Available in PDF, EPUB and Kindle. Book excerpt: The microcirculation of the gastrointestinal tract is under the control of both myogenic and metabolic regulatory systems. The myogenic mechanism contributes to basal vascular tone and the regulation of transmural pressure, while the metabolic mechanism is responsible for maintaining an appropriate balance between O2 demand and O2 delivery. In the postprandial state, hydrolytic products of food digestion elicit a hyperemia, which serves to meet the increased O2 demand of nutrient assimilation. Metabolically linked factors (e.g., tissue pO2, adenosine) are primarily responsible for this functional hyperemia. The fenestrated capillaries of the gastrointestinal mucosa are relatively permeable to small hydrolytic products of food digestion (e.g., glucose), yet restrict the transcapillary movement of larger molecules (e.g., albumin). This allows for the absorption of hydrolytic products of food digestion without compromising the oncotic pressure gradient governing transcapillary fluid movement and edema formation. The gastrointestinal microcirculation is also an important component of the mucosal defense system whose function is to prevent (and rapidly repair) inadvertent epithelial injury by potentially noxious constituents of chyme. Two pathological conditions in which the gastrointestinal circulation plays an important role are ischemia/reperfusion and chronic portal hypertension. Ischemia/reperfusion results in mucosal edema and disruption of the epithelium due, in part, to an inflammatory response (e.g., increase in capillary permeability to macromolecules and neutrophil infiltration). Chronic portal hypertension results in an increase in gastrointestinal blood flow due to an imbalance in vasodilator and vasoconstrictor influences on the microcirculation. The book reviews imaging modalities, diagnostic work up, physiologic tests, traditional open surgical techniques and novel endovascular approaches. Technical aspects of both open surgical and endovascular techniques are provided by experts in the field, with illustrations and photographs of key steps for each type of procedure. Results of epidemiologic studies and national databases are summarized, as well as large institutional experiences. An evidence-based approach is used for recommendations regarding best therapies. Diagnostic approaches including imaging and novel physiologic tests, including gastric tonometry and oxygen light spectroscopy are covered. Mesenteric Vascular Disease: Current Therapy will serve as a very useful resource for clinicians, surgeons, interventionalists, gastroenterologists and researchers dealing with and interested in mesenteric vascular diseases.

Book excerpt: This is the first comprehensive study guide covering all aspects of pediatric critical care medicine. It fills a void that exists in learning resources currently available to pediatric critical care practitioners. The major textbooks are excellent references, but do not allow concise reading on specific topics and are not intended to act as both text and study guide. There are also several handbooks available, but these are usually written for general pediatric residents and lack the advanced physiology and pathophysiology required for the higher level pediatric critical care practitioner

Book excerpt: This fourth edition of Autonomic Failure (now available in paperback) covers the many recent advances made in our understanding of the autonomic nervous system. There are 20 new chapters and extensive revisions of all other contributions. Autonomic failure, fourth edition makes diagnosis increasingly precise by fully evaluating the underlying anatomical and functional deficits, thereby allowing more effective treatment. This new edition continues to provide practitioners from a variety of fields, including neurology, cardiology, geriatric medicine, diabetology, and internal medicine, with a rational guide to aid in the recognition and management of autonomic disorders. The book starts with an updated classification of autonomic disorders and a history of the autonomic nervous system. The first two sections of the book deal with the fundamental aspects of autonomic structure, function, and integration. There are new chapters dealing with neurobiology, nerve growth factors, genetic mutations, neural and hormonal control of the cerebral circulation, innervation of the lung, and pathophysiological mechanisms causing nausea and vomiting. Advances in the clinical management of autonomic disorders are critically dependent on the bridge made between the basic and applied sciences.

Available in PDF, EPUB and Kindle. Book excerpt: The microvasculature refers to the smallest blood vessels, arterial and venous, that nurture the tissues of each organ. Apart from transport, they also contribute to the systematic regulation of the body. In everyday terminology, the microcirculation is "where the action is." Microcirculation is directly involved in such disease states as Alzheimers, inflammation, tumor growth, diabetic retinopathy, and wound healing- plus cardiovascular fitness is directly related to the formation of new capillaries in large muscles. Microvascular Research is the first book devoted exclusively to this vital systemic component of the cardiovascular system and provides up to date mini-reviews of normal functions and clinical states. The contributing authors are senior scientists with international reputation in their given disciplines. This two-volume set is a broad, interdisciplinary work that encompasses basic research and clinical applications equally. * Broad coverage of both basic and clinical aspects of microvasculature research * Contains 167 chapters from over 300 international authors * Each chapter includes key figures and annotated references
