Book excerpt: Advances in Physiological Sciences, Volume 27: Factors Influencing Adrenergic Mechanisms in the Heart is a collection of papers presented at the 1980 satellite symposium of the 28th International Congress of Physiological Science, held in Visegrád, Hungary. This symposium covered the achievements that modify the traditional views of adrenergic regulation of cardiac muscle and coronary blood vessels. This volume is composed 18 chapters and begins with a discussion of the effect of adrenaline on some aspects of electrical and mechanical activity in the frog heart. The subsequent chapters describe the heart rate changes in neurogenic hypertension, the developmental changes of the catecholamine-induced chronotropic responses related to the blood pressure responses, and the inverse reciprocal regulation of cardiac post-synaptic a- and b-adreneceptors by thyroid hormones. Other chapters deal with the mechanism of adenosine-induced inhibition of adrenergic neurotransmission in the ventricular myocardium; the comparative morphological studies of localized exogenous adenosine in heart muscle; adenosine sensitivity of canine coronaries; and the significance of nickel ion in the coronary vascular tone regulation. These chapters are followed by discussions of the physiological and pathological effects of beta modulator release and the histological study of experimental diabetic angiopathy. The last chapters examine some aspects of myocardial noradrenaline metabolism in the ischemic heart and the activity of alpha-adrenoceptor without employing beta blocking agents.

Book excerpt: Stress-induced myocardial ischemia is a frequent manifestation of coronary heart disease, and sympathetic activation is an important precipitating and aggravating factor in such stress induced ischemia. However, the complex interplay between the sympathetic initiation of myocardial ischemia, ischemia-induced alterations in sympathetic neurotransmission, as well as changes in adrenoceptor density and post-receptor signal transduction that can occur during ischemia remains incompletely understood. Not only the activation of myocardial fJ adrenoceptors, but also the activation of coronary IX-adrenoceptors can contribute to myocar dial ischemia. However, the role of fJ-adrenoceptor-mediated increases in contractility relative to heart rate in the initiation of ischemia is not clear, and the significance of IX-adrenoceptor mediated changes in coronary vasomotor tone, as well as the responsible IX-adrenoceptor subtypes are highly controversial. Malignant arrhythmias may be triggered by both IX- and fJ-adrenergic mechanisms. Current research in this field is focussed not only on the underlying physiological and pathophysiological mechanisms, but also on clinical treatment strategies, e. g., by fJ-blockade, IX-blockade, bradycardic agents and calcium antagonists. Recent findings were presented and future research directions discussed during the 61" International Titisee Conference, held at the Schwarzwald-Hotel, Titisee, March 29-31, 1990 under the sponsorship of the Boehringer Ingelheim Foundation. Book excerpt: "The present series of experiments provided evidence that the generally accepted distinction between alpha- and beta-adrenergic receptors is not immutable. In frog hearts and rat atria 3H-phenoxybenzamine (3H- POB) blocked inotropic responses to catecholamines only at temperatures below l7°C and potentiated responses ab ove 23°C, and considerably more radioactivity was bound to the myocardium at lower temperatures. The converse was true for l4C-propranolol, and the potency of alpha- and beta-adrenergic agonists was shown to parallel the effectiveness of the blocking agents. Alkylation of alpha-adrenergic receptors by POB at l4°c prevented the appearance of beta-adrenergic receptors when the temperature was subsequently raised ta 24°C. Phentolamine prevented block by 3H-POB and considerably reduced its binding to the myocardium at low but not at high temperatures. After exposure to unlabelled POB in the presence of phentolamine at low temperature and thorough washing, exposure to 3H-POB produced a "positive label" of the adrenergic receptors, which was localized in a 20,000 g ("mitochondrial") and in a "soluble protein" fraction. Pretreatment of rats with 6-hydroxydopamine, but not with reserpine, prevented the appearance of alpha-adrenergic receptor characteristics in left atria at low temperatures; this indicated that adrenergic innervation, but probably not stores of neurotransmitter, is necessary to allow transformation of the receptors. It is suggested that alpha- and beta-adrenergic receptors may be different allosteric conformations of the same molecule." --
