Book excerpt: Recent reform proposals in the healthcare systems of Ireland and Northern Ireland have highlighted the need to improve population health and to deliver more care in the community. There are significant variations between the two healthcare systems, providing an opportunity to examine how different approaches and policies impact on a range of outcomes. The aim of this research study is to analyse the primary care systems of Ireland and Northern Ireland. In doing so, it seeks to inform the reform proposals for both systems. The analysis will focus on primary care due to the key differences between the primary care systems of Ireland and Northern Ireland, including the higher prevalence of user charges in the Irish healthcare system.

Book excerpt: The European economic crisis has been ongoing since 2008 and while austerity has spread over the continent, it has failed to revive economies. The media have played an important ideological role in presenting the policies of economic and political elites in a favourable light, even if the latter’s aim has been to shift the burden of adjustment onto citizens. This book explains how and why, using a critical political economic perspective and focusing on the case of Ireland. Throughout, Ireland is compared with contemporary and historical examples to contextualise the arguments made. The book covers the housing bubble that led to the crash, the rescue of financial institutions by the state, the role of the European institutions and the International Monetary Fund, austerity, and the possibility of leaving the eurozone for Europe’s peripheral countries. Through a systematic analysis of Ireland’s main newspapers, it is argued that the media reflect elite views and interests and downplay alternative policies that could lead to more progressive responses to the crisis.

Available in PDF, EPUB and Kindle. Book excerpt: This volume examines the provision of primary medical care in twenty-three different, predominantly developed countries, selected because of their variety and importance. An analysis is made of the problems and fundamental requirements of primary care (known as general practice in many countries). Arguments are put forward on priorities of spending and allocation of resources. Reasons for failure in the provision of adequate medical care are advanced. The role of the medical profession in education and in the quality of advice it has given to governments throughout the world are examined. Who should provide primary care? How should doctors be paid? Does the method of payment influence the quality of treatment received by the patient? Are any general lessons to be drawn from particular countries that could be of help in others? These and other questions are examined and answered.

Book excerpt: In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

It covers the practical, philosophical and scholarly issues pertinent to the delivery, financing, planning, ethics, health politics, professional and technological development, resources, and monitoring in primary health care. Contributors are from a diverse range of academic and professional backgrounds, bringing together collective expertise in mainstream medicine, nursing, allied health, Chinese medicine, health economics, administration, law, public policy, housing management, information technology and mass communications. As such, the book does not follow the common clinical practice or service-based approach found in most texts on primary care. The contents will serve as a useful reference work for policymakers, researchers, community health practitioners, health executives and higher education students.

Available in PDF, EPUB and Kindle. Book excerpt: Ask for a definition of primary care, and you are likely to hear as many answers as there are health care professionals in your survey. Primary Care fills this gap with a detailed definition already adopted by professional organizations and praised at recent conferences. This volume makes recommendations for improving primary care, building its organization, financing, infrastructure, and knowledge base--as well as developing a way of thinking and acting for primary care clinicians. Are there enough primary care doctors? Are they merely gatekeepers? Is the traditional relationship between patient and doctor outmoded? The committee draws conclusions about these and other controversies in a comprehensive and up-to-date discussion that covers The scope of primary care. Its philosophical underpinnings. Its value to the patient and the community. Its impact on cost, access, and quality. This volume discusses the needs of special populations, the role of the capitation method of payment, and more. Recommendations are offered for achieving a more multidisciplinary education for primary care clinicians. Research priorities are identified. Primary Care provides a forward-thinking view of primary care as it should be practiced in the new integrated health care delivery systems--important to health care clinicians and those who train and employ them, policymakers at all levels, health care managers, payers, and interested individuals.

Available in PDF, EPUB and Kindle. Book excerpt: Population health management is being increasingly adopted by health systems yet the importance of primary health care in influencing population mortality and the mechanisms that explain it are not well understood. Too often, primary health care is regarded as a service for minor health problems and for managing access to secondary care. This limited view is no longer tenable and it is time to be much more ambitious about the place of primary health care in health systems worldwide. In delivering and planning health care and in re-building health systems after the pandemic, practitioners and policymakers in low-, middle- and high-income countries need evidence on how primary health care affects population mortality and practical advice to effect change. Primary Health Care and Population Mortality fulfils this need. Drawing on his long experience as both a practitioner and researcher, the author Richard Baker describes how primary health is crucial to the effect of health systems on population mortality, including its potential for reducing inequalities in mortality. This accessible new book will provide invaluable information to leaders in service development and delivery, academics in primary health care and those working within international organisations that are promoting primary health care for improving population health. It will also be of practical value to general practitioners, primary health care nurses and managers and public health staff.

Available in PDF, EPUB and Kindle. Book excerpt: Economic shocks pose a threat to health and health system performance by increasing people's need for health care and making access to care more difficult - a situation compounded by cuts in public spending on health and other social services. But these negative effects can be avoided by timely public policy action. While important public policy levers lie outside the health sector, in the hands of those responsible for fiscal policy and social protection, the health system response is critical. This book looks at how health systems in Europe reacted to pressure created by the financial and economic crisis that began in 2008. Drawing on the experience of over 45 countries, the authors:' analyse health system responses to the crisis in three policy areas: public funding for the health system; health coverage; and health service planning, purchasing and delivery 'assess the impact of these responses on health systems and population health' identify policies most likely to sustain the performance of health systems facing financial pressure' explore the political economy of implementing reforms in a crisisThe book is essential reading for anyone who wants to understand the choices available to policy-makers - and the implications of failing to protect health and health-system performance - in the face of economic and other forms of shock.--

Book excerpt: The New Politics of the NHS has become established over a quarter century and five editions as the key overview of the NHS and its processes and paths of influence. This latest edition remains a clear, easy-to-read guide to often complex debates. It encompasses both the background of the evolution of the NHS since its foundation, and a completely up-to-date picture of its present and future in a more pluralistic - and possibly more financially austere - era in which deference to medical expertise is eroding and information on health and care is far more widely available. Assuming no prior knowledge of NHS politics and systems, The New Politics of the NHS focuses on management, structure, centralisation, funding, economic performance, challenges, current party political debates, interest groups and rationing, and also on the NHS's institutional and cultural continuity as a tax-funded service providing comprehensive, universal healthcare free at the point of delivery. It is a vital update for all healthcare professionals, NHS managers, policy makers and shapers, and those in special interest groups including patient advocacy organisations. From reviews of previous editions: ' A valuable insight into the discussions and decisions that determine such an important aspect of our daily life.' CANCER CARE NEWS
