Book excerpt: In this thesis, the purpose of the first study is to further aggregate data on major depressive disorder (MDD) and dropout from treatment as it is among the most common diagnoses reported in clinical settings and one that is treatment dropout. A mean weighted Cohen's d was calculated and found a small effect size of 0.20 among 10 studies. The purpose of the second study was to explore whether certain diagnoses associate with dropout from psychotherapy in mental health facilities throughout the state of Texas. Rather than directly sampling patients/clients, this study assessed dropout data via clinician report at these agencies. In other words, dropout is examined from the perspective of the treatment providers rather than the clients. No previous study has undertaken this kind of survey on such an extensive scale. Descriptive analyses revealed that during treatment and follow-up had the most instances of dropout. Chi-squared analyses revealed that mood and behavioral disorders were highly related to dropout, as well as age and sex.

This study uses the Pekarik (1992) definition, which means that dropout is a unilateral decision by the client to abandon therapy against the counsel of the therapist. Viewing dropout and its associations with Readiness/Motivation, Psychological Mindedness and Therapeutic Alliance may help understand why the rates are so high. Using a 3 items of an online survey, this study analyses the reported intensity and timing of dropout from mental health facilities across Texas. The second part of this study was a mini-meta analysis used to interpret the association between Therapeutic Alliance and dropout in the current literature. Study 1 found that the three factors were significantly associated with each other specifically that the factors were reported being Low when dropping out. The analysis viewing the factors and their timing of dropout did not find any significant results but did show promising tendencies for future research. The mini-meta analysis was not significant but showed a mean weighted effect size of d = -.16. The effect size is in the opposite direction, showing that those who dropped out had more therapeutic alliance. Future studies would be needed to delve deeper into the literature and find more significant results.

Book excerpt: Client dropout remains a challenging issue in psychotherapy despite acceptance of mental health issues among the general public and several strategic measures designed to reduce it. Client dropout has been of particular concern in university training facilities where intern therapists experience disappointment when clients do not return for scheduled appointments and tend to associate it with inexperience and failure. This study was based on the current trend toward client-centered progress research to explore the patterns of client dropout by looking at client feedback of therapeutic alliance. The Session Rating Scale (SRS) was used in this study to measure the quality of therapeutic alliance. The SRS is a clinical tool designed to address client expectations on a session-to-session basis, thus helping to prevent client dissatisfaction with treatment. An ex post facto research design, this study used archival data from a university training center in Texas that has an accredited program for Marriage and Family Therapy (MFT). The participants were from the surrounding community who attended therapy at the center. The dependent variable was therapeutic alliance, and independent variables were client dropout and demographic variables of the client and therapist. Due to its capacity to handle multilevel data and nested data with missing data, Hierarchical Modeling procedures were used to analyze the longitudinal data of therapeutic alliance and its relationship to dropout and client/therapist variables. The results showed variability in the therapeutic alliance scores based on whether clients dropped out or continued in treatment; decreasing alliance scores predicted risk for dropout. Other covariates were found to influence therapeutic alliance were client modality and interaction of client and therapist ethnicity. Individual clients were found to form better alliances with the therapists than families or couples. Therapists' ability in relating with ethnic and culturally different groups of clients was also found to influence therapeutic alliance. Other demographic variables of the clients, such as age, gender, marital status, referral source, fees, and education, were not found to influence variability in the therapeutic alliance scores. Hierarchical Modeling was practical and useful in analyzing the multilevel and nested data in the study as it allowed for the heteroscedastic and autocorrelated nature of the data. Future research in the area of dropout is recommended using a larger sample size and data from general practices.

Book excerpt: The primary goal of this study was to provide an updated meta-analytic review of the psychotherapy dropout literature in a methodologically rigorous fashion. In addition to analyzing research that had not yet been included in a meta-analytical review of premature termination, this study analyzed variables that were not examined in the most recent meta-analysis of premature termination (Wierzbicki & Pekarik, 1993). The meta-analysis was conducted using 73 studies published from 1990 to 2006. Study descriptor variables and empirical findings were coded, and effect size was calculated using Cohen's d (Cohen, 1988). Data were analyzed according to Shadish and Haddock (1994) and Hedges and Olkin (1985). Moderator analyses were conducted to examine the potential impact of study descriptor variables according to Hedges (1994). Therapeutic alliance, treatment expectations, patient motivation, patient self-efficacy, patient hostility, and patient impulsivity were found to be moderately strong predictors of premature termination. Patient psychological variables (symptom severity and diagnosis), therapy and therapist variables (treatment length, treatment type, and therapist training), were less strongly related to premature termination, consistent with previous reviews of psychotherapy dropout literature. Simple patient demographic variables (age, gender, marital status, and level of education) were found to be weak predictors of premature termination. An additional analysis calculated a 35.26% average dropout rate across 110 studies; average dropout rate varied significantly according to country of study, study design, and primary diagnosis. The current study did not find that dropout rate varied as a function of dropout definition. Limitations of the present meta-analysis as well as directions for future research were discussed.
