the effects of multiple forms of stress (i.e., general, parenting, and disease-related) on treatment responsibility. Further, little is known regarding how parent perception of adolescent general and disease related EF and various forms of parent stress are related to treatment responsibility. The present aimed to assess the contribution of various forms of stress and parent perception of general and diabetes related executive functioning on the transition of responsibility from parent to adolescent in adolescents with T1DM. The findings from the study underscore the role of parent reported diabetes distress as it relates to the transition of responsibility. Specifically, diabetes distress was a significant predictor of greater parent responsibility above and beyond the role of general and disease related executive functioning. Nonetheless, results suggest that parents transition greater responsibility to their adolescent when they perceive their adolescent to have a greater ability to remember tasks associated with their illness. General and perceived stress did not influence the transition of responsibility from parent to adolescent. The present study expanded previous research by examining the role that various forms of stress and parent perception of adolescent's executive function play in their decision to transition responsibility for diabetes management among adolescents with T1DM. Despite limitations within the present study, the findings suggest clinical implications and future directions for research. Specifically, future interventions should target decreasing parent diabetes distress, while also focusing interventions on improving adolescent's diabetes related memory for managing their disease. Future research should include a smaller age range and should be longitudinal in nature to better understand how the dynamic nature of the associations among various forms stress, executive functioning, and treatment responsibility may have an impact on treatment adherence and glycemic control.

Book excerpt: Treatment non-adherence for Type 1 patients; especially during adolescence, is viewed as the precursor for healthy, or unhealthy patterns of self-care in adulthood. The risk for long-term health problems such as blindness, renal failure, heart attack, lower limb amputations, and stroke are exacerbated if healthy monitoring of the condition is not adhered to consistently, and early after diagnosis. The present study sought to examine theoretical constructs which are important to overall treatment adherence among adolescents with Type 1 diabetes and their maternal caregivers. Self-efficacy and perceptions of diabetes self care between child and mother were disseminated through the inclusion of three separate measures designed for adult caregivers, and their children with Type 1 diabetes. Despite the literature which has established significant findings in the treatment domains separately, the present study did not yield significant results across the domains, but was able to offer new insights into previously held findings in the Type 1 literature. The author concludes with an analysis of the barriers in conducting research with chronically ill populations, and offers suggestions for future research in this area.

Book excerpt: This qualitative study aimed to explore the experiences of adolescents living with type 1 diabetes mellitus (T1DM) and how society perceives them. T1DM is a chronic condition characterized by abnormally high blood glucose due to lack of insulin production by the cells in the pancreas. The treatment is insulin injections or infusions, as there is no medical cure at the moment. More specifically, the study examined how the adolescents manage the illness and cope with the perception and stigmatization related to this condition. Despite the existence of research into living and coping with some chronic conditions, research on TIDM, particularly using qualitative methodologies, is limited. A review in literature also indicated insufficient studies on stigma associated with chronic conditions that have no overt symptoms. In-depth interviews were conducted with nine diverse volunteers after obtaining informed consent (five females and four males). The volunteers were aged between sixteen to nineteen years with a duration of diabetes at a minimum of two years since diagnosis. An analysis of interview results was based on ground theory methodology whereby many categories were generated. From these two core categories, which are assimilation and accommodation, stigmatization emerged. These represent the adolescents’ attempt to cope with the experience of living with TIDM and the views of society on this condition with no visible physical abnormalities. Parts of the implications of these findings confirm that medical model management needs to consider psychosocial aspects in order to facilitate adjustment to the chronic condition. Further qualitative research using a bigger sample over a longer period was recommended. The DAWN study (attitudes, wishes, and needs in diabetes) has since been carried out.

Book excerpt: The Health Belief Model (HBM), as proposed by Janz & Becker (1984), is a theory of adherence to medical recommendation. Findings on the association between the HBM and adherence and metabolic control in adolescents with Type 1 diabetes have been inconsistent (e.g. Brownlee-Duffeck et al., 1987; Patino et al., 2005). The current study aimed to investigate the relationship between parent and adolescent health beliefs and adherence/metabolic control. Further, the study aimed to identify variables that may moderate the parent/adolescent health belief and adherence/metabolic control relationship; namely, amount of parent responsibility for diabetes care, parent emotional support, and executive functioning of parent/adolescent. Findings were generally unsupportive of HBM and moderation hypotheses but patterns emerged that may be important in future research and clinical work with this population. These themes include the tendency for Caucasians and younger adolescents to have better metabolic control, and the importance of parent perception of severity and susceptibility of diabetes complications and the significant correlation both parent and adolescent executive functioning abilities have with adherence/metabolic control. Clinically, results speak to the need of further assessment and intervention targeting health beliefs and executive functioning in parents of adolescents with type 1 diabetes. Results further indicate the need for additional and more nuanced research on health beliefs in this population.

Book excerpt: Type 1 diabetes (T1DM) is a serious health condition, with the potential for significant adverse consequences. Medication adherence is crucial to survival. Poor medication adherence is a common issue among T1DM adolescents, and combined with insulin resistance related to puberty, glycemic control can be difficult to maintain. The nurse is well-placed to interact with the T1DM adolescent to assist with medication adherence issues. Review of literature revealed diabetes education, provided by a certified diabetes educator (CDE), has been insufficient to promote medication adherence. Novel technological advances hold interest for many adolescents; however, technology alone is insufficient to address medication adherence. Addition of a psychosocial intervention is needed to improve adherence. An online support group intervention for adolescents with T1DM utilizing a video-chat format is proposed to address the adherence issue. A six-month long, one-hour, bi-weekly support group is facilitated by the nurse, CDE, and/or social worker to provide emotional support, peer interactions, problem-solving, and adolescent coping strategies. The health promotion model (HPM) is applicable as a theoretical framework for the support intervention. Individual characteristics and experiences, behavior specific cognitions and effects, and behavioral outcomes are hallmarks of the HPM, and can be applied to the T1DM adolescent as he/she learns to make lifestyle choices and behaviors affecting state of health. Evaluation endpoints include change in glycosylated hemoglobin (HbA1c), self-efficacy for diabetes assessment, and pharmacy dispensing records. Group participants will be organized into groups based on demographic data, and will complete a participant satisfaction questionnaire to evaluate perception of effectiveness of the intervention. Planned dissemination of findings includes the pediatric diabetes clinic, the local institution, CDE regional meeting, and presentation at a national conference such as Pediatric Endocrinology Nursing Society.

Book excerpt: Children and adolescents living with Type 1 Diabetes Mellitus (T1DM) face many challenges in their daily lives due to the extensive care tasks that the illness requires. Adolescence is a period of development in which treatment adherence and metabolic control has been found to greatly decline. Research examining correlates of this decline in self-management has tended to focus on familial and psychosocial variables such as parental involvement and T1DM-related conflict. The period of adolescence is also marked by several changes in the development of the frontal lobes and prefrontal cortex, which are areas of the brain that are central to executive functioning abilities. The present study will examine executive functioning among adolescents with T1DM to explore its relationship with treatment adherence, metabolic control, and with measures of family involvement in the management of T1DM. Eighty four adolescents diagnosed with T1DM (ages 12-18) and their parents completed the study. Parents and adolescents completed questionnaires assessing adolescents' executive functioning, parental involvement, monitoring, and conflict. Adolescents completed neuropsychological measures assessing several aspects of their neuropsychological functioning including their executive functioning, intelligence, and memory. In addition to this, adolescents' medical records were reviewed to collect hemoglobin A1c (HbA1c) values, which represent levels of metabolic control. In general, results supported our hypotheses, in that adolescents with better developed parent and self-reported executive functions tended to display better adherence to their T1DM treatment regimen. Additionally, parent and self-reports of adolescent executive functioning were shown to be significant predictors of adherence beyond the contributions of several demographic and family functioning variables. Examination of parents' contributions to adolescents' T1DM management revealed that parental involvement was a significant moderator of the relationship between adolescents' executive functioning and treatment adherence, such that parental involvement had a larger impact for adolescents who demonstrated poorer executive functions. Overall, the study finds support for measuring executive functioning abilities in adolescents with T1DM as a potentially important contributing factor in aiding adolescents with the complex management of this illness.

By contrast, research on health compliance involving pediatric populations was less popular during the same period; however, interest in this topic -- as evidenced by the contributions to this volume -- is on the increase. Four main themes -- relating to theory, measurement, prevention, and intervention -- emerge and are interwoven among the chapters. These themes help to bind and unify the volume into a conceptual whole because although the sections are divided along thematic lines, contributors often include elements of some or all of the themes in their chapters. This state of affairs reflects the interdependence of these thematic issues and suggests how important they are for the state of the art.
