Book excerpt: These guidelines have been approved by the four organizations that make up the Cooperating Parties for the ICD-10-CM: the American Hospital Association (AHA), the American Health Information Management Association (AHIMA), CMS, and NCHS. These guidelines are a set of rules that have been developed to accompany and complement the official conventions and instructions provided within the ICD-10-CM itself. The instructions and conventions of the classification take precedence over guidelines. These guidelines are based on the coding and sequencing instructions in the Tabular List and Alphabetic Index of ICD-10-CM, but provide additional instruction. Adherence to these guidelines when assigning ICD-10-CM diagnosis codes is required under the Health Insurance Portability and Accountability Act (HIPAA). The diagnosis codes (Tabular List and Alphabetic Index) have been adopted under HIPAA for all healthcare settings. A joint effort between the healthcare provider and the coder is essential to achieve complete and accurate documentation, code assignment, and reporting of diagnoses and procedures. These guidelines have been developed to assist both the healthcare provider and the coder in identifying those diagnoses that are to be reported. The importance of consistent, complete documentation in the medical record cannot be overemphasized. Without such documentation accurate coding cannot be achieved. The entire record should be reviewed to determine the specific reason for the encounter and the conditions treated.

As a consequence, the members of the Swiss Society for Astrophysics and As tronomy voted that "The Galactic Interstellar Medium" should be the subject of the 1991 course. The 21st Saas Fee Advanced Course took place in Les Diablerets from 18 to 23 March 1991, gathering together about 80 participants from all over the world, but mostly from Europe. According to a rule that has proved to lead to success, but also to chal lenge the lecturers' energy, the format of a Saas-Fee Advanced Course consists traditionally of 28 lectures of 45 minutes which take place in the morning and late afternoon, leaving ample time for discussions, self-study, hiking or skiing. Despite the inordinate work load imposed, this year's lecturers felt that the subject was sufficiently dense to increase the lecture time by 1/3! This proved judicious and left more time for questions and discussions during the lectures.

Kwong-Sang Cheng, a. k. a. one of the three Musketeers, documented the "accidental" development in writing in the Proceedings of the 1997 Pacific Rim Conference on Stellar Astrophysics (Volume 138 of the ASP Conference Series)! The meeting at Hong Kong University of Science and Technology covered three major topics: binary stars, compact stars and solar type stars. The conference was extremely successful. There was a general feeling among the participants that the conference on stellar astrophysics provided a good means to share ideas between such closely related disciplines. Unfortunately after the very successful meeting at HKST, Kwing L. Chan (another Musketeer) thought that he had already served and would not like to chair for another LOC for at least five years! After a few drinks at one of the watering holes in Wan Chai district of Hong Kong, Kwong-Sang Cheng was in very hiRh spirit and volunteered to taking on the responsibility of hosting the 51 Pacific Rim Conference at Hong Kong University in 1999.

Accordingly, they exhibit scattering, e.g., x-ray, electron, and neutron, but not the diffraction patterns characteristic of crystals. The intensity distributions from NC solids are usually transformed into radial distribution functions (RDF), but the interpretation of the RDF's is not unique. The lack of long-range order, and the non-uniqueness of the structural interpretation, have constituted the main obstacles to the usual solid state physics approach which has been so successful in dealing with crystals. As a corrolary, questions of local order and structure have frequently been de-emphasized. This monograph contains a collection of chapters many of which emphasize local-structure and chemical bonding as opposed to long-range order. Most of the chapters were chosen from talks given at the international symposium, "Structure and Bonding in Noncrystalline Solids," held in Reston, Virginia in May of 1983. Other chapters, however, were simply submitted independently of the Reston conference. Thus, this book is not a proceedings of that conference, nor was it ever intended to be. The chapters presented here range from theory of bonding and structure, to structurally oriented measurements of various kinds, e.g., ESR, Raman, to more applied chapters. Our goal was to produce a monograph that enhances understanding of the structures of NC solids.
